Abstract

The article considers the physiological mechanisms of regulation of gastric acid production; the characteristic of the modern groups of drugs used in the treatment of acid-related disorders, especially proton pump inhibitors, are described. It is emphasized that the use of modern drugs provides a high level of acid control, quality of life of patients, and the possibility of treatment in outpatient settings. The ways to improve the effectiveness and safety of pharmacological methods of correction of intensity of hydrochloric acid secretion are discussed.

Abstract

The clinical course of chronic pancreatitis (CP) is often defined by its combination with other diseases. According to recent research, the combination of CP and gastroesophageal reflux disease (GERD) is not uncommon. Against the background of acute pancreatogenic pathology, symptoms of GERD are often «ignored», which is certainly not only affects the quality of treatment of these patients, but also affects the socio-economic aspects of comorbidities. Therapy of patients with combination of HP and GERD should be based on a comprehensive medical approach, including polyenzyme drugs, antisecretory agents, and other medications when appropriate.

Abstract

Diverticular disease is the most common organic pathology of the colon in patients older than 60 years. Uncomplicated (i.e. without endoscopic signs of inflammation) diverticular disease in 20-35 % of patients occurs with severe pain, flatulence and/or impaired stool that leads to reduction of quality of life of patients and requires prescribing of drugs of various pharmacological groups in addition to dietary recommendations. Standard therapy with antispasmodics is not always high effective, because patients with diverticular disease of the colon represent heterogeneous group. This requires an individual approach to the treatment. The article discusses the various groups of drugs and the potentials for their application depending on the predominance of certain symptoms.

Abstract

The paper considers the current views of the problem of functional disorders of the biliary tract in children. It presents the Rome III Consensus guidelines. Diagnostic difficulties are discussed at the nosological level. The publication gives the clinical manifestations of biliary dyskinesia, which are most important from the internist’s point of view. Particular attention is given to up-to-date technologies. The major drug groups used to correct functional disorders of the biliary tract are viewed. Some novel drugs, such as Creon, Sanguiritrin, and current probiotics are shown to have a high efficacy and a good tolerability.

Abstract

The emergence of various disorders of the gastrointestinal tract in infants (including constipation, regurgitation, crying/irritability, cramps and excessive flatulence) is a common problem and can cause numerous visits to the pediatrician. Around the world, occurrence of these disorders often leads to the replacement of infant formula due to the fact that parents or clinicians regard these symptoms as manifestations of intolerance to infant formula. As a rule, rates of physical and neuropsychological development of children are not violated. Group of pediatric gastroenterologists and pediatric allergists from Europe, Latin America, Asia and the US, has developed recommendations and practical algorithms designed to assist physicians in the management of patients with functional disorders of the gastrointestinal tract.

Abstract

The study was aimed to the comparative assessment of the effectiveness of therapy with probiotics or drugs, coordinating intestinal motility (trimebutine) in patients with irritable bowel syndrome (IBS). 121 patients aged 5 to 17 years with disorders corresponding to clinical criteria for IBS were under observation. Patients were divided into seven groups depending on the therapy. The greatest effect on the relief of IBS symptoms was achieved in the combination therapy group (trimebutine + probiotics). Among monotherapy with different probiotics, best result was obtained from the treatment with Enterol. Results of study confirm the significance of the impact on key points of «vicious circle», characteristic of IBS.

Abstract

The article presents the results of evaluation of 62 patients with chronic hepatitis C and 103 patients with hepatic cirrhosis of viral etiology. Besides clinical parameters, vegetative features were examined by analysis of heart rate variability at rest and active orthostasis. Indicators of frequency analysis of heart rate in the studied patients with chronic hepatitis C and hepatic cirrhosis of viral etiology have confirmed increase in sympathetic activity and weakening of parasympathetic activity in all parameters, with the sympathetic influence amplified in active orthostasis. Indices of regulatory systems of the autonomic nervous system (stress index and the index of autonomic regulation) are also indicated a statistically significant predominance of sympathicotonia at rest and active orthostasis, indicating serious violations of adaptation in the studied patients. The article presents the main groups of drugs that were used in the study, the properties of silymarin and indications for its use in hepatology are described in detail.

The article presents the modern classification of ulcerative colitis; latest guidelines for the diagnosis and treatment of various forms of the disease are considered. Treatment of ulcerative colitis includes dietary advice, administration of drug therapy (mesalazine, sulfasalazine, glucocorticosteroids, infliximab, azathioprine), psychosocial support, and surgery when appropriate. The choice of treatment is determined by the activity of inflammation, the volume of lesions of the colon, the presence of extra-intestinal manifestations, duration of history, efficacy and tolerability of previously received therapy, and the risk of complications of the disease.

The article is dedicated to the evaluation of the efficacy of antispasmodic drugs used in the treatment of patients with irritable bowel syndrome (IBS). According to the results of numerous studies, hyoscine butylbromide has a physiological effect on the secretory and motor functions of the gastrointestinal tract, can effectively eliminate these violations and reduces the number of relapses. The authors conclude: prolonged use of hyoscine butylbromide in the complex therapy of patients with IBS is not only safe, but also prognostically justified for these patients, because it contributes the recovery of visceral sensitivity and significantly reduces the incidence of recurrence of the disease.
