Abstract

The review is devoted to the pharmacotherapeutic approaches in the treatment of dyscirculatory encephalopathy (DEP). The modern view of the etiology and pathogenesis of DEP is considered in detail. The basic groups of drugs of choice and their advantages and disadvantages are described. The results of meta-analyses and reviews of clinical trials evaluating the efficacy and safety of drugs used commonly in DEP are presented. It is emphasized that multiple aspects in the pathogenesis of DEP should be considered in the process of choice of therapy, that makes combination of agents affecting the various links in the pathogenesis of the disease and its individual symptoms as the most promising.

Abstract

In recent years, reperfusion therapy in our country has become a part of routine clinical practice; the results of the introduction of thrombolytic therapy (TLT) in terms of safety and effectiveness are comparable to foreign data. Given the opportunity of available stroke units operating using a uniform procedure and standards of care, it is absolutely real and necessary to increase the number of TLT procedures through active public information campaigns in order to maximize the early hospitalization of patients with acute stroke in specialized units, as well as further organizational improvement of medical care for patients with stroke at all stages of care.

Abstract

The article considers the basic mechanisms of pathogenesis of acute ischemic stroke (AIS). The staging of formation of cerebral infarct is emphasized; special attention is paid to the area of ischemic penumbra, the impact on which may be the most important direction in the treatment of AIS. The sequences of key points of biological and pathophysiological processes observed in the acute and delayed brain lesions in ischemia are presented. The importance of activation of reparative and regenerative processes underlying neuroplasticity as an important mechanism for compensation of dysfunctions is emphasized. Considering the staging of pathological process, correct choice of treatment strategies, in particular neurotrophic and neuroprotective therapy in different stages of the disease, is required. The results of experimental and clinical studies of original domestic drug Cellex are presented; the potentials for the use of this drug in the complex treatment of AIS patients are discussed.

Abstract

This review describes the main issues related to the use of nebivolol in patients with arterial hypertension (AH). The features of nebivolol are discussed in detail, as it is highly selective 3rd generation β-blocker having the action on the endothelial function and stimulating the synthesis of nitric oxide. Possible mechanism of organoprotective action of the drug ids described. Evidence of antihypertensive efficacy is presented. The effects of nebivolol on the state of the target organs in arterial hypertension, the metabolic effects of nebivolol, and the potentias for the ose of the drug in patients with diabetes mellitus, obesity, and metabolic syndrome are considered.

Abstract

The article discusses the problem of the relationship between blood glucose levels and outcomes of acute coronary syndrome (ACS). Over the past decade, data on the adverse effects of hyperglycemia on the course and prognosis of ACS have been accumulated. It has been established that elevated blood glucose levels on admission are associated with high mortality both during inpatient treatment and during the first year after a heart attack regardless of the history of diabetes mellitus. ACS patients with elevated blood glucose levels should be considered as high risk group for cardiovascular complications. This is necessary not only in connection with a worse prognosis, typical for this group of patients, but also for selection of patients in need for extra attention, priority use of aggressive techniques, and use of additional methods of treatment.

Abstract

Article reviews the results of clinical trials of a new drug for the treatment of alcohol addiction «on demand». In contrast to classical approaches to the treatment of alcoholism, aimed to the achievement of total abstinence and prevention of relapses of alcohol consumption, alternative approach involves a gradual reduction in the amount of ethanol consumed by the use of the drug «on demand». Nalmefen is considered as the drug of choice for an alternative approach to the treatment of alcoholism. The article presented the evidence confirming that nalmefene is an effective drug for reduction of alcohol consumption in patients with alcohol addiction who do not want to completely stop the consumption of ethanol.

Abstract

Cardiovascular diseases are the leading cause of death in the Russia. Among them, myocardial infarction (MI) takes important place in both men and women. The article presents the results of evaluation of the clinical course and laboratory functional parameters in 1403 male and female patients (43.98% of women) of different age groups in the period of acute myocardial infarction. It was found that women with MI were significantly older than men with IM. Female patients had more severe somatic status including comorbid conditions such as diabetes mellitus, dyslipidemia, disorders of lipid metabolism, arterial hypertension. Complicated course of myocardial infarction was more frequently observed in women. In-hospital mortality among men and women has been increased with age.

Opyt primeneniya azilsartana medoksomila (Edarbi) u patsientov s arterial'noy gipertenziey v ambulatornoy praktike

Abstract

The article presents the results of the clinical evaluation of the the efficacy and tolerability of monotherapy with azilsartan medoxomil in 120 patients with I-II degree arterial hypertension. It was shown that azilsartan medoxomil used as monotherapy for 3 months at doses of 40 and 80 mg is an effective antihypertensive agent. It significantly improves the stiffness of the elastic and mixed type arteries, which helps to reduce the rigidity of the vascular wall, prevents vascular remodeling and development of atherosclerotic lesions. The drug was well tolerated by patients. There was a statistically significant decrease in blood glucose levels during treatment with azilsartan medoxomil. It is shown that azilsartan has nephroprotective properties, and significantly reduces the microalbuminuria. As a result, the drug significantly improves the quality of life of patients and reduces the level of anxiety according the Hamilton scale score.

Abstract

The study was aimed to the evaluation of the effectiveness of combined antihypertensive pharmacotherapy, including antidepressant or dihydropyridine calcium antagonist, in patients with uncontrolled arterial hypertension and depressive disorders. Patients have received an angiotensin-converting enzyme inhibitor perindopril, thiazide diuretic indapamide SR in combination with an antidepressant escitalopram (n=78) or calcium antagonist amlodipine (n=82) for 24 weeks. According to the analysis of results of 24-hour blood pressure monitoring, it was revealed that the therapy including antidepressant provided greater improvement in the main indicators of 24-hour blood pressure monitoring, as well as normalization of diurnal blood pressure profile in a larger number of patients compared to therapy with the calcium antagonist. In addition, the prescription of antidepressants contributed to a significant reduction of severity of depressive disorders.

Abstract

Masked arterial hypertension (MAH) is often observed in young people who have no clinical signs of cardiovascular disease, and is accompanied by end-organ damages to the same extent as a stable arterial hypertension (SAH). For the diagnosis or exclusion of MAH in individuals without obvious signs of arterial hypertension, a sensitive, specific and cost-effective test is required. The present study has evaluated the hypertensive reaction to 30-second arbitrary breath-holding (breath-holding test) in 269 young subjects with no evidence of cardiovascular diseases. Of 269 normotensive subjects with office BP <140/90 mm Hgб53 had a positive test results, and 34 (12.6%) were diagnosed with MAH confirmed by positive results of 24-hour blood pressure monitoring. None of the 201 people with negative results of breath-holding test had MAH as confirmed by 24-hour blood pressure monitoring. The results of the pressor response to the breath-holding test and 24-hour blood pressure monitoring were in line in 93% of cases, and false-negative test results were absent. Breath-holding test can be used effectively to exclude MAH in healthy individuals and to identify individuals with MAH who require further examination and observation.

Limitations and contraindications for the use of certain types of local anesthetics and risk of komedikatsiya in hypertensive patients are discussed. The algorithm of emergency care during exacerbation of AH and hypertensive crisis before the arrival of ambulance team is presented.

Abstract

In acute and chronic back pain, it is advisable to administrate B vitamins in addition to the therapy with non-steroidal anti-inflammatory drugs (NSAIDs) and muscle relaxants: in acute back pain - an injectable form of the drug Milgamma, in chronic back pain - tablet forms (Milgamma compositum); this allows to achieve pain relief more quickly, reduce the dose and duration of treatment with NSAIDs. In case of presence of neuropathic component (lightning, burning pain) in acute and chronic back pain, it is necessary to add antidepressants (amitriptyline) and anticonvulsants (gabapentinoids - gabapentin or pregabalin) to the treatment.

The presence of cognitive impairment after acute ischemic attacks not only impairs the quality of life of the patient and his relatives, but also is associated with less effective neurorehabilitation. Therefore, early assessment of cognitive status (assuming clear consciousness) should be performed and therapy should be started as early as possible. As the drug of choice for post-stroke cognitive disorders, reversible NMDA receptors blockers can be recommended (Akatinol Memantine); their use leads to signficant cognitive improvement and regression of the degree of disability in general.
