Antibiotiki v dermatovenerologicheskoy praktike

Abstract

The article discusses in detail the principles of rational choice of antimicrobials in dermatovenerology, its objectives and risk factors. The data on the effectiveness of macrolides are presented. Particular attention is paid to roxithromycin (Esparoxy), which combines a wide spectrum of antimicrobial activity, good pharmacokinetic properties, favorable tolerability profile and usability. The results of clinical trials indicating high efficacy and safety of drug in bacterial infections of various localization, including urogenital tract and skin, are presented. The article considers data on health-care action of Balzamed preparations, actively restoring the barrier function of the skin and preventing the development of infectious complications.

Abstract

Hereditary angioedema (HAE) is one of the orphan diseases, presented in their - register defect of the complement system (D 84.1). HAE is a potentially life-threatening, genetic disease associated with deficiency or reduction in the function of C1 inhibitor (C1INH), characterized by recurrent swelling of the deep layers of the dermis of different localization. Currently, the issues of underdiagnosis of the disease, associated with primary immunodeficiency, are very urgent. For the first time, pediatricians, general practitioners, surgeons, otolaryngologists are faced with clinical manifestations of this disease. The article presents the principles of treatment of the HAE. The organizational and legal aspects of care for patients with this disease are discussed.

Abstract

Treatment of atopic dermatitis (AD) is based on arresting two cardinal signs of the disease - dryness and itching of the skin. For this purpose, patients requires routinely use of emollients and topical anti-inflammatory drugs - corticosteroids and/or calcineurin inhibitors. However, there are studies strongly supporting the presence of subclinical inflammation, accompanied by disturbances of skin barrier function, cytokine production and lymphocyte infiltration. The new strategy of therapy - proactive therapy - is aimed to achieving control and maintenance of remission of atopic dermatitis. This will allow to achieve control of AD faster and to reduce the risk of recurrence of the disease.

Abstract

The article highlights the issues of prevalence of allergic conjunctivitis (AC); the current classification of AC according to the form, mechanisms of development, severity and stage of the course is presented. The clinical manifestations of the disease are discussed in detail. Modern methods of diagnosis and differential diagnosis of different clinical forms of eye diseases are considered. Modern principles of therapy and prevention of AC are presented. The main types of therapy AC include non-pharmacological treatment, pharmacotherapy, and allergen-specific immunotherapy. Among the pharmacological treatments, antimediator drugs are widely used, primarily H 1-receptor antagonists, as well as mast cell stabilizers, and anti-inflammatory agents - cromoglicic acid and glucocorticosteroids.

Kombinirovannaya tsitostaticheskaya i angioprotektivnaya terapiya bol'nykh psoriazom

Abstract

The article presents the results of evalution of the effects of combined cytostatic therapy and sulodexide on the course of dermatosis and indicators of endothelial dysfunction in 125 patients with psoriasis taking into account comorbid conditions. Before therapy in psoriatic patients significant decrease in nitric oxide and increased endothelin-1 and vascular endothelial growth factor in serum were revealed. Among patients who received cytotoxic therapy, the normalization of these parameters was not observed; in patients who received adjuvant therapy with sulodexide, there was a trend to increased levels of nitric oxide, a decrease in the content of endothelin-1 and vascular endothelial growth factor. Thus, the inclusion of sulodexide in the treatment of patients with psoriasis leads to a more rapid regression of cutaneous process and normalization of vascular component of the pathogenesis of dermatosis.

Opyt primeneniya shampunya klobetazola propionata pri psoriaze volosistoy chasti golovy

Abstract

The efficacy and safety of the use of 0.05% clobetasol propionate shampoo in patients with psoriasis of the scalp in the acute phase. The effectiveness of the therapy was evaluated in 60 adult patients on 0, 7, 14, 21, and 28th days using a dynamic calculation of mPASI, TSS indices, and the intensity of itching. The safety of therapy was assessed by the frequency of side effects. It was shown that a daily 15-minute application of clobetasol propionate shampoo for 28 days promotes regression of psoriatic rash of the scalp, decrease of lesion area, as evidenced by a statistically significant change of dermatological mPASI, TSS indices and itching. Distinct therapeutic effect and absence of side effects have contributed to the high adherence to topical therapy of patients with psoriasis of the scalp.

Abstract

The article presents the results of evaluation of the effectiveness of the drug Belara (Ethinylestradiol+Chlormadinone) in combination therapy of patients with papulopustular acne. Efficacy was evaluated according to the data of clinical, functional and ultrasonographic diagnostics of skin condition. Based on the assessment of the dynamics of morphological changes of the skin by confocal microscopy in vivo, positive structural changes of the dermal collagen matrix during therapy were revealed.

Abstract

The authors describe two cases of toxic epidermal necrolysis (Lyell's syndrome): in 25 years-old man after the use of paracetamol and in 66 years-old woman after the application of Maninil. The basic principles of treatment of this category of patients are discussed in detail. SCORTEN scale allowing to predict mortality in this disease is presented. The importance of prompt diagnosis and appropriate treatment initiated for a good prognosis is emphasized.
