Abstract

Prevention of gastric cancer is one of the main direction in the management of patients with Helicobacter pylori-associated diseases. Due to increasing resistance of H. pylori to antibiotics, quadrupletherapy with bismuth and concomitant therapy play central stage in the treatment of infection. Given the low resistance of H. pylori to clarithromycin, traditional triple therapy keeps meaning in Russia. Now it is proved that there are a lot of different microorganisms in the stomach that can influence the development of pathology. The use of probiotics helps to reduce the frequency of side effects and increases the effectiveness of H. pylori eradication.

Abstract

To date, it is known that not only chronic viral hepatitis, but also non-alcoholic fatty liver disease (NAFLD) are a significant risk factors for the development of liver cirrhosis. Since the clinical picture of NAFLD is asymptomatic, its treatment should first of all be directed to factors that contribute to its development and progression, correction of associated metabolic disorders, and a reduction in the risk for developing cardiovascular complications.

PRENOSOLOGICAL PREVENTION OF INTERNAL DISEASES

Abstract

The significance of prenosological approach to the prevention of many internal diseases is represented. Attention is focused on the essential preventive orientation of this approach, which prevents the emergence of many diseases. Prenosological approach can directly reduce incidence and mortality from many diseases. There are two main range of diseases (intestinal and extra intestinal pathology), which may be prevented using prenosological approach. Attention is paid to chronophysiological aspect of prenosological approach, allowing elimination of the functional deviations from circadian intestine rhythm. Early recovery of the regularity of the circadian rhythm of the intestine can help prevention of the occurrence of the intestinal pathology: ulcerative colitis and Crohn's disease, diverticular and hemorroid diseases, polyps, colon cancer, and colorectal cancer. Prenosological approach also aims to prevent the emergence of the extra intestinal pathology: obesity and metabolic syndrome, hypertension and atherosclerosis, cholelithiasis and varicose veins.

GASTRIC MICROBIOTA AND POTENTIALS OF PROBIOTICS IN THE ERADICATION OF H. PYLORI

Abstract

Gastric microbiota is much more diverse than previously thought; there are 5 predominant major types of microbes among gastric bacterial community: Proteobacteria, Firmicutes, Bacteroidetes, Actinobacteria, and Fusobacteria. In patients infected with Helicobacter pylori, there was a significant decrease in the number of bacterial types, especially the number of Bacteroidetes, except for the Proteobacteria. H. pylori creates conditions and certain niches, which are filled with other microbes in the process of progressing atrophy. Reducing microbial diversity may predispose to the development of gastric cancer. A gradual shift in the profile of the gastric microbiota from non-atrophic gastritis to intestinal metaplasia, and then to intestinal type of gastric cancer was observed. Taking into account the possibility of safe and comprehensive impact on the composition of the gastric microbiota, and presence of immunomodulatory effects, probiotics may be promising direction of therapy. Monotherapy with Lactobacillus reuteri (Helinorm) had no significant difference in achieving eradication of H. pylori compared with the standard scheme, but led to a significant decrease in inflammatory changes in the gastric mucosa.

CLINICAL AND PATHOGENETIC PARALLELS GASTROESOPHAGEAL REFLUX DISEASE AND OBESITY

Abstract

Clinical and pathogenetic features of gastroesophageal reflux disease (GERD) with concomitant obesity are presented. In abdominal obesity, an important role is played by neurohumoral factors, including peptide hormones leptin and ghrelin. The development of GERD in obesity is mediated by a number of pathophysiological mechanisms, such as high intra-abdominal pressure and increased gastroesophageal gradient, hypotension of the lower esophageal sphincter, and increased frequency of its spontaneous relaxation. In addition, a hiatal hernia occurs more often in obesity than in the general population. Pathological forms of eating behavior cause not only the growth of body weight, but also the formation of GERD symptoms. Treatment of comorbid patients with GERD and obesity requires the therapy of both diseases. Such patients may have low sensitivity to acid-suppressive therapy. Possible reasons of this phenomenon and ways to overcoming refractoriness are considered.

Abstract

The article presents the results of evaluation of modern epidemiological and clinical and laboratory features of campylobacteriosis. The primary transmission of campylobacteriosis with improper thermal and hygienic processing of chicken meat is shown. A comparable frequency of detection of severe forms of the disease in elderly and young patients is established, as well as a reduction in the severity of infection with co-infection with other intestinal pathogens. The high efficacy of macrolide antibiotics in the therapy of campylobacteriosis is discussed.

Abstract

The problem of hemorrhagic complications of duodenal ulcer disease (DUD) remains relevant. The DUD patients with history of blee-ding were different from those with uncomplicated disease by age and gender characteristics, as well as hypercoagulable changes in blood plasma that correlated with the frequency and duration of hemorrhagic episodes. In patients with uncomplicated DUD and DUD with a history of bleeding, no differences in the histological and microhemocirculatory parameters of the gastroduodenal mucosa were observed. Secondary prophylaxis of hemorrhagic complications in DUD should include not only control of the infection of Helicobacter pylori, the use of non-steroidal anti-inflammatory drugs and concomitant pathology, but also mandatory monitoring and correction of plasma hemostasis.

Abstract

Combined pathology of the oral cavity and internal organs plays an important role in modern practical medicine of doctors of the stomatological and gastroenterological profiles, because oral cavity has a close anatomical and pathogenetic connection with the organs of the gastrointestinal tract. Recently, acid-related diseases occupy a significant place among diseases of the digestive tract, a classic example of which is gastroesophageal reflux disease (GERD). Coordination dysfunction of the lower esophageal sphincter underlies the pathogenesis of GERD. The constant reflux of aggressive contents from the stomach into the esophagus has a negative effect on the mechanisms of the development of inflammatory processes in the periodontal tissues and oral mucous membrane, leading to the destruction of hard tooth tissues.

Abstract

The article presents modern aspects of pathogenetic and clinical interrelation of erosive and ulcerative lesions of the gastroduodenal zone, dyspeptic syndrome, periodontitis and Helicobacter pylori infection. It is concluded that the data accumulated to date do not give grounds for considering H. pylori infection as the only etiological factor for the combined pathology of the oral cavity and gastrointestinal tract. The results of the study aimed to the evaluation of the efficacy of eradication schemes involving macrolides and bismuth in the treatment of patients with H. pylori-associated pathology of the gastroduodenal zone are discussed, and treatment regimens for patients with functional dyspepsia syndrome are analyzed.

Abstract

The article considers the issues of the diagnosis of exocrine pancreatic insufficiency in chronic pancreatitis with focus on nutritional status (NS). Modern views on methods of diagnostics of functional pancreatic insufficiency are presented, their diagnostic specificity and accuracy are discussed. An evaluation of the changes in the anthropometric and laboratory parameters of NS (water- and fat-soluble vitamins, proteins, lipids, minerals and trace elements in blood serum) has been performed. It has been shown that these parameters can be used to assess the likelihood of exocrine insufficiency, as well as to monitor enzyme replacement therapy and NS correction.

SAFETY OF ERADICATION THERAPY OF HELICOBACTER PYLORI INFECTION: SYSTEMATIZATION OF LITERATURE DATA

Abstract

The article systematizes literature data on the safety of eradication therapy (ET) of Helicobacter pylori infection. Data on the individual safety profile and tolerability of antibacterial drugs included in the ET regimens are presented. It is shown that the risk of development of side effects with the use of ET is 24% on average and varies depending on the specific treatment regimen: triple therapy with clarithromycin (21-24%), triple therapy with metronidazole (30%), bismuth-containing quadrupletherapy (23%), quadrupletherapy therapy without bismuth preparations (24%), sequential therapy (22%), triple therapy with levofloxacin (15-27%), hybrid therapy (26-32.9%). The factors that increase the risk of side effects include the duration of the course of therapy, incorrect dosage of drugs, the presence of concomitant liver pathology, including cirrhosis. A significant reduction in the incidence of side effects associated with ET can be achieved only with the use of adjuvant therapy with the probiotics.
