Prevention and correction of age-related changes of the skin

Abstract

The review is devoted to the analysis of modern data on regressive processes in the skin associated with chronological aging and photoaging. Morphological changes in the skin occurring at the age of 35, 40 and 50 years are discussed. The need for early prevention of aging, aimed at increasing the elasticity of the skin, its oxygenation and nutrition, is noted. The authors’ own experience with the use of a professional line of cosmetological anti-aging products is presented. It is concluded that it is possible to recommend the use of agents containing low-molecular hyaluronic acid as preventive measures that allow delaying regressive skin changes.

Algorithm for topical combined therapy of otitis externa

Abstract

The article considers the modern concepts of the etiology, pathogenesis and treatment of otitis externa. The authors discuss the most frequent causes of the development of diseases of the external ear, and the main nosological forms. The role of individual microorganisms in the genesis of the disease, the relationship of the type of pathogen, its biological properties and the nature of the course of inflammation are described in detail. The authors consider the possibilities of therapy of otitis externa taking into account the expected type of pathogen. The prospects for the use of topical therapy, the place of combined drugs in the treatment of acute and chronic diseases of the external ear are discussed. Authors'own clinical experience of the effective use of a combined preparation containing gentamycin, betamethasone and clotrimazole, in the form of ointment and cream is presented. The authors conclude that the topical therapy of otitis externa with combined preparation containing gentamycin, betamethasone and clotrimazole is possible.

Modern opportunities of treatment of genital warts

The article considers the questions of etiology, pathogenesis, epidemiology, clinical picture and prevention of genital warts. The information on the main methods of treatment, their advantages and disadvantages, possible complications is analyzed in detail. The authors provide their own data on the treatment of 40 patients with genital warts using of a domestic drug containing interferon alpha-2b+taurine+benzocaine, and the subsequent destruction of anogenital warts. The effectiveness of therapy was 80%.

Efficacy and safety of application of topical β-adrenoblocator for treatment of juvenile hemangioma

Abstract

Juvenile hemangiomas (JH) are the most common tumors of childhood, which, according to various researchers, occur in 3-12% of newborns. A new approach to the pharmacotherapy of JH is based on the effectiveness of the β-adrenergic blocker (β-AB timolol), and topical therapy of JH with β-AB appears the most relevant method of treatment of focal surface hemangiomas. Objective: to evaluate the properties of timolol in vitro, its clinical efficacy and safety in topical antiangiogenic therapy in children with superficial focal JH. Methods. Cytotoxicity, the effect of timolol on metabolism and active migration of cells in vitro (LDG test, WST test, cell migration and restoration of confluency of monolayer of cell culture) were evaluaeted. The clinical efficacy and safety of the use of β-AB in the treatment of children with superficial JH (n=56) was assessed. Results. The experimental studies in vitro indicate the absence of a negative effect of timolol on the viability, migration and mobility of endothelial cells, fibroblasts and keratinocytes. The results of the studies indicate safety and high clinical efficacy: recovery was observed in 87.5%, a significant improvement - in 7.1%, and effect of treatment was absent in 5.3%. Side effects in the treatment of children (cases of hypoglycemia and heart conduction disorders) were not detected. Conclusion. The results of the study make it possible to recommend the widespread introduction of active anti-angiogenic therapy with non-selective ß-AB timolol for surface JHs.

Application of 0.1% methylprednisolone aceponate in the therapy of atopic dermatitis in children of the first year of life

Abstract

The article presents modern ideas about the skin features of children of the first year of life, and discusses factors that increase the risk of atopic dermatitis (AD). Objective: to evaluate the effectiveness and safety of the use of the methylprednisolone aceponate emulsion for children of the first year of life suffering from AD. Methods. The emulsion was applied to the inflammation sites of 26 patients of the first year of life who suffered from AD. There were 14 boys and 12 girls aged 4 to 12 months. All patients had erythematous-squamous foci of lesions on the cheeks with elements of exudation, 18 patients had lesions to the skin of the extremities and the buttocks, and the appearance of scaly crusts on the scalp was note in 6 patients. Results. The results obtained confirm the previously described high therapeutic efficacy and safety of topical therapy using the methylprednisolone aceponate emulsion for children of the first year of life. The course of treatment contributed to the rapid regress of the inflammatory phenomena, the restoration of night sleep, the elimination of itching and anxiety. There were no side effects. Conclusion. The efficacy and safety of topical application of 0.1% methylprednisolone aceponate in children of the first year of life, including on vulnerable areas of the skin, has been demonstrated.

Urolenital chlamidiosis in men: approaches to therapy

Abstract

In order to evaluate the efficacy of clarithromycin in the therapy of urogenital chlamydiosis, 39 male patients were examined. In 26 of them, chlamydiosis occurred in acute form, in 13 men, a chronic course with duration of up to 2.6 years was diagnosed. Patients were assigned to receive clarithromycin at a dose of 500 mg twice a day for 10 days. 4 weeks after the end of treatment, the control of cure was carried out by PCR; and the eradication of the pathogen was noted in all patients. The results obtained make it possible to recommend clarithromycin as an effective treatment for chronic urogenital chlamydiosis. The immunomodulating properties of clarithromycin, proven in a number of studies, allow to reduce the dose of immunomodulating drugs or exclude them from the treatment regimen, which in turn makes it possible to increase the compliance with therapy by reducing the cost of treatment and the number of drugs used.

Features of the management of patients with exacerbation of atopic dermatitis against the background of pregnancy

Abstract

Objective: to evaluate the effecacy, safety and tolerability of a series of Neotanine preparations in pregnant women with exacerbation of atopic dermatitis (AD). Methods. 21 pregnant women with exacerbation of AD at the age of 20 to 40 years were examined at the gestation age of 8 to 30 weeks. The diagnosis was confirmed based on data of medical history and clinical picture of the disease. The examination was carried out 5 times for 14-16 days, therapy was prescribed by 2 stages. At the first stage (1-5 days), Neotanine spray or lotion (suspension) was prescribed 3-4 times a day until the elements were completely dried. At this stage, the patients were divided into 2 groups. Patients of the first group (11 persons) were prescribed Neotanine lotion; the use of Neotanine spray was recommended for patients of the second group (10 persons). At the second stage, all patients were prescribed Neotanine cream until the disappearance of the skin manifestations. Results. After the therapy, almost all patients managed to achieve positive results. Conclusion. The results of this study allow to recommend the Neotanin preparations as a safe and effective means that can be used in the treatment of pregnant women suffering from AD.

Halo phenomenon: literature review and rare clinical cases

Abstract

Halo phenomenon is described as a rim of depigmentation around a variety of skin lesions with different histogenesis. Most frequently it has been reported in cases of melanocytic nevi with an estimated frequency of 1%. Focal depigmentation of hair (leukotrichia, poliosis) represents the rare clinical presentations of halo phenomenon and can be the only clinical sign of halo nevi. A case-report of halo nevus, located on the scalp and manifested by local leukotrichia, as well as report of halo phenomenon around the nevus sebaceous of Jadassohn are described in the article. A review of the literature on halo phenomenon is given in the article, differential diagnosis of leukotrichia is discussed.

Dermatoscopy of rare dermatoses: pitted keratolysis

Abstract

Pitted keratolysis (PKL) is a special form of bacterial skin lesion, which is almost not accompanied by an inflammatory response under normal conditions. Due to the low prevalence, the diagnosis of PKL is difficult. At the same time, it can be accompanied by itching, unpleasant odor and hyperhidrosis of the feet, which adversely affects the quality of life of the patient. A clinical case of PKL in a high-risk patient is presented; PKL had a classical clinical picture but was not immediately recognized. Dermatoscopy conducted to exclude oncology of the skin is of particular interest. Dermatoscopic observations in PKL are occasional, and vascular changes in the dermatoscopic pattern were not previously described. The presented observation raises the question of the possibility of an inflammatory reaction in PKL and the specificity of the dermatoscopic picture of such inflammation.

Giant condyloma Buske-Levenstein: clinical case

Abstract

To date, the most common viral infection of the genital tract is the human papillomavirus (HPV); its 13 types possess high oncogenic risk. Despite the fact that HPV-6 and HPV-11 are classified as viruses with low oncogenic risk, they can cause severe damage up premalignant and malignant neoplasms of anogenital localization. Authors presents their own clinical observation of the giant condyloma Buschke-Levenshtein in a 31-year-old man with a burdened history of lymphangitis, lymphedema of the upper and lower limbs and lymphadenopathy, which for several years made it difficult to conduct destructive surgical treatment. The patient underwent extensive surgery with total excision of giant condyloma followed by administration of imiquimod 5% cream after complete postoperative recovery of the skin.

Breaking of the skin barrier and products of medicinal cosmetics for skin care in atopic dermatitis in children

Abstract

Atopic dermatitis (AD) is one of the most frequent skin diseases in childhood; its development is determined by the influence of genetic factors and the involvement of the innate and adaptive immune system in the formation of the inflammatory process in the skin. In recent years, great importance is attached to reducing the barrier function of the skin in the pathogenesis of AD. The development of structural and functional disorders in the epidermal barrier contributes to a more severe course of AD, a decrease in the quality of life of the child, and is a risk factor for bronchial asthma. Restoring the skin barrier using nourishing and moisturizing agents can help improve the effectiveness of AD treatment. The article highlights modern ideas about the role of the damaged skin barrier in the pathogenesis of AD, and presents the overview of modern products of medicinal cosmetics for the atopic skin care in children.
