Abstract

Cardiac amyloidosis (CA) is one of the most severe manifestations of systemic amyloidosis. The current classification of amyloidosis is based on differences of amyloidogenic precursor proteins. To date, there are about 30 such proteins; therefore, there are about 30 forms of amyloidosis. The article discusses the morphology and pathogenesis of amyloidosis, disturbances of cardiac function, and prognosis depending on the variant of amyloidosis, cardiac disorders, and blood levels of markers of cardiomyocyte damage, as well as depending on the presence and severity of extracardiac manifestations of amyloidosis. Treatment of systemic amyloidosis depending on the variant of the disease is presented.

Abstract

The article discusses the strategy for the prevention and treatment of cardiovascular diseases in elderly patients. Conventionally, cardiovascular prevention includes lifestyle adjustments (control of physical inactivity, smoking cessation, dietary measures) and treatment of arterial hypertension (AH), dyslipidemia, and diabetes mellitus. The special attention is paid to the therapeutic approach to the treatment of arterial hypertension in elderly patients; an algorithm for management of elderly hypertensive patients with comorbid conditions is presented.

The results of the examination of drug Cortexin, a complex of polypeptides of the cerebral cortex of cattle, are presented. It is shown that Cortexin reduces cerebral and focal symptoms, improves clinical status and performance of daily physical activity in ischemic stroke.

Abstract

The article considers the modern approaches to the treatment of patients after ischemic stroke (IS). Improving the effectiveness of rehabilitation can be achieved through administration of drugs with neuroprotective and neurotrophic actions, which include Cellex, a tissue-specific protein-peptide complex of fetal tissue of farm animals. The drug was clinically tested and currently is registered as a treatment for patients with acute cerebrovascular diseases as a part of combined therapy in acute and early periods of rehabilitation. Preliminary studies have demonstrated good tolerability of Cellex, and its good compatibility with other drugs.

Abstract

The article presents the diagnostic algorithm, which involves the use of computer and magnetic resonance tomography in patients with suspected acute cerebrovascular disease (CVD). The principles of basic and specific therapy for CVD are discussed. Particular attention is given to primary and secondary neuroprotection. The choice of neuroprotective drugs depends on the level of lesion (brainstem, subcortical and hemispheric) and deficit of neurotransmitters at a particular stage of the process (acute, subacute, rehabilitation). Comprehensive approach to the treatment of ischemic CVD taking into account the stage of disease and predominant effects of various drugs allows to personalize neuroprotective therapy and, consequently, improve the prognosis of life and rehabilitation of patients.

Abstract

Migraine represents a neurologic disease which is characterized by repetitive attacks of the severe headache and specific accompanying symptoms. In spite of the fact that migraine doesn't threaten life of patients, this disease leads to the reduction in the quality of life. As opposed to many other neurologic diseases, diagnosis of migraine does not require additional methods of examination (diagnostics is based on the analysis of the anamnesis, complaints and physical examination of patient). It is possible to select three main objectives in the treatment of migraine: prevention of attacks, their treatment and prevention. One of effective drugs for the relief of migraine attacks is combined preparation Nomigren containing 0,75 mg of ergotamine tartrate, 80 mg of caffeine, 200 mg of propyphenazone, 20 mg of mecloxamine citrate and 25 mg camilofine chloride. Active substances of Nomigren affect different links of migraine pathogenesis. Due to synergy effect of the active substances, the summary effect is reached against the background of use of smaller doses of different components that reduces probability of development of side effects.

Abstract

The study was aimed to determine the role of testosterone deficiency in the development of insulin resistance in men with type 2 diabetes mellitus (DM2) and coronary artery disease (CAD). Patients with DM2, CAD and both diseases had a severe hypotestosteronemia. It was established that the decrease in testosterone levels plays a role in the development of atherogenic dyslipidemia. Severe insulin resistance accompanied by compensatory hyperinsulinemia was revealed in all groups of patients. Hyperleptinemia was detected in patients with DM2, CAD and their combination. Atherogenic dyslipidemia was identified in all study groups.

Abstract

The results of treatment of 30 patients (mean age, 16.2 years) with myofascial pain syndrome using transdermal therapeutic system Versatis* were analyzed. The findings suggest the effectiveness of Versatis for adolescents after 10 days of application. Versatis significantly reduces pain intensity, increases motor activity of patients, reduces the impact of pain on the patient’s daily life, increases self-control, and reduces the emotional and affective support for the pain.

Abstract

Degenerative diseases of the spine are one of the most pressing problems of clinical medicine. Our own previous systematic reviews of the molecular mechanisms revealed the importance of magnesium in a number of physiological processes - regulation of excitability of neurons, vasodilation, maintenance of the structure and function of connective tissue. Magnesium deficiency significantly aggravates the clinical manifestations of degenerative spine disease, leading to the increased and chronic pain. The effects of therapy with magnesium orotate (MO) in the course of rehabilitation of 60 patients after operation for excision of hernia of intervertebral disk were evaluated. Patients were randomized into two groups: 30 patients were treated with MO in combination with standard therapy, 30 - only the standard therapy for 4 months. Patients treated with MO in the complex therapy were characterized by a statistically significant reduction in clinical signs of magnesium deficiency: the weakening of seizures (odds ratio [OR], 0.2, P = 0.038), reduction of symptoms of arrhythmia (OR, 0.1, P = 0.01); sleep became calmer (OR, 0.3, P= 0.045), and blood pressure was normalized. Significant decrease of OSWESTRY and Visual Analog Scale pain scores was an important result in patients treated with the MO. Thus, the use of MO (drug Magnerot ®) in the combined therapy provides more effective rehabilitation of patients after excision of hernia of intervertebral disk.

Abstract

Cardiocerebral syndrome includes violations of the functional activity of the central nervous system, which are based on the change in the cerebral hemodynamics. The article presents the observation of complex heart rhythm disorders in patient with coronary heart disease. This clinical case demonstrates that changes in cardiac output arising from cardiac arrhythmias affect the activity of brain structures, causing a decline in their functional activity that manifested in paroxysmal slow waves following the episode of cardiac arrhythmia.
