The article considers the features of depression in old- and elderly patients, the prevalence of which reaches 30%. It is emphasized that late-life depressions in the majority of cases have «somatization» nature, i.e., manifest mainly with «somatic» and not «emotional» symptoms. The relationship of depression with such age-related older diseases as stroke, Parkinson's disease, Alzheimer's disease, chronic pain syndromes, cognitive impairment is discussed. The principles of diagnosis of late-life depression, as well as approaches to its treatment, including using Velaxin (venlafaxine), are presented.

However, following the ancient principle of «treat not a disease, but the patient», the individualized approaches to diagnosis and treatment of this non-specific condition can be completely different, forming the basis of modern personalized medicine.

CHRONIC FATIGUE SYNDROME

Chronic fatigue syndrome (CFS) - one of the most common pathologies, the main feature of which is pronounced unmotivated weakness, which eliminates a person from active daily life for a long time. Also, the patient complains of musculoskeletal pain, sleep disturbance, deterioration in short-term memory and concentration, and headache. The modern concepts of the pathogenesis of CFS, and the criteria for its diagnosis are considered. Typical CFS risk factors include poor environmental and health living conditions; frequent and prolonged stress leading to maladjustment; violation of the physiological response to stress; monotonous hard work; lack of exercise with excessive improper nutrition; the lack of prospects in life and a wide range of interests in life. The problems of complex treatment of CFS are discussed.

The neuronal damage in ischemic cerebral diseases is associated with series of molecular mechanisms, which can be a target for pharmacological neuroprotective interventions. Currently, there is no consensus about the potentials of neuroprotective therapy in hyperacute ischemic stroke. However, a number of studies had shown the positive effects of citicoline by reducing the severity of residual neurological deficit. In chronic cerebrovascular insufficiency, the effect of therapy on the rate of progression of cognitive disorders can be considered as a criterion of efficiency of neuroprotective therapy. Available evidence allows to discuss neuroprotective effect of antihypertensive and neurometabolic therapy.

The article presents clinical data on the possibility of use in the treatment of psoriasis and psoriatic arthritis monoclonal antibodies (MA): their structure, mechanism of action, dosages and routes of administration. Drugs are divided into groups according to the results of the certification. Data from international studies on the efficacy and tolerability of MA in different nosological entities, as well as the results of own author’s clinical observations are analyzed. Practical recommendations on the use of MA in psoriasis and psoriatic arthritis are presented.

In clinical practice, we are often faced with patients who have back pain and joint pain in combination with a variety of comorbid medical conditions (arterial hypertension, coronary artery disease, cerebrovascular disease, chronic obstructive pulmonary disease, liver disease, kidney disease, etc.). Therefore, the issues of frequency of systemic complications of therapy with nonsteroidal anti-inflammatory drugs (NSAIDs) and its potential interaction with drugs receiving by the patient have identified the relevance of this article. Now it is proved that among a number of NSAIDs, the original meloxicam (Movalis) is characterized not only by low-risk for the development of comorbid somatic side effects, but a good safety spectrum in combination therapy, making it possible to use this drug in general medical practice.

Acute lower back pain (LBP) - a common cause for seeking medical care, and, in this situation, physician’s task includes quick, efficient and safe arrest the pain to minimize the risk of pain chronization. It should be noted that in most patients suffering from LBP even complete examination fails to identify any specific cause of pain. For relief of LBP, analgesics (paracetamol), nonsteroidal anti-inflammatory drugs (NSAIDs) and muscle relaxants are used. In the presence of contraindications to NSAID treatment or patient’s reluctance to take such drugs, alternative treatment with homeopathic drugs Traumeel S and Spaskuprel can be offered. The article presents the scheme of treatment with these drugs.

EFFICACY AND CARDIOVASCULAR SAFETY OF AMTOLMETIN GUACIL IN THE TREATMENT OF PATIENTS WITH ANKYLOSING SPONDYLITIS: FINAL RESULTS OF KORONA STUDY

The article presents the final results of a prospective, open 12-week study, KORONA study, which was aimed to the evaluation of the efficacy and cardiovascular safety of amtolmetin guacil (AMG) in patients with chronic lower back pain and controlled arterial hypertension. The study included 60 patients aged 18 and older, met with the modified New York criteria for ankylosing spondylitis, with the intensity of back pain of 4.0 and more points according to the numerical rating scale. Patients were divided into 2 groups: the first group (n=41) - patients with controlled arterial hypertension (AH), the second group (n=19) - without arterial hypertension. During first 14 days, patients received AMG at a dose of 180 mg/day, followed by 600 mg/day for 12 weeks. It has been shown that AMG is effective and safe in treating lower back pain in the in the short term. Application of AMG requires monitoring of blood pressure in individuals with high risk of cardiovascular events, especially in the first two weeks of treatment.

摘要

The effects of comorbid depression on quality of life and features of autonomic regulation in patients with psoriatic arthritis (PsA) were evaluated. It was found that patients with PsA and comorbid depression have predominant influence of the sympathetic nervous system, resulting in an imbalance of divisions of the autonomic nervous system and low heart rate variability. PsA patients with depression have worse clinical and functional parameters and quality of life than patients without depression. Depressive disorders worsen heart rate variability.

The analysis of the register of epilepsy and epileptic syndromes in Neurological Center of Epileptology, Neurogenetics and Brain Research of University Clinic was performed. It was found that patients with epilepsy and epileptic syndromes patients represent a major proportion in the structure of patient of Center. Among the clinical forms of epilepsy, symptomatic focal epilepsy (G40.2) and epileptic syndromes with complex focal seizures have predominance. The proportion of cryptogenic (unspecified etiology) epilepsy was 1/5 of all observed cases. Adverse side effects (ASE) in patients receiving antiepileptic drugs (AEDs) were recorded in every fourth patient. Attack aggravation as a serious ASE was recorded in 11.34% of patients with ASE. The incidence of the ASE has no gender-related and age-related differences. The structure of the ASE is different for the ASE analyzed. Obesity, gastrohepatopathy and alopecia dominate in the structure of ASE of AED.

The results of application of the drug Chondrogard (chondroitin sulphate) in 200 patients with fractures of various segments of the limbs. The introduction of the drug Chondrogard was carried out intramuscularly in a dose of 1 ml (100 mg) a day (first 3 injections), the fourth and subsequent injection of 2 ml (200 mg) through the day. During the observation period, which was 36 days, all patients had completed 18 injections of the drug. The result of the study revealed Chondrogen clinical efficacy in the treatment of patients with fractures of different localization.

The glucocorticoids (GC) are widely used in modern clinical practice. However, side effects developing on the background of long-term use of drugs of this group are often equal to the severity and health-social value of the underlying disease. One of these complications is a glucocorticoid-induced osteoporosis (GIO) and associated bone fractures. In current clinical guidelines, vitamin D active metabolites occupy separate place in the treatment of GIO, because they are able to neutralize many of the negative effects of GC on bone, muscle tissue and calcium-phosphorus metabolism in general. In particular, alfacalcidol can be successfully used for the treatment of GIO both as monotherapy and in combination with bisphosphonates.

The comparative analysis of consumption of antiepileptic drugs by adult patients in out-patient practice according to the system of preferential drug provision of the Krasnoyarsk Territory for 2011-2014 was performed. ATC/DDD-methodology was used for analysis; consumption levels are represented as the number of DDD/1000 beneficiaries a day. It was found that the highest level of consumption observed for valproic acid medications (319.48-273.77). High levels of consumption of benzobarbital (221.05-153.14), carbamazepine (95.88-87.18), and topiramate (43.13-48.06) are shown. The greatest increase in consumption during this period was marked for leve-tiracetam and lacosamide preparations.
