Clinical guidelines and real antimicrobial pharmacotherapy of patients with rhinosinusitis

Abstract

The pharmacotherapy of inflammatory diseases of the parana-sal sinuses of 104 patients was analyzed, which were hospitalized to the otorhinolaryngological department in the period from January 2020 to December 2021. All patients were divided into two groups: group 1 (n=28) - with acute polysinusitis, group 2 (n=76) - with chronic polysinusitis. According to the results of a bacteriological study, 38,2% investigations of the total number were positive in the first group, 75% - in the second group. The main causative agents of sinusitis were S. aureus and S. epidermidis. In both groups, most of the patients received ceftriaxone 2 g/day, which does not take into account the current guidelines for curbing the growth of antibiotic resistance.

The validity of prescribing drug prevention of gastroduodenal bleeding in patients with chronic ischemic heart disease

Abstract

A retrospective analysis of 200 case histories of patients aged 37 to 83 years who were hospitalized in the cardiology department of the Clinical Hospital «Russian Railway -Medicine», Voronezh» in 2020-2021 was carried out. The main diagnosis: chronic coronary heart disease. Evaluation of pharmacotherapy showed that proton pump inhibitors (PPIs) were prescribed to all patients with a high risk of gastrointestinal bleeding. At the same time, in 9 cases, erroneous prescriptions of these drugs were noted, which amounted to 4,5%. These were patients with one small risk factor for gastroduodenal bleeding (age 65 and older and gastroesophageal reflux disease). It is recommended in patients with stable coronary artery disease to more carefully determine the risk of gastroduodenal bleeding and indications for prophylactic use of PPIs.

The influence of environmental factors in patients with cardiovascular diseases living in rural areas has been studied. The influence of noise, vibration, atmospheric air pollution, nearby enterprises was noted by 25%, 2%, 2% and 19% of patients with hypertension (GB) living in rural areas, respectively. Correlations were revealed between atmospheric air pollution with a history of myocardial infarction (strong direct link, r=0.79), living in rural areas and concomitant diseases (moderate direct link, r=0.44).

Abstract

Pharmacogenetic testing (PGT) is one of the promising personalized approaches to prescribing drugs in clinical practice. The PGT’s feasibility plays an important role in the oral anticoagulants (DOACs) administration, especially in the event of adverse reactions -hemorrhagic and thrombotic complications. According to the results of our study, personalized pharmacotherapy of dabigatran etexilate 110 and 150 mg reduced the frequency of bleeding (17,79 vs 18,10) and slightly exceeded the cost (cost difference was 216,55 rubles) compared with standard therapy in patients with atrial fibrillation in combination with chronic kidney disease. The PGT application can be used to improve the safety of DOAC pharmacotherapy. In the future, it is possible to decrease the costs of a medical organizations by reducing the cost of genetic tests.

Unproductive cough is one of the most frequent complaints in COVID-19, therefore, levodropropizine, a peripheral antitussive agent, was prescribed to 98 patients for its treatment (basic group). 2022 marks the 145-th anniversary of the birth of academician N.I. Leporsky. Purpose: to present facts from the life of N.I. Leporsky, his merits. Results. In 1923-1942, Professor N.I. Leporsky headed the Department of Hospital Therapy of the Medical Faculty of the Voronezh University (since 1930 - the Voronezh Medical Institute). N.I. Leporsky owns 120 scientific papers, 5 monographs, 7 doctoral dissertations were defended under his supervision. He proposed a way to study the function of the stomach using a «trial breakfast», his work «Diseases of the pancreas» was internationally recognized. Conclusions. Among the students of N.I. Leporsky are 2 academicians, 9 professors, many worthy scientists and doctors.
