MANAGEMENT OF PATIENT WITH GASTROESOPHAGEAL REFLUX DISEASE

Abstract

The article presents modern data on prevalence, basic mechanisms of development and tactics of conducting patients with gastroesophageal reflux disease (GERD). Effectiveness of primary and maintenance therapy with proton pump inhibitors (PPI) was analyzed. Author demonstrates the role of kinetic disorders and especially the composition of reluctate, in particular the presence of duodenal contents, in the lack of effectiveness of treatment of patients with GERD. The necessity of adding to antisecretory therapy PPI and prokinetics drugs ursodesoxycholic-howling acid for the correction of these pathogenetic mechanisms is justified.

Abstract

The review deals with the analysis of the modern view of the etiopathogenesis, diagnosis and treatment of irritable bowel syndrome (IBS). The basic international criteria for the diagnosis, classification considered conventional forms of IBS. The high need for analysis of the possible causes of IBS development. The main approaches to the treatment of IBS, developed by a special committee of the American College of Gastroenterology and the Cochrane community on the basis of several large meta-analyzes and randomized clinical trials. For each of the therapeutic approaches are considered the advantages and disadvantages, as well as the level of evidence. It is concluded that pathogenetically justified approach in the treatment of IBS is the use of drugs with combined anti-inflammatory and anxiolytic activity.

GASTRODUODENAL BLEEDING IN PATIENTS WITH CARDIAC PATHOLOGY

Abstract

Erosive and ulcerative gastroduodenal bleeding (GDB) in patients with cardiac pathology is threatening complication, in which the mortality rate ranges from 9,6 to 80%, reaching the highest numbers of patients of elderly and senile age. The need for acceptance of pathogenetic therapy, including antiplatelet agents, nonsteroidal anti-inflammatory drugs and anticoagulants of direct action greatly complicates the treatment of erosive and ulcerative GDB, which in itself is also a threat to the life of the patient. All this dictates the need for a rational treatment strategies for these patients to minimize the threat of the death of the patients from both the GDB and from heart disease. Designed by the authors of the algorithm treatment of different variants of the GDB allowed to reduce the mortality rate to 16,2%. Treatment of patients with this comorbidity requires a differentiated approach, depending on the nature and intensity of bleeding or the threat of its recurrence in the background of the need for pathogenetic therapy of the underlying disease.

The article presents data on changes in indicators of respiratory function in healthy women during physiological pregnancy. The peculiarities of the state statistical and pneumatic tachometrics respiratory function in different periods of gestation revealed major changes in terms of volume and speed parameters of respiratory function during pregnancy, isolated periods of possible deterioration of bronchial obstruction and flow obstructive bronchopulmonary disease during pregnancy. THE ROLE OF ALFLUTOP IN THE TREATMENT OF PRIMARY OSTEOARTHRITIS

Abstract

The review of literature presents data on clinical efficiency of Alflutop in patients with osteoarthrosis with intra-articular and intramuscular injection and also in patients with lower back pain with paravertebral injection. It is shown that Alflutop has expressed analgesic and anti-inflammatory activity in the treatment of patients with different localization of the pathological process. Alflutop exerts regenerative effects on joint cartilage: a proven increase in the height of the cartilage, increasing the density of cartilage in some patients, which may be explained by either reparative processes or by increasing the hydrophilicity of the cartilage. Proven the development of rapid clinical effect, save it for a few months, no radiographic progression of osteoarthritis during 3 years of observation. No data on the development of any adverse reactions to the use Alflutop.

NECK PAIN: POSSIBILITY OF TREATMENT

Abstract

Neck pain is a common cause of referring the patient to the doctor. The most common type of pain is nonspecific ("mechanical", axial) pain in the neck. The disease may be different, but most patients are completely rid of the disturbing symptoms. For relief of acute back pain most widely used nonsteroidal antiinflammatory drugs. In principle, a new class of drugs for the treatment of pain syndromes, incl neck pain, are selective activators of neuronal potassium channels - SNEPCO (Selective Neuronal Potassium Channel Opener). Representative of this class of drugs is flupirtine (Nolodatak) with a wide range of pharmacological actions. The effectiveness of flupirtine was demonstrated in many randomized controlled trials in which it was compared with other analgesics or placebo. Several studies carried out in patients with degenerative diseases of the spine and myofascial syndrome. In these diseases flupirtine has a pronounced analgesic effect and reduces initially increased muscle tone.

Abstract

Gallstone disease - one of the most common human diseases, it ranks third after cardiovascular disease and type 2 diabetes. In Russia the incidence of this condition is 12%. According to the literature over the past decade a doubling of the proportion of patients diagnosed with cholelithiasis has been seen. The literature describes a number of "masks" of the flow of the disease, it is noted that 10% of carriers of gallstones have no clinical manifestations of the disease. The present clinical case demonstrates the complexity of the diagnosis in the case of polymorbidity when "screening" disease (dorsalgia), dominates the clinical picture and is confirmed by instrumental methods of diagnosis, for a long time masks progression of the other disease (gallstone disease), leading to complications.
