Adenomyosis and leiomyoma of the uterus: the clinical and morphological aspects of combined pathology

Abstract

According to the published data, the frequency of the combination of adenomyosis (AM) and leiomyoma (LM) of the uterus is 50-85% of cases of AM. Recognition of such a combination makes a complex problem to the clinician and linked with large diversity of features of these diseases with the absence of typical symptoms, and various forms of changes.

We performed a detailed comparative clinical and morphological analysis of 50 cases of AM and 150 cases of AM/LM combination. This analysis revealed the considerable hypodiagnostics of AM, especially by its combination with LM and/or by the presence of perifocal leiomyomatous hyperplasia. The regularity of perifocal leiomyomatous hyperplasia with microscopic forms of LM and/or the diffusive thickening of myometrium has lead to the increase of uterus size in AM. The immunohistochemical investigation revealed the correlation of expression of the estrogen and progesterone receptors in glandular epithelium and stromal cells of the AM foci, and in the leiomyocytes in perifocal hyperplasia areas of myometrium. This indicated the uniformity of the pathogenetic mechanisms of AM and LM. Differences in the symptomatology and clinical course of AM and of AM/LM combination were also established in our work. The diagnosis of AM demands the comprehensive investigation of patients taking into consideration the frequency of the combined forms and distinct features of their clinical and morphological parameters.
