Traumatism, orthopaedic sick rate and specialized medical care for children in Nord-West Federal Region

Abstract

On the basis of statistics from the Ministry of Health and Social Development of Russia and Commettee of Health of the Saint-Petersburg Government for 2001-2005 years, an analysis of traumatism and orthopaedic sick rate, invalidism and children mortality rate due to external factors was carried out.

A questioning of paediatric traumatologists and orthopaedists in the Nord-West Federal Region and in Saint-Petersburg as to questions of organization, prevention and medical care for the period 2001-2005 has allowed to evaluate the present state of traumatologic and orthopaedic care of children and to make proposals for its improvement.

Postgraduate medical training in the North-Western region of Russia

Abstract

Postgraduate training of doctors (internship training, residency, clinical residency training and postgraduate studies) in the North-Western Federal district of the Russian Federation has been analysed in the work. The major training is carried out in the Higher Educational Establishments of the Federal Agency of Health and Social Development with the contribution of Higher Educational Establishments and Scientific Research Institutes of other agencies. The most required specialisations for training on the part of the doctors have been specified. The increase of the number of internship and resident students demands creation of new organizational and educational structures adapted to the teacher specific purposes including regional educational centres which improves the quality of medical training. Thus the programme of multilevel medical training including scientific training in postgraduate studies is being gradually implemented in the region.

Efficacy of catecholamines on the rats remodelling miocardium and possibility of its reverse development

Abstract

The results of experimental studies of the Wistar line male rats demonstrated dopamine and adrenalin to effect differently the remodeling of the right and left cardiac chambers in rats. Data obtained give evidence of a more marked structural changes and a lower level myocardial remodeling reversion in the right ventricle vs the left one.

Comparative study of meteoadaptogenic properties of peptide and nonpeptide drugs in volunteers

Abstract

The meteoadaptogenic properties of pharmacological drugs of both peptide (cortexin, noopept, dilept) and nonpeptide (vinpotropil) strucrure were investigated in climate termobarocomplex Tabay (Japan) in healthy volunteers aged 20-24. All drugs studied possess meteoadaptogenic action. Its power depended on environment conditions to be used (overcooling, overheating, hypobaric hypoxia). Vinpotropil, optimizing physiological component of the functional state, can be recommended as a meteoadaptogen for both the cold and the hot climate as well as for hypobaric hypoxia, where it elevated psychological component of the functional state. Cortexin was qualified as an adaptogen and actoprotector only for hypobaric hypoxia conditions (uplands). Noopept, affecting the psychological component of the functional state positively, can be used for rapid adaptation to the cold and hot climate. In the hot climate, noopept enhanced physical efficiency. Dilept elevated the psychological component of the functional state preferably and can be considered as a psychomotor enhancer and adaptogen. Therefore, all the drugs studied (vinpotropil, cortexin, noopept and dilept) can be recommended as the drugs of activation, support and recovery of physical and psychological efficiency in rapidly changing environment conditions.

Polymorphism Glu298Asp of endothelial nitric oxide synthase gene and arterial hypertension

Abstract

The aim of the study was the analysis of the influence of polymorphism Glu298Asp of endothelial nitric oxide synthase gene in patients with arterial hypertension (AH). The study included 98 AH patients at stage // and //I (male, Russian nationals, average age 53,2). The following analyses were performed: clinical analysis of the blood, general analysis of the urine, lipid spectrum, plasma electrolytes, creatinine, glucose, electrocardiography, echocardioscopy, examination of the eye vessels, ultrasound examination of the carotid arteries, microalbuminuria. Other risk factors were taken into consideration (hereditary predisposition, smoking, obesity). The polymorphism Glu298Asp of endothelial nitric oxide synthase gene was detected with two methods: polymerase chain reaction and restrictase reaction. The control group for Glu298Asp polymorphism detection included 102 healthy Russian donors aged 18 to 50. Statistical significance of the differences was probed with the ч2 criterion. The genotypes ratios in AH patients were as follows: GG 54%, GA 36%, AA 10%, in the control group: GG 53%, GA 36%, AA 11%. The polymorphism Glu298Asp of endothelial nitric oxide synthase gene was independent of such risk factors as hereditary predisposition to AH, dyslipidemia, obesity, diabetes mellitus. Among the target organ pathology in AH, retinal angiopathy, carotid arteries atherosclerosis, hypertrophy of the left ventricle and microalbuminuria were found to be independent of the polymorphism Glu298Asp of endothelial nitric oxide synthase gene. The Glu298Asp polymorphism did not affect the incidence of associated clinical complications, such as obliterating atherosclerosis of the lower extremity vessels, ischemic heart disease, and acute insufficiency of cerebral circulation, whereas chronic heart failure had a significantly lower incidence in the group of AA homozygotes as compared to patients with GA and GG genotypes.

Of some hemostasis characteristics before surgery in orthopedic patients depending on biorhythms

Abstract

Pre-operative dynamic study of 974 patients selected for primary total hip replacement and 401 - for revision hip arthroplasty demonstrated considerable scattering of such characteristics as the number of thrombocytes and the time of capillary blood coagulation measured according to Soukharev, exceeding a possible error of the method. Testing of seven practically healthy volunteers performed daily during 30 days revealed similar scattering as well as an oscillating character of the parameters under the study. The level of thrombocytes (T) and the time of coagulation beginning, duration and completion (CB, CD, CC) oscillated with periodicity close to circaseptanic. These oscillations occurred around the trend, the magnitude of which changed with circadiseptanic (СВ, CC) and close to circavigintanic (T, CD) periods. Basing on their findings the authors come to the conclusion that these biorhythms may have caused described scattering.

Chemical composition of hard dental tissue in inhabitants of industrial regions of the Northwest of Russia

Abstract

The chemical composition of teeth enamel and hair of persons of the different age groups constantly living in industrial centers of Northwest of Russia (Saint Petersburg, Apatites, Monchegorsk) by mass-spectrometry method with inductively coupled argon plasma is studied. The distinctions in chemical compositions of tissues of inhabitants of various cities using the cluster analysis are established. Symbasis of environmental changes of microelement composition of hair and tooth hard tissues is revealed.

Theoretical foundation of the development of a vaccine against atherosclerosis

Abstract

The author, basing on his experience in the field of research of morphogenesis and pathogenesis of atherosclerosis and taking into account at the same time analysis of the appropriate scientific literature, presented his personal idea on the problem of preventive heaith care and treatment of atherosclerosis. Nowadays it is not possible to disclaim the fact that today it is impossible to develop the strategy of atherosclerosis treatment without taking into consideration those deep changes into the immune system that occur during development of the disease. Prescribing any therapy to a client, basing only on the values of the lipids level in blood, means to return back to the XX century. So, on the other hand, development of modern methods of treatment of atherosclerosis that are based on pathogenetic knowledge need new methodology, on the background of which development of a vaccine against atherosclerosis becomes possible.

Abstract

The review is devoted to the analysis of the role of melatonin in the clinic of internal diseases. There are many data testified that melatonin level and its circadian changes may be considered as the markers of the risk of the development of several diseases and pathologies during the arrangement of the program of prophylaxis. The biological effects of melatonin in the mechanisms of therapeutic pathology are considered. It has been shown that the regulation of biological rhythms is the most well-known biological function of melatonin. The role of melatonin as pharmacological substance for the treatment and rehabilitation of the patients with internal pathology is discussed.
