Venous return under influences of environmental factors of and during experimental hypertension

Abstract

The paper includes the general characteristics of the venous return shifts under pressor and depressor stimuli of different modality on the systemic circulation in normal conditions and under hypoxia, hypothermia, gravitation loads and during renal experimental hypertension. The data obtained reveal the participation of the adrenergic receptors in the systemic circulatory shifts at normal conditions and during hypertension not only in arterial, but in venous department as well. The pumping function of the chest participates in compensation of circulatory shifts during postural loads. The hypothermia compensates the circulatory shifts under hypoxia.

The results of microsurgical and endovasculartreatment of acute aneurysmatic hemorrhage in 118 patients are analyzed in retrospective manner.The main factors having an influence on disease outcome are defined, treatment strategy in intracranial aneurysm based on microsurgical and endovascular techniques developed. Significant improvement of treatment results is marked then endovascular coiling applied unrelated to hospital.

Surgical treatment of carotid artery stenosis

Abstract

Petersburg, 194044. Here we analyze the state of special medical care for patients with carotid artery pathology in Russia in 2006-2009. In spite of the overall increase in the number of operations on brachiocephalic arteries, which amounted to 6857 interventions in 2006 and 8187 interventions in 2007, their number in relation to the population size is still only 58 per million. In the majority of developed countries this number exceeds 200 operations per million, which highlights the low availability of special vascular surgical care to the Russian population. There are several reasons why this area of vascular surgery is lagging behind, such as insufficient collaboration between neurologists and vascular surgeons, as well as low numbers and qualification level of specialized surgeons.

Surgical treatment of thoracic aorta diseases

Abstract

Here we have analyzed the results of surgical treatment of 85 patients with thoracic aorta pathology in the period between 2000 and 2010. The average age of the patients was 47±5,2 years, the majority were mail (56 persons, 66%). 84 patients underwent operations. The patients with dissecting aortic aneurysm, regardless of its localization, were operated urgently after a necessary minimal examination. 13 patients died (15.3%), one of them before the surgery, because of a bleeding that started as a result of infection and rupture of xenopericardial valved conduit that had been introduced earlier. Postoperative lethality amounted to 14.1 % (12 patients): 7 persons died of acute myocardial weakness, 2 of bleeding, 2 of polyorganic insufficiency syndrome, and 1 of respiratory insufficiency. Success rates depended on the technical equipment and the concentration of patients with aortic pathology in the most experienced centers.

Abstract

Clinic experience on rendering of the specialized help is presented at a acute coronary syndrome of 202 patients: thrombolytic therapy is spent 32,1 % of patients, primary angioplasty with stent procedure 44,1 %. The basic problems arising at rendering assistance of this category of patients are analysed: the long time passing from the moment of occurrence of the first symptoms before receipt of the patient in a hospital, absence of accurate interrelation and coordination of work of the structures connected with rendering of the specialized help, insufficient maintenance thrombolytic preparations.

Till the present time ischemic heart disease is a dominate pathology in structure of cardio-vascular diseases. Heart failure considered to stenotic and occluding lesions of coronary vessels and their complications remain the main reason of mortality and disability of population in all industrially advanced countries. Using of modern methods of surgical treatment of ischemic heart disease lets to provide myocardial revascularization on beating heart of any complexity with low mortality and complication rates in patients with different risk of lethal outcome with good immediate and follow-up results.

Surgery visceral arteries: achievements and perspective

Abstract

Проанализирован опыт хирургического лечения больных с хронической ишемией органов пищеварения (ХИОП) и ишемической болезнью почек (ИБП), вызванных преимущественно атеросклеротическими поражениями висцеральных артерий. Оперированы 48 больных с ХИОП, 7 из них в экстренном порядке из-за развившегося ишемического инфаркта тонкой кишки: выполнено 15 антеградных и 26 ретроградных шунтирований чревного ствола (ЧС) и верхней брыжеечной артерии (ВБА) с резекцией тонкой кишки в 7 случаях, чрескожная ангиопластика и стентирование ВБА проведены у 5 и ЧС у 1 больного, у 2 осуществлена декомпрессия ЧС. Протезной инфекции не было. Летальность составила 14,6%. В отдаленном периоде (от 1 до 288 мес) состояние 34 больных (82%) из 41 было удовлетворительным при проходимых шунтах и артериях. Результаты позволили заключить, что использование по показаниям отмеченных способов восстановления кровообращения в мезентериальном бассейне является единственно эффективным методом сохранения целостности органов пищеварения и жизни больных. Оперирован 121 пациенте атеросклеротическими поражениями почечных артерий (ПА) и ИБП: выполнены 73 различных шунтирований ПА и 83 чрезкожных ангиопластик преимущественно со стентированием. Не оперированы 23 больных. Открытые реваскуляризации почек (РП) сопровождались 12,7%-й летальностью, после эндоваскулярных вмешательств ее не было. Купирование реноваскулярной гипертензии (РВГ) произошло у 7%, течение ее стало мягким у 57% и у 36% больных она не изменилась. Почечная функция улучшилась у 25%, осталась без динамики у 57% и у 18% больных ухудшилась. Сравнительный анализ показал, что отдаленная выживаемость оперированных больных существенно лучше, чем неоперированных (р = 0,00001), за счет достоверного снижения цифр АД и стабилизации почечной функции РП, а назначение ингибиторов ангиотензинпревращающего фермента обеспечивало лучшую функциональную жизнеспособность почек, чем блокаторов кальциевых каналов (р = 0,041).

Miniinvasive technologies in the treatment of critical limb ischemia

Abstract

The natural course of critical limb ischemia (CLI) leads to a bad prognosis for the limb and the patient's life. Within 6 months 25% of the patients die, 30% of the survivors undergo a large amputation, and 20% continue suffering from CLI. We have used subintimal angioplasty (SA) for recanalization of extended chronic arterial occlusions. We propose a method of directed SA of the shin artieries, which allows reconstitution of blood flow to the trophic defect zone. This is particularly relevant for patients with diabetes mellitus (DM) who have poorly developed collateral blood supply of the foot. The results of 255 endovascular interventions in patients with CLI have been analyzed. The success rate of transluminal balloon angioplasty amounted to 93%. In 65% of cases no CLI recurrence was observed within 1 year, which is comparable to the results of surgical revascularization.

Electroencephalographic con-elates of hemo-liquor-dynamics disturbances at various affection of a brain

Abstract

194 patients have been selected for the research (88 had in anamnesis traumatic brain injury and 106 cerebral disturbance of not traumatic character). At the result of the visual and spectral analysis in their EEG have been found out patterns in the form of flashes of group teta-waves ora regular teta-rhythm in frontal assignments. These patterns were regarded as indirect signs of infringement intracranial hypertension. 49,5 % surveyed had backbone diseases. Magnetic resonance imaging and Computed Tomography researches have confirmed availability of the changes connected with an intracranial hypertension at 37 from 40 patients (92,5 %).

Mechanisms of cardioprotective effect of uridine-5'monophosphate in acute heart ischemia

Abstract

It was found that prophylactic treatment with uridine-5'-monophosphate led to the stabilization of energy metabolism in ischemic myocardium. This appeared in the restoration of ATP and creatine phosphate levels, which decreased 60 minutes after occlusion of the left coronary artery. UMP also prevented accumulation of lipid peroxidation products and antioxidant system dysfunction in the heart at the early stages of acute ischemia. It was established that 5-hydroxydekanoat, a selective blocker of mitochondrial ATP-dependent K+-channel, eliminated the protective effect of uridine-5'-monophosphate. It was concluded that these channels involve in the cardioprotective mechanism of this uridine nucleotide. This data agree with the results of our previously study which had shown the anti-ischemic and anti-arrhythmic effect of uridine-5'-monophosphate consisting in reduction of zone of ischemia and the amplitude ofT-wave on ECG and decrease of heart rhythm disturbances on the model of acute myocardial infarction.

The characteristics of metabolic blood cell response to viral infections of various etiology

Abstract

We have found that the response of POL processes and a number of membrane values, to a viral attack is dependent on the biolodgical characteristics of the pathogene including the presense of lipid viral envelope, the peculiarities of viral multiplication and the release of newly synthetized viral particles from aggested cells of the host (gemmation together with a part of cellular membrane, host cell, lysis, etc.). The reaction of cyclic nucleotide system to the DNAand RNA-containing viruses is shown to be different which is likely to be explained by the peculiarities of the viral multiplication in the injrured cells of the microorganism as well as the necessity of IKK protective factors production (e.g., lymphokines including interferon and immunoglobulins).

Morphological and pathogenetical characteristics of chronic HBeAg-negative hepatitis В and chronic hepatitis С

Abstract

Aim to compare the frequency and severity of morphological changes in chronic HBeAg-negative hepatitis В (СНВ) and hepatitis С (CHC) and define their significance in the disease pathogenesis Materials and methods. We have analyzed morphological changes in liver bioptates of 39 patients: 18 with HBeAg-negative CHB and 21 with CHC. Classical signs of histological activity, as well as small cell and large cell hepatocyte dysplasia (SCHD, LCHD) were assessed, and correlative relationships between morphological and clinico-laboratory data were determined. Results. НВѴ-infection markers were rarely found in HBeAg(-) hepatitis: "Ground-glass-like" hepatocytes were detected in 27,8 % of patients, and no «sandy» nuclei were present. Hepatocyte fatty dystrophy, stepwise necrosis and inflammatory infiltration were more frequent and more severe in patients with CHC compared to CHB. SCHD was 10-fold higher than LCHD in both types of hepatitis. Positive correlations were found between aminotransferases and inflammation intensity, fatty dystrophy, and SCHD in CHC; between number of lymphocytes, erythrocyte sedimentation rate, inflammatory infiltration and SCHD in CHB. LCHD correlated with the duration of the disease. Conclusion. Our analyses have revealed more evident morphological signs of hepatocellular inflammation in CHC than in HBeAg(-) CHB, which correlated with the clinico-laboratory signs of necrotic-inflammatory syndrome. SCHD dominated over LCHD in both types of hepatitis.

A pancreatic cysts: difficulties differential diagnostics and option surgical approach

Abstract

The results of surgical treatment 193 patients with pancreatic cysts are presented. Pseudocysts revealed in 167, true cysts in 26 (cystous tumors 18 and native cysts 8) patients. Paracentetic methods under US-control applied in 78 patients with pseudocysts, operations of external catchment accomplished in 36, internal in 58 patients. In case of distal localization of pseodocysts, in their intimate union with adjacent organs and tissues absence, and also in nativeand timorous cysts radical operations are applied (25 patients). Established that strict adherence of diagnostic and medicinal algorithms, based on reliable laboratory, instrumental and morphological criteria, in aggregate with surgical technologies abilities, allow to minimize postoperative complications and mortality by good quality of life in majority patient with pancreatic gland cysts.

Kinetic and thermodynamic analysis of ligand interactions with albumin

Abstract

Drug interactions with serum albumin are of great use for the understanding of pharmacokinetic and pharmacodynamic mechanisms. In the paper interactions of atenolol, ibuprofen, phenylbutazone and warfarin with bovine serum albumin (BSA) were investigated in vitro by fluorescence quenching method. It was proved that the fluorescence quenching of BSA by ibuprofen, phenylbutazone and warfarin was mainly the result of static quenching due to complex formation, while in the case of atenolol BSA fluorescence was quenched dynamically. Binding constant К volumes at 25 °С were 3.3x102,3.6x106,1.4x105, and 7.9x10s M_1 for atenolol, ibuprofen, phenylbutazone and warfarin, respectively. Quenching analysis at different temperatures allowed calculating thermodynamic parameters such as AC, AH and AS. The results analysis according for thermodynamic principles achieved that the binding of ibuprofen and warfarin with BSA is enthalpy driven, whereas phenylbutazone binding is entropy driven. Interaction of atenolol with BSA was accompanied by equivalent changes of enthalpy and entropy. The significance of thermodynamic and kinetic data forthe interpretation of binding mechanisms is discussed.
