Abstract

While home-based chronic care management systems are widely discussed in international scientific papers, this is not the case in Russian publications. The main objective of this article is to fill this gap, dealing with aspects such as systems implementation. This includes current work processes and possible lists of patient home equipment. Special attention is given to exploring the relatively new term «health care coordinators and the different approaches to defining that role and its responsibilities. In the final part of the article, a short evaluation of existing projects worldwide is provided, followed by a discussion of how this experience can be utilized within Russia. This investigation is not only theoretical, but practical as well-drawing on data from both scientific and real-world examples
