To what classification of gestosis (preeclampsia) should adhere a doctor in daily work?

Abstract

The present-day designation of gestosis as preeclampsia and eclampsia excludes the notion of “pre-eclampsia” as a period immediately preceding the attack of eclampsia what leads to the untimely and inadequate therapy. The authors suggest in the everyday work of obstetricians to use terminology that includes gestosis of various degree, pre-eclampsia and eclampsia.

EFFICACY OF GONADOTROPIN-RELEASING HORMONE AGONISTS TREATMENT OF THE SIMPLE ENDOMETRIAL HYPERPLASIA IN WOMEN OF REPRODUCTIVE AGE WITH OBESITY

Abstract

A prospective, randomized, open, comparative study. There were 105 women of reproductive age with obesity and simple endometrial hyperplasia without atypia (EH) included into the study. 13 patients had left the study.31 patients took an agonist of gonadotropin-releasing hormone (aGnRH) Buserelin at the dose of 3.75 mg intramuscularly every 28 days. 33 patients took 10 mg of norethisteron per day. 28 patients took combined estrogen-gestagen drugs (30 mkg of ethinylestradiol + 150 mkg of desogestrel). The base therapy of obesity (a balanced diet and a physical activity)had been administered to all of the patients. Treatment in all groups lasted 6 months. The results of this study showed that usage of aGnRH in management of endometrial hyperplasia in patients of reproductive age with obesity is most effective and safe.

The tocography variants of uterine activity hyperstimulation

Abstract

We performed an analysis of 85 histories of labor. Depending on perinatal outcome and application of uterotonics all stories of labor were divided into 3 groups. We present typical signs and shapes of uterine activity hyperstimulation in the application of uterotonic in labor. We established that with the appearance of any signs of hyperstimulation, it is necessary to limit or suspend the infusion of drug or to revise the plan of labor conduction in favor of the Cesarean section.

Combined therapy of extensive genital endometriosis with aromatase inhibitors

Abstract

The article represents our experience in evaluation of aromatase activity in endometriotic lesions. We revealed that the most important parameter in evaluating aromatase activity is the area of its expression. Increased activity of this enzyme in the foci of endometriosis was identified, which proved the prescription of aromatase inhibitors in postoperative period for 68 patients of reproductive age with genital endometriosis. It was shown that the combined therapy of endometriosis-aromatase inhibitors and progestagens (19-nortestosterone derivatives) is effective, safe and well tolerated way of treatment of the disease, but, undoubtedly, this scheme requires further research

Current data contributing to the origin and development of endometriosis (E) — sever common disease of reproductive age women are reviewed. Numerous theories and hypothesis as well as basic pathological mechanisms underlying E are briefly outlined. Whole gene-net and particular groups of candidate genes involved in E are presented. The later include genes of detoxification system, sex hormones and their receptors, cytokines gene, onco- suppressors the genes contributing to embryonic development of female reproductive organs and cell proliferation Major molecular genetic data imply epigenetic impact in the origin of disease which results from progressive disregulation of endometrium genome functions provoked by unfavorable combinations of candidate genes of some signaling pathways and defects in their expression due to abnormal methylation pattern. In line with systemic genetic concept initial stages of E could be provoked by different genetic, epigenetic and even external factors which outcome is the same pathophysiological and pathomorphological damages clinically classified as E. Though the researches of the pathophysiological mechanisms of growth of adhesions are going on the attempts to determine effective methods to prevent adhesions do not rule to expected results. Advances in molecular biology identify number of biologically active molecules that regulate the inflammatory response, angiogenesis and tissue reconstruction. Those are crucial for normal peritoneal wound healing or for growth of tissue fibrosis.
