Hormonal profile changes under the influence of environmental factors in children residing in the Aral environmental disaster region

Abstract
BACKGROUND: Since the mid 20th century, the Southern Aral Sea Region is notorious as a region of ecological disaster, where anthropogenic influence led to the desiccation of the Aral Sea. Aridization of these territories is the cause of accumulation of endocrine disrupting chemicals — pesticides and toxic metals in soil and water.
AIM: The aim of this study was to assess the changes in hormonal profile under the influence of environmental factors in prepubertal children living at different distances from the epicenter of the Aral ecological disaster.
MATERIALS AND METHODS: The study included 58 male children aged 11–13 years. All children from birth lived in two regions of the Aral Sea Region and were divided into two groups depending on their place of residence. The group “North” (unfavorable conditions) consisted of 27 children from Muynak, Kungrad, Karauzyak, Takhtakupyr districts. The “Nukus” group (relatively favorable conditions) included 31 volunteers from Nukus city. All children were determined the concentration of IGF-1 — Insulin-like growth factor 1, GH — growth gormone, TT — total testosterone, E2 — estradiol, FSH — follicle-stimulating hormone, LH — luteinizing hormone, TSH — thyroid-stimulating hormone, TT3 — total triiodothyronine in blood sample. The statistical significance of the differences of the mentioned hormones in children from the “North” and “Nukus” groups was checked using the Mann–Whitney test.
RESULTS: This analysis revealed statistically significantly higher values of TT, FSH and LH in children from the “Nukus” group compared to their peers from the “North” group. The concentrations of IGF-1, GH, E2, TTG and T3 in blood plasma in volunteers from both groups were not statistically significantly different.
CONCLUSIONS: The environment of the Aral ecological disaster region with increased content of organochlorine compounds, pesticides and heavy metals changes the endocrine status in local prepubertal children. This is expressed in a moderate decrease in the activity of androgens (but not estrogens) and gonadotropic hormones. This research work can be considered as a pilot study, which determines the need for further monitoring of endocrine disorders in children and adults living in negative environmental conditions.

АКТУАЛЬНОСТЬ
Южный Казахстан, западный Узбекистан и северный Туркменистан, именуемые Приаральем, печально известны из-за экологической катастрофы, существующей там с середины ХХ в. [11, 37, 144]. Традиционно экономика этого региона основывалась на животноводстве, земледелии, в первую очередь хлопковом, рыболовстве в Аральском море, реках Сырдарье и Амударье [37, 144]. В Советском Союзе хлопок был важной стратегической культурой, которую считали необходимой для оснащения армии [37, 144]. Внедрение масштабной ирригационной схемы в 1950-х годах для поддержки хлопковых полей, а также увеличение забора воды из Амударьи и Сырдарьи для промышленных, сельскохозяйственных [11, 37, 144] и хозяйственно-бытовых нужд сократило сток двух основных рек Аральского моря менее чем наполовину [11]. С 1918 по 1960 г. забор воды для орошения хлопковых полей увеличился примерно на 40 %, с 1960 по 2008 г. — более чем на 200 %. К 2008 г. было потеряно почти 90 % объема воды Аральского моря и 74,3 % его прежней площади [11, 117, 165]. В результате образовалась новая пустыня, названная Аралкум (рис. 1), размер которой сейчас составляет около 250 км в поперечнике, а её почва имеет сильнейшее засоление [17]. Соляные корки, образовавшиеся на высохшем дне бывшего озера, богаты различными минералами и тяжелыми металлами — натрием, хлоридами, магнием, кальцием, сульфатами, боратами, литием, калием [11], цинком, никелем, хромом, медью, мышьяком и свинцом [117, 134], а также ураном [57].

Десятилетия сельскохозяйственной практики с применением химикатов привели к высоким уровням токсичных пестицидов в местной окружающей среде [111]. Избыточная минерализация почвы и грунтовых вод, их засоление, избыточное испарение жидкости из почвы привели к нарушению естественной миграции и элиминации пестицидов, их накоплению и распространению по всей территории Приаралья [83].
С 1960-х годов хлорорганические соединения (ХОС): дихлордифенилдихлорэтилен (ДДЭ) и дихлордифенилтрихлорэтан (ДДТ), пропанид, гексахлорциклогексан (ГХГ), гексахлоран, а также бутифос использовались в достаточном количестве в качестве пестицидов [11]. Наряду с пестицидами и тяжелыми металлами окружающая среда Приаралья загрязнена диоксинами [122, 152]: полихлорированными бифенилами (ПХБ), полихлорированными дибензо-п-диоксинами (ПХДД) и полихлорированными дибензофуранами [11]. Допустимые концентрации превышают полихлорбифенил, полихлордибензодиоксины, полихлордибензофураны, 2,3,7,8-тетрахлордибензо-п-диоксин (2,3,7,8-ТХДД) [111]. Эти соединения используются в промышленности [11]. Основным источником выбросов считается деятельность нефтеперерабатывающих заводов, таких как «Испат-Кармен», «Балхашмыс», «Акчатау», «Жезказганцветмет», «Шалкия» и «Шымкент»[11].
В организм человека ХОС [114] и тяжелые металлы [128, 135] могут попадать через плаценту и через молоко матери при грудном вскармливании [66, 96, 109, 171]. При анализе донорского молока у женщин Приаралья обнаружены превышение концентрации ХОС [15, 69]. Пренатальный период влияния ХОС [29] и тяжелых металлов (свинец) [5, 6, 39, 67, 68, 73, 128] считается критическим окном негативного воздействия этих соединений. Нарушение работы эндокринной системы более выражено при влиянии ХОС в период новорожденности [6–8, 39, 68, 70, 73, 128], чем у подростков [9, 39, 45, 115].
В организм взрослого человека пестициды попадают в основном с пищей [59] при потреблении молочных продуктов и рыбы [40, 138]. В Приаралье высокие концентрации ПХБ и ТХДД обнаружены при анализе бараньего, куриного жира, хлопкового масла, рыбы, яиц, молочных продуктов [112].
В плазме крови ХОС связываются с жирами и распространяются вместе с ними по органам и тканям [107]. В опытах на крысах [146] и свиньях [35], при аутопсии у человека [44] установлены высокие концентрации ХОС в жировой ткани, мышцах, головном мозге, почках, печени. Тяжелые металлы, как правило, локализуются в костях [65], печени, почках [100] яичках и яичниках [133].
Хлорорганические пестициды остаются в организме достаточно долго — их полураспад может составлять до 8–10 лет [27, 94]. В результате масштабного анализа, проведенного в 1990–2000 гг., в организме у детей и взрослых обнаружены высокие концентрации ДДТ [12–14, 75], гексахлорана [75], ПХБ [75, 112], ТХДД [69, 111], свинца [75], мышьяка, натрия, ртути, брома и никеля [31, 50].
Многочисленные исследования выявили высокую распространенность поражения респираторной системы и желудочно-кишечного тракта, анемий, почечной патологии [37, 116, 144], нарушений ментального здоровья [136], онкологических заболеваний [99] у жителей Приаралья. Все перечисленные ХОС [23, 27, 62, 173] и некоторые тяжелые металлы [25, 28, 30, 36, 72, 125, 149] являются эндокринразрушающими соединениями (ЭРС). Это означает, что они способны вмешиваться в биосинтез, инкрецию, лиганд-рецепторные взаимодействия и в эффекты практически всех системных гормонов [62]. В литературе имеются только немногочисленные данные, описывающие влияние условий среды Приаралья на гормональный профиль у детей, проживающих в регионе [101, 172]. Высокая распространенность патологии репродуктивной системы среди женщин и мужчин, проживающих в данном регионе, возможно, — одно из проявлений влияний ЭРС [85, 151]. В предыдущих исследованиях мы предположили, что изменения телосложения могут быть следствием ЭРС-опосредованного изменения активности гормонов, контролирующих рост и развитие у жителей региона Аральской экологической катастрофы [1–3]. Однако указанные исследования производились с участием взрослых, в то время как дети — наиболее уязвимый контингент для влияния ЭРС на эндокринный статус [26].
Поэтому целью данной работы стала оценка изменения гормонального профиля под воздействием факторов окружающей среды у детей препубертатного возраста, проживающих на разном удалении от эпицентра Аральской экологической катастрофы.
МАТЕРИАЛЫ И МЕТОДЫ ИССЛЕДОВАНИЯ
В работу были включены 58 детей мужского пола в возрасте 11–13 лет, в 2018–2022 гг. обратившихся в частную медицинскую клинику г. Нукус для диспансерного наблюдения или медицинского обследования. В выборку вошли добровольцы с достаточным питанием, не имеющие ожирения и дефицита массы тела, задержки полового развития или иной эндокринной патологии, с отсутствием тяжелых заболеваний в анамнезе, включая COVID-19, которые согласились предоставить образец крови. Все дети с рождения проживали в двух регионах Приаралья на разном удалении от эпицентра Аральской экологической катастрофы и были разделены на две группы в зависимости от их места жительства (рис. 2). Группу «Север» составили 27 детей из Муйнакского, Кунградского, Караузякского, Тахтакупырского районов. Считается, что жители северных регионов Приаралья находятся в самых неблагоприятных условиях из-за максимальной аридизации и пестицидной нагрузки [4, 11, 117]. В группу «Нукус» вошел 31 доброволец из г. Нукус, который признан относительного благополучной зоной для проживания [4, 11, 117], поскольку там активно реализуются меры по ликвидации последствий экологической катастрофы, а именно общегородские мероприятия по очищению воды1,2.

В ходе обследования в клинике у всех добровольцев были взяты образцы венозной крови между 7:00 и 8:30 утра после ночного голодания с последующим центрифугировнием при 1500 об/мин в течение 10 мин для получения сыворотки. По нашей просьбе в лаборатории был исследован гормональный профиль у каждого участника. Определяли концентрацию инсулиноподобного фактора роста 1 (ИПФР-1), соматотропного гормона (СТГ), общего тестостерона (ОT), эстрадиола (E2), фолликулостимулирующего и лютеинизирующих гормонов (ФСГ и ЛГ), тиреотропного гормона (ТТГ) и трийодтиронина (T3) методом прямого твердофазного хемилюминесцентного иммуноферментного анализа («сандвич»-метод) с использованием коммерческих наборов для тестирования на микропланшетном ридере MR-96A Mindray, (Shenzhen Mindray Bio-Medical Electronics Co, Ltd., Китай).
Статистическую значимость различий концентрации указанных гормонов у детей из группы «Север» и «Нукус» проверяли с применением теста Манна – Уитни. Статистически значимыми считали результаты при p < 0,05. Если данные имели статистически значимые различия, рассчитывали стандартизированный размер эффекта по Коэну (d) по общеизвестной формуле [142]. Стандартизированный размер эффекта интерпретировали по методике, предложенной авторами исследования [142]: 0–0,2 — незначительный эффект; 0,2–0,5 — небольшой эффект; 0,5–0,8 — умеренный эффект; более 0,8 — высокий эффект. Вычисляли разницу средних и значения p для нее с применением t-критерия Стьюдента, а также 95 % доверительный интервал (95 % CI) для разницы средних. Если вычисленный 95 % CI для нее не включал в себя значение «0», то оцениваемое этим интервалом неизвестное значение разности средних статистически значимо отличалось от «0». Это дает основания отклонить нулевую гипотезу о равенстве значений гормонов у испытуемых из двух групп и принять альтернативную об их неравенстве.
Вычисления производились с применением программы статистической обработки данных Past version 2.17, Norway, Oslo, 2012. Для вычисления размера эффекта и стандартизированного эффекта по Коэну использовали приложение для Exel из прикладного пакета Microsoft Office 2010 Exploratory Software for Confidence Intervals (ESCI-JSMS), Melbourne, Australia, 2001. Столбчатые диаграммы подготовлены с использованием программного обеспечения для анализа данных OriginPro 2021 (OriginLab, США). Все данные на диаграммах представлены в виде средних значений (μ) и 95 % CI.
РЕЗУЛЬТАТЫ ИССЛЕДОВАНИЯ
Анализ данных выявил статистически значимо большую концентрацию следующих гормонов с умеренным размером эффекта у детей из группы «Нукус» по сравнению с их сверстниками из группы «Север» (рис. 3): ОТ (p = 0,0101; d = 0,67), ФСГ (p = 0,03334; d = 0,62) и ЛГ (p = 0,02105, d = 0,64). Рассчитанные 95 % CI для разностей средних значений содержания ОТ [0,68 (0,14; 1,22)], ФСГ [1,26 (0,19; 2,33)] и ЛГ [0,77 (0,16; 1,70) у данных субъектов не включали значения «0», следовательно, оцениваемое этим интервалом неизвестное значение разности средних статистически значимо отличалось от «0» на уровне значимости α = 0,05.
Концентрации ИПФР-1 (p = 0,06359), СТГ (p = 0,1092), Э2 (p = 0,2638), ТТГ (p = 0,1187) и T3 (p = 0,3338) в плазме крови у добровольцев из обеих групп статистически значимо не отличались (рис. 3).

ОБСУЖДЕНИЕ
В результате наблюдений произведена попытка охарактеризовать негативное влияние экологически небезопасной окружающей среды Приаралья (Узбекистан) на эндокринную систему современных подростков. Насколько нам известно, это первое исследование с участием юношей из Узбекистана, проживающих в эпицентре Аральской экологической катастрофы. Тем не менее сопоставимые результаты были получены с участием детей из Западного [172] и Южного Казахстана [18]. Низкие уровни СТГ, ИПФР-1, ЛГ, ФСГ, ТТГ, тироидных гормонов обнаружены у детей препубертного и пубертатного возраста, проживающих в селе Кобда Актюбинской области Казахстана, расположенного примерно на 750-километровом удалении от Аралкума [172]. В недавнем исследовании с участием девочек-подростков из Сарыагачского и Сайрамского районов Туркестанской области Казахстана (около 850 км от пустыни Аралкум) выявлено снижение ФСГ, ЛГ и E2 и обратная корреляция этих гормонов с содержанием в организме ДДТ и диоксинов [18].
В результате проведенных наблюдений необходимо заключить, что окружающая среда региона Аральской экологической катастрофы с повышенным содержанием ХОС, пестицидов и тяжелых металлов изменяет эндокринный статус у местных детей предподросткового возраста. Это выражено в умеренном снижении активности андрогенов (но не эстрогенов) и гонадотропных гомонов. Эти результаты согласуются с данными других исследований, где описывается антиандрогенный и антигонадотропный эффекты пестицидов [27, 33, 38, 55, 56, 62, 76, 108, 132, 139–141, 157, 168, 173] и токсических металлов [74, 102, 125, 131] у детей и взрослых, проживающих на иных загрязненных территориях. В ходе многочисленных исследований было показано, что ДДТ, ДДЭ и ГХГ обладают андрогенингибирующим влиянием за счет угнетения синтеза и блокады рецепторов мужских половых стероидов [27, 39, 49, 51–56, 62, 76, 108, 157, 168, 173] и способствуют задержке наступления пубертата [58, 64, 80, 90, 143, 147, 169]. Пренатальное воздействие ДДТ [43, 154–156, 163] и ПХД [20, 165, 166] приводит к более позднему началу продукции андрогенов и усиленному превращению тестостерона в эстрогены [77] после полового созревания.
В литературе сообщается об обратной корреляции содержания тестостерона в плазме и токсических химических элементов-металлов: марганца, ртути, хрома, мышьяка, цинка, кальция, меди, молибдена [74, 97, 98, 102, 104, 125, 131] и, в особенности, кадмия и свинца [10, 106, 124, 149].
Результаты данной работы согласуются с тем, что ХОС могут изменять биосинтез за счет воздействия на пульсирующую секрецию гонадотропин-рилизинг гормона [32, 33, 46, 47, 63, 118, 126, 127, 150, 162], а также транспорт, клиренс, лиганд-рецепторное взаимодействие гонадотропинов — ФСГ и ЛГ [110, 123, 129, 164, 170]. Содержание селена и меди было обратно взаимосвязано с концентрацией ЛГ [41, 91, 93, 103]. В некоторых работах показано снижение ФСГ и ЛГ, сопряженных со снижением сроков начала пубертата под воздействием свинца [64, 78, 81, 143, 145, 158, 159–161] и кадмия [20, 72, 81], мышьяка, ртути, никеля и хрома [19, 137], магния [48]. На примере кадмия демонстрируется отрицательное влияние тяжелых металлов на функцию гипоталамо-гипофизарной оси [88, 89].
Несмотря на декларируемый в литературе антитиреоидный эффект у хлорорганических пестицидов (ДДТ и ПХБ) [16, 21, 22, 60–62, 84, 105, 119–121, 130, 148, 173] и тяжелых металлов (кадмия, свинца, ртути, мышьяка) [16, 32, 33, 65, 79, 82, 167], снижение активности гормонов щитовидной железы у юношей предподросткового возраста, проживающих в регионе Аральской экологической катастрофы, не нашло подтверждения. Не доказанным является также изменение активности СТГ и ИПФР-1 у субъектов, обследованных в данной работе, хотя ДДТ [174] и ПХБ [34, 46] могут снижать концентрацию СТГ [71] и ИПФР-1 [34, 71, 174] у жителей загрязненных территорий [34]. Свинец [24, 129], цинк [95] никель [87, 95] кадмий [95] снижают концентрацию СТГ и ИПФР-1. Предполагается, что свинец блокирует связывание СТГ с его рецептором [24, 19].
Считается, что эффект влияния ЭРС [42, 62, 86, 147, 153] и тяжелых металлов [113] на эндокринную систему человека дозозависим. Низкие, средние и высокие концентрации этих веществ в окружающей среде обусловливают направление изменения гормонального статуса (ингибирование или стимуляция) и ее выраженность (наличие или отсутствие реакции) [62, 113, 153]. В настоящей работе обнаружено, что у детей, родившихся в 2007–2011 гг. и проживающих в регионе с повышенным загрязнением ЭРС, имеет место умеренный антиандрогенный и антигонадоторопный эффекты, в то время как СТГ, ИПФР-1 и тироидные гормоны остаются неизменными по сравнению с их сверстниками из благоприятных регионов. В предыдущих работах при обследовании взрослых 1995–2000 гг. рождения нами обнаружен антиандрогенный и слабый эстрогенстимулирующий эффект воздействия ЭРС окружающей среды Приаралья [1]. Таким образом, можно предположить, что полученные умеренные изменения гормонального профиля у детей могут быть следствием снижения концентрации поллютантов и пестицидов в окружающей среде с конца 1990 по 2010 г. вследствие реализации программы экологического мониторинга, проводимой в Республике Узбекистан. Еще одной возможной причиной отличий содержания гонадотропных гормонов у добровольцев из обеих групп могут быть индивидуальные особенности концентрации ФСГ и ЛГ у детей препубертатного возраста [99, 92].
ЗАКЛЮЧЕНИЕ
Необходимо отметить, что проведенное исследование можно считать пилотным, которое обусловливает необходимость дальнейшего мониторинга эндокринных нарушений у детей и взрослых, проживающих в негативных экологических условиях. В перспективе представляется возможным получить результаты анализа гормонального профиля и антропометрического статуса у субъектов различных возрастных групп, проживающих в Приаралье. Полученные данные откроют возможность изучения способов коррекции негативных последствий влияния окружающей среды на здоровье человека.
