Нетрадиционные методы коррекции нарушений микробиоты у детей, рожденных путем кесарева сечения

Аннотация
Рост частоты кесарева сечения, ассоциированный с увеличением числа осложнений этой операции, беспокоит не только акушеров-гинекологов во всем мире, но и педиатров. Избыточная медикализация такого биологически естественного процесса, как роды, увеличение частоты кесарева сечения, отсутствие контакта плода с микрофлорой влагалища матери, последующие сложности в налаживании грудного вскармливания непосредственно оказывают влияние на формирование микробиоты кишечника ребенка первого года жизни. На сегодняшний день микробиом кишечника рассматривается как отдельный орган человека, определяющий предрасположенность к развитию таких заболеваний, как ожирение, сахарный диабет, инсулинорезистентность, аллергия, аутизм и даже депрессия. Корректируя микробиоту кишечника ребенка первого года жизни, можно воздействовать на риски формирования этих нозологий. На сегодняшний день предложены различные способы коррекции микробиоты кишечника детей, рожденных путем кесарева сечения. Самый простой и физиологичный способ — это налаживание длительного грудного вскармливания, поскольку именно олигосахариды грудного молока способствуют росту бифидобактерий. Перспективной представляется и пробиотическая коррекция. В обзоре представлены новые технологии восстановления микробиоты кишечника детей, рожденных путем кесарева сечения, избежавших по объективным причинам контакта с микрофлорой влагалища матери. Некоторые из этих технологий являются достаточно спорными, ряд методик, как, например, вагинальное обсеменение, представляются легко выполнимыми и перспективными. Дальнейшее их изучение на больших когортах пациентов позволит оценить их безопасность и эффективность в профилактике нарушений микробиома детей первого года жизни.
