Urolithiasis: organization of medical care in military medical institutions of central subordination

Abstract

The results of the analysis of the structure of patients with urolithiasis, operations performed in medical institutions of the Ministry of Defense of the central subordination, their equipment with modern equipment, organizational and staffing schedule in 2015–2019 are presented. A steady increase in the number of patients with urolithiasis was noted. The largest number are military pensioners, military personnel – no more than a third. The organizational and staff structure and the provision of equipment for urological departments (centers) in military medical organizations of the 3rd level generally comply with the requirements of the governing documents. Operational activity in head offices increased by 18.1%, in branches – by 10.3%. The most demanded surgeries are minimally invasive interventions. Specialized medical care for patients with urolithiasis in medical institutions of central subordination can be provided to an exhaustive extent in 3 urological centers of head institutions and 2 branches, which are institutions of an expert class. The ways of improving the system of rendering specialized urological care are proposed.

Disinfection of postal correspondence as a way to counter postal bioterrorism (Literature review)

Abstract

The article is devoted to the problem of ensuring biological safety when handling postal correspondence. A comparative description of the ways to prevent the defeat of people by pathogenic biological agents through contaminated postal items is given. Requirements for the method of mail correspondence disinfection are formulated. To prevent contamination of technical personnel and final addressees, it was proposed to conduct mail disinfection at correspondence processing points. The requirements for this method of ensuring biological safety when handling postal items are formulated. A comparative analysis of existing disinfectants has been carried out. It is shown that electron-beam disinfection of mail correspondence meets all the requirements.

Features of the clinic and diagnosis of the new coronavirus infection COVID-19 under conditions of the consultative and diagnostic center of the Ministry of Defense of the Russian Federation

Abstract

The purpose of the study was to study and systematize the clinical manifestations of the new coronavirus infection COVID-19, its diagnosis in a large consultative and diagnostic center of the Ministry of Defense. The study included 317 military personnel (contract, reserve, and retired) aged 20–65 who had survived COVID-2019. The disease was more often detected in the age categories 31–40 and 41–50 years old, in senior officers, in most cases it was mild. The peak incidence in October – December 2020 is due to the lack of vaccination. The subsequent decline is associated with the mass vaccination of military personnel. There is a low level of complications in the form of viral lung damage. Typical were subfebrile and moderate febrile fever, dry cough, sore throat, and general weakness, which made it difficult to diagnose and expanded the indications for additional studies and exclusion of other types of infections. The main diagnostic criteria were the results of PCR testing (swabs from the nasopharynx and oropharynx) for SARS-CoV-2, laboratory blood parameters (specific markers – C-reactive protein, D-dimer, ferritin), coagulogram indices, general blood count (leukocytopenia and thrombocytopenia).

Actual aspects of ophthalmology in the context of the pandemic of the new coronavirus infection COVID-19

Abstract

Presents data on the ocular manifestations of the new coronavirus infection (COVID-19), the risks of infection, and the need to use personal protective equipment. Transmission of the virus through the ocular surface is possible during the incubation period, in persons with the asymptomatic course, in any, including the lung, course of the disease, during the period of convalescence. Conjunctivitis may be the first and only manifestation of the disease. The mucous membrane of the eye can be both an entrance gate for infection and a source of infection, therefore, any patient should be considered potentially dangerous. It is necessary to comply with all safety measures when working with patients with a confirmed disease and with suspicion of it. The risk of spreading the virus is high when patients are examined by doctors of various specializations, especially ophthalmologists, who require close and intimate physical contact with the patient during standard ophthalmological examinations.

Resuscitation as a mandatory preliminary stage in the treatment of patients with severe necrotizing pancreatitis in the early stages of the disease

Abstract

The presented review helps to explain the mechanisms of the development of destructive pancreatitis. The research is based on the analysis of domestic and foreign literature. It was found confirmation of the presence of mechanisms of microcirculatory disorders, the basis of which is the development of cellular and tissue ischemia, initiated by cytokine intercellular interactions, ultimately leading to multiple organ failure. The universality of the mechanism of development of the described pathophysiological phenomenon is the body’s response to super aggression, developed in the process of evolution and genetically encoded in the chronobiological program of the living system. Critical conditions, different in etiological characteristics, are a monopathogenetic process, the biological expediency of which is built on the criteria of «economy», simplicity and efficiency. The applied value of the obtained results lies in the fact that they create the basis for building the pathogenetic treatment of patients with critical conditions, which are correlated to the clinical equivalents of the pathophysiological phenomenon in the form of microcirculation disorders of organs and tissues.

Possibilities and effectiveness of dental rehabilitation using dental and zygomatic implants

Abstract

To assess the survival rate and analyze complications when using orthopedic structures of zygomatic and root dental implants as support, data from a retrospective clinical observation of patients with permanent dentures are presented. A total of 29 people of different sex and age were under observation, who, due to complete or partial loss of teeth, were installed 68 zygomatic and 137 root dental implants in a specialized department of the hospital. Implants were installed for dental rehabilitation of complete loss of teeth in conditions of severe atrophy of the jaws using fixed dentures with screw or cement fixation. The observation period for the patients ranged from 5 to 8 years. Complications of zygomatic and dental implantation at different periods of observation are shown, their incidence and causes of development are analyzed.

Experience in the use of prosthetics for prosthetics based on dental implants

Abstract

The article presents the organization and results of orthopedic treatment of 30 patients who received fixed prostheses based on dental implants (as replacement of dentition defects previously restored with removable prostheses), as well as 5 patients with complete edentulous lower jaw and the second type of edentulous lower jaw according to Keller (previously used removable dentures). Based on the results of the questionnaire survey after 6 months, it can be concluded that with adequate oral hygiene, the patient satisfaction rate with the results of orthopedic treatment based on dental implants was 100%. At the same time, the quality of life of patients has significantly improved due to an increase in chewing efficiency, restoration of aesthetics, as well as psychological rehabilitation.

Organization of internal quality control of the circulation of drugs and medical devices in a military medical organization

Abstract

Effective monitoring is necessary to ensure the provision of drug care and the use of medical devices in the diagnosis and treatment of patients. This is a complex management process aimed at providing high-quality and safe resources for the main production process of a military medical organization for the provision of high-quality medical care. It is proposed to use practical experience in the implementation of internal quality control of the circulation of medicines and medical devices in the 12th Consulting and Diagnostic Center of the Ministry of Defense, including the development and implementation of standard operating procedures in all areas of circulation of medicines and medical devices, planning of internal quality control in all departments, timely identification and elimination of risks associated with the circulation of drugs and medical devices, the organization of constant interaction with the regulatory authorities of the medical service of the Armed Forces and the Ministry of Health of Russia.

Assessment of the diffusion capacity of the lungs in persons who have undergone a new coronavirus infection COVID-19

Abstract

Comprehensive outpatient studies were carried out in 100 patients who underwent a new coronavirus infection complicated by bilateral viral lung disease to assess the diffusion capacity of the lungs. 6% showed a decrease in this indicator from the borderline to a significant degree. Changes in the diffusion capacity of the lungs can be observed not only in patients with impaired pulmonary ventilation but even in their absence. This allows us to conclude about the possible pathology of the pulmonary vessels, to determine the scope of the diagnostic search, and to correct the treatment of patients.

Routing of drug supply for military personnel in military medical organizations

Abstract

The article is devoted to the problem of ensuring the continuity and consistency of pharmacotherapy in the treatment or medical rehabilitation of servicemen in the treatment-and-prophylactic and sanatorium-resort organizations of the RF Ministry of Defense. It is proposed to include information on the prescription and use of drugs in the medical record of a serviceman. The presence of these data will provide the doctor with objective information about the previous use of drugs in sequential treatment. The routing and continuity of pharmacotherapy will allow preventing an interruption in the intake of drugs by patients when they go to the place of treatment (medical rehabilitation), to exclude cases of excess delivery (consumption) of drugs. In addition, this will make it possible to record information about the emergence of drug resistance in a serviceman, adverse reactions.

Fleet celebrates the 85th anniversary.

Abstract

The 1469th Naval Clinical Hospital of the Northern Fleet celebrates its 85th anniversary. In June 1936, the department of the Murmansk Naval Hospital in Polyarny was renamed the Northern Fleet Naval Hospital, which became the main hospital for the fleet, equipped with advanced medical equipment and staffed with the best medical personnel to treat the most complex pathologies. The hospital has trained 12 honored doctors of the Russian Federation, 3 doctors of medical sciences, currently there are 11 candidates of medical sciences. In 2020, as part of the fight against the new coronavirus infection, a multifunctional medical center with 100 beds was built as a unit of the hospital, which has a unique radiology department. The Naval Clinical Hospital of the Northern Fleet is the largest medical, scientific and educational center in the North-West of Russia; more than 30 thousand military sailors, members of their families, pensioners of the RF Ministry of Defense, and other privileged categories of citizens receive treatment here annually.

Ivan Rapchevsky (1855–1939) – hygienist, bacteriologist, worker of Russian military medicine

Abstract

The scientific biography of Ivan (Jan Anton) Filippovich Rapchevsky (1855–1939) – a prominent military doctor, bacteriologist, member of the Military Medical Scientific Committee (1896–1918), editor of the Military Medical Journal (1908–1917), author of several developments in the field of military medicine. During the 1880s he carried out priority work on the study of cholera (in Spain) and cutaneous leishmaniasis (in Turkestan). In 1890–1917 he was a full-time employee of the Main Military Medical Directorate, where he headed the work of the department’s central laboratory. During the Russo-Japanese War, disinfection chambers of the I.F.Rapchevsky system were available in all hospitals. The broad international contacts of the scientist are considered, the range of his publications is characterized. In 1923, Rapchevsky moved to Poland, where he continued his work in the field of military medicine.

Formation and evolution in the 1920s–1930s of the concept of the use of medical aviation (Literature review)

Abstract

A review of literature materials devoted to the theory and analysis of application practice in the 1920s – 1930s is presented. aviation as a fundamentally new vehicle. The research carried out was based on the experience of the First World War and the local armed conflicts that followed. Priority was given to the speedy air delivery of the wounded to medical facilities. This factor directly influenced the results of the treatment of combat surgical pathology. However, the list of tasks solved with the help of aviation technology has increased every year. The best practices in the use of air ambulances were regularly discussed at thematic conferences, congresses, and scientific publications. The data on the programs and materials of the first international forums devoted to this problem is given. The active exchange of opinions of specialists, their publication in the press allowed clarifying many theoretical provisions and formulating promising ways of the most expedient use of aviation for medical and sanitary purposes.

Abstract

In August 2021, within the framework of the Army-2021 International Military-Technical Forum, on the territory of the Patriot Military-Patriotic Culture and Leisure Park of the Armed Forces of the Russian Federation, the sites of the Fan Club of the Military Medical Relay competition were deployed. Masterclasses on the provision of medical care for visitors of all age groups were held, visitors were told about the rules and peculiarities of the competitions of military doctors, informed about the participating countries, demonstrated materials about international competitions in military medical all-around. The event was organized by the Main Military Medical Directorate and the S.M.Kirov Military Medical Academy.
