DIFFERENTIAL DIAGNOSTIC FEATURES OF HYPERTENSION IN METABOLIC SYNDROME AND HYPERTENSIVE DISEASE

Abstract

Metabolic syndrome (MS) can rightly be considered as an extra-nosological clinical phenomenon and a starting pathobiochemical and dysmetabolic status of the body, which is subsequently often transformed into cardiovascular and endocrinological diseases and contributes to the damage to target organs, and also frequently to the development of complications with high mortality rates. The paper highlights pathobiochemical and clinical differential diagnostic criteria for hypertension in both MS and hypertensive disease (HD), as well as the characteristics of the formation of MS components in childhood. It explains the necessity of elaborating algorithms and criteria for the differential diagnosis of hypertension in HD and MS, which will help the clinician to diagnose and prescribe the most adequate therapy. Since MS is a reversible condition, correction of dysmetabolic disorders in the early stages contributes not only to lowering blood pressure, but also to preventing their evolution into HD and improving the metabolic profile, by preventing the development of type 2 diabetes.

USE OF L-CARNITINE IN GENERAL MEDICAL PRACTICE

Abstract

The paper presents the results of a number of experimental and clinical studies proving the prospects for using L-carnitine in the clinic of internal diseases. Due to its antioxidant and antihypoxant properties, L-carnitine used in addition to the main etiopathogenetic therapy is prescribed by cardiologists, nephrologists, neurologists, and gerontologists. The experimental studies previously conducted by the authors showed no effect of L-carnitine on cytochrome P450 3A4 activity, which lowers the risk of P450 (CYP) 3A4 metabolic drug interactions. When used as part of comprehensive pharmacotherapy, L-carnitine has an increased safety profile in comorbid patients taking the drug.

A NEW ENDOSCOPIC VEIN HARVESTING TECHNIQUE FOR CORONARY ARTERY BYPASS SURGERY

Abstract

During coronary artery bypass surgery, the great saphenous vein is traditionally harvested via soft tissue dissection along the entire length of the extracted conduit, which is very traumatic and leads to the development of wound complications. The emergence of endoscopic harvesting techniques has substantially could reduce the frequency of the latter. However, the data published and the results of meta-analyses indicate the worst mid-term patency of endoscopically harvested shunts. This is associated with an excessive mechanical impact on the vein wall and with carbon dioxide insufflation. Vein harvesting in the flap with the surrounding tissues, which is called a «notouch» technique, allows virtually eliminating any direct mechanical effect on the venous conduit wall, ensuring the morphofunctional integrity of a future shunt. Due to this, the procedure has the best medium- and long-term results, but it is more traumatic for the lower limb. The technique developed at the Research Institute of Cardiology, Tomsk National Research Medical Center, to endoscopically harvest the great saphenous vein in the flap with the surrounding tissues and that for its traditional open harvesting underwent comprehensive comparative assessment. The study enrolled 315 patients who were prospectively randomized into endoscopic (n = 156) and open (n = 159) vein harvesting groups. Intraoperative data, postoperative clinical course, and features of wound healing were analyzed. Endoscopic harvesting of the greater saphenous vein was shown to reduce the total duration of interventions and the number of wound complications. Thus, the developed endoscopic harvesting technique not only allows one to adhere to the «no-touch» technique, but is also less traumatic for the lower extremities.

Efficacy of venlafaxine in chronic pain syndrome

Abstract

Along with analgesics, adjuvant medications are currently recommended for the treatment of various chronic pain syndromes. The antidepressants with proven analgesic effects include venlafaxine that has a more favorable tolerability profile than tricyclic antidepressants traditionally used to treat patients with chronic pain. Many studies have shown that venlafaxine is highly effective in the therapy of neuropathic pain, while the efficacy of the drug for other types of chronic pain has not been studied sufficiently. Although most current guidelines do not consider venlafaxine as a first-line drug treatment for chronic pain, there are grounds for its use in clinical practice in many cases, for example, those with poor tolerance or contraindications to other antidepressants and anticonvulsants. Further investigations are necessary for more and additional information about the features and spectrum of the analgesic activity of venlafaxine.

Acute isolated sphenoiditis: tactics and treatment

Abstract

Isolated sphenoiditis is a rare condition and is often undiagnosed due to scarce non-specific clinical signs. The acute onset of the disease is represented mainly by neurological disorders, such as headache, diplopia, and cranial neuralgia. Rhinological manifestations of the disease are extremely rare. The diagnosis of isolated sphenoiditis is based on radiological findings (computed tomography (CT), magnetic resonance imaging, and paranasal sinus (PS) radiography. By taking into account the proximity of the sphenoid sinus to the vital structures (pituitary, internal carotid artery, pairs of cranial nerves III, IV, V, VI), PS CT are of the greatest diagnostic and tactical value, which allows for a detailed assessment of not only the basilar sinus, but also the adjacent intranasal structures. Timely diagnosis can prevent the development of important intracranial complications, such as meningitis, cavernous sinus thrombosis, brain abscess, subdural abscesses, episcleritis, and retrobulbar neuritis. Therefore, patients with isolated sphenoiditis should be examined by a neurologist and, in the presence of visual impairment, by an ophthalmologist at the prehospital stage and during treatment. Endoscopic endonasal sphenotomy is the optimal treatment for isolated sphenoiditis. Expanding the natural ostium of the basilar sinus and the nasal cavity restores mucociliary function, evacuates the pathological content in the sinus, and permits the patient to achieve stable remission. The paper describes a clinical case of acute isolated sphenoiditis in a 25-year-old patient with migrainelike headache. This case shows that even timely initiated medical therapy is insufficient in some cases; there is a need for surgical intervention.

The health status of patients with diabetes mellitus (DM) and their quality of life directly depends on their daily well-being and on the uncomfortable conditions that may not be so noticeable in the presence of macrovascular complications, but nevertheless, can cause great suffering. The cause of daily discomfort can be dry eye syndrome (DES), a set of corneal and conjunctival changes caused by systematic instability of the tear film. The current treatment and prevention strategies for DES involve the use of drugs that moisturize the eye surface. There is a wide range of tear substitutes on the market; however, when making a choice, it is important to remember that the drug should be long present on the eye surface and therefore the absence of drug components can have a negative impact on the eye.

Disseminated small-cell lung cancer in a pulmonologist’s practice

Abstract

Difficulties in diagnosing disseminated lung diseases are explained by the presence of similar nonspecific manifestations in different etiologies, pathogenesis, and morphology. Patients with disseminated small-cell lung cancer (SCLC) are long followed up and treated if they are diagnosed as having disseminated tuberculosis, pneumonia, and alveolitis. SCLC is characterized by rapid progression, early lymphogenic and hematogenous metastastic spread. The given case demonstrates objective difficulties in a differential diagnostic search from the moment of hospital admission to the final diagnosis. Despite the widespread use of highly informative (both visualization and laboratory) methods for examining patients with pulmonary diseases, the differential diagnosis of disseminated lung processes leads to problems in a physician’s practice. Lifetime verification of pulmonary disseminations is not always possible, which creates prerequisites for their further comprehensive study.
