Fibular canal syndrome: modern principles of diagnosis and treatment

Abstract

Fibular tunnel syndrome is the most common tunnel syndrome of the lower extremity. Compression of the fibular nerve most often occurs at the level of the fibular head. The article describes in detail the etiology and pathogenesis of this tunnel neuropathy. Clinical manifestations of fibular tunnel syndrome are considered, methods of diagnosis and treatment are described. The prognosis of peroneal nerve neuropathy is favorable, and in most patients there is a complete or almost complete restoration of nerve function. If conservative treatment is ineffective, surgical techniques are recommended.

Problem

The state of the psychoemotional sphere and coping strategy in therapists during the pandemic of the novel coronavirus infection COVID-19

Abstract

Objective. To establish the features of the state of the psychoemotional sphere and the coping strategies being implemented among medical workers of therapeutic specialties of COVID hospitals, medical personnel of polyclinics, and medical personnel of multidisciplinary hospitals during the COVID-19 pandemic.

Subjects and methods. The investigators examined the following groups of physicians: 1) 111 physicians of therapy specialties (TS) in the COVID hospitals; 2) 106 TS physicians in the multidisciplinary hospitals; and 3) 102 TS physicians in the polyclinics; and 4) 190 employees of engineering, technical, and economic specialties who are not related to work in the healthcare facilities (these are healthy). The development of psychoemotional disorders in TS physicians was studied by the integrative anxiety test (IAT). The coping strategies were assessed using the procedure designed by R. Lasarus and C. Folkman.

Results. The work of TS physicians during the COVID-19 pandemic was related to a number of specific risk factors associated with the nursing of infected patients, as well as with significant changes in the organization of medical activity and its safety, which had contributed to an increase in stress levels, anxiety, and the use of coping strategies.

Conclusion. To assess the state of the affective and anxiety spheres in the TS physicians, it is recommended to use IAT and to assess the asthenic component of anxiety, emotional discomfort and promises. Also, when evaluating the severity of psychological reactions in TS physicians, coping strategies are recommended, by using the questionnaire "Coping Strategies" designed by R. Lasarus.

Every day a practicing physician faces various clinical and laboratory syndromes when supervising the patients. To make a differential diagnosis of many conditions and diseases, the physician must have not only practical experience, but also a broad outlook, including that in a number of related specialties. It is increasingly necessary to attract a multidisciplinary team in solving diagnostic puzzles.

The presented clinical case considers the stages of a diagnostic search in a patient with abdominal aortic aneurysm. The found etiology of this condition has changed the patient's further management tactics.

Psychodermatology: practical aspects of diagnosing autodestructive dermatoses on the example of our own clinical observations

Abstract

The study of the features of the relationship between the mental state of the patient and skin pathological processes at the present stage is especially relevant. This is due not only to an increase in the prevalence of psycho-dermatological comorbidity, but also to the pathogenetic features of the formation of the clinical picture of this pathology, which make it difficult to diagnose. Skin lesions can either be a consequence of a primary psychiatric illness or induce the development of concomitant psychosocial illnesses. At the same time, mental disorders masked by skin manifestations combine, as a rule, skin lesions due to self-harm. Their clinical picture can mimic the symptoms of a number of nosologies, including not only dermatological pathology, but also systemic diseases with skin manifestations. The artificiality of rashes may not always be obvious, which often leads to the erroneous involvement of doctors of various specialties in the diagnostic search and prolongs the time for making the correct diagnosis.

The article presents our own clinical observations of patients with autodestructive dermatosis, in which the verification of the diagnosis was accompanied by diagnostic errors, which caused pronounced destructive changes. In the analysis of the above clinical observations, attention is focused on the features of the skin pathological process, aspects of differential diagnosis, as well as insufficient awareness of primary health care professionals about this problem. Increased awareness of doctors of related specialties can contribute to clinical alertness, reduce the time of diagnosis and timely start of therapy for these patients by a psychiatrist, which will reduce the risk of an increase in the severity of the disease and social exclusion of patients.

Streptobacillosis (haverhill fever): clinical case analysis

Abstract

The article presents a case of infection caused by Streptobacillus moniliformis, demonstrating the features of the clinical picture of the disease from the group of zoonoses, which are extremely rarely diagnosed in the Russian Federation. The source of the infectious agent was a gray rat purchased at a pet store. The disease was characterized by fever, symptoms of intoxication, rash with characteristic localization, and the development of arthritis. The described clinical case demonstrates the importance of epidemiological history, the complexity of clinical and laboratory diagnosis of systemic zoonotic infection with the risk of disability due to the development of bacterial arthritis. Timely diagnosis is hampered by difficulties in cultivating the pathogen and the lack of widely available diagnostic test systems.

Features of mild duration asthma diagnostics in childbearing aged females

Abstract

Asthma diagnostics especially of it’s mild forms, is considered to be a serious problem within primary health care unit due to the absence of exact evaluation algorithm of subjective, clinical and functional manifestations.

Objective. The goal of our study was to create recommendation on early asthma diagnostics in childbearing aged females, based on particular analysis of complaints, clinical and functional test results and to make them available to all primary medical care units.

Materials and methods. 2460 females of childbearing age with various severity of asthma duration were studied with complex of clinic and functional methods.

Results. Patients with various grades of asthma severity could be compliant free, same time the can appear signs of bronchial obstruction during auscultation. The marked discordance leads to mismatch in therapy administration. PEF50 decrease while initial spirometry and its’ increase after probe with bronchodilator is considered to be the earliest sign of bronchial obstruction. Asthma diagnostics at prehospital stage is to based upon complete chart collection, complaints analysis, auscultation signs while forced expiration as well as on spirography data on volume and forced expiration dynamics and PEF50.

Conclusion. Results of large population study on clinical feature of asthma duration and functional diagnostics of mild asthma duration could be recommended for primary health care physicians as method of early asthma diagnostics and therapy administration.

Effectiveness and safety of vitamin U in patients with dyspepsia in real clinical practice

Abstract

The relevance of improving treatment methods for patients with dyspepsia is important due to widespread prevalence, negative impact on the quality of life and regularly updated pathogenetic conception of this syndrome.

Objective. To evaluate the effectiveness and safety of methylmethionine sulfonium chloride as part of combination therapy in patients with epigastric pain in real clinical practice.

Materials and methods. An anonymous, retrospective, non-interventional clinical study of 408 patients with a primary diagnosis of dyspepsia (K30.0) treated with a combination of omeprazole (20 mg) and methylmethionine sulfonium chloride (300 mg) or omeprazole (20 mg) for 30 days for epigastric pain syndrome.

Results. In patients receiving combination therapy, epigastric pain was relieved in 80.1% versus 68.3% in patients receiving monotherapy (p=0.007). Adverse effects of therapy in the group of patients receiving vitamin U occurred in 13.1% versus 6.9% in the group of patients receiving omeprazole monotherapy (p=0.03), but this did not affect patient adherence to treatment.

Conclusion. The reliable effectiveness of methylmethionine sulfonium chloride as part of combination therapy in patients with dyspepsia in real clinical practice demonstrated.

Spleen lymphoid tissue in white mice with an albumin model of experimental amyloidogenesis

Abstract

Objective. Study the reaction of the white pulp of the spleen of laboratory mice in an experiment on parenteral administration of human pharmacy albumin.

Materials and methods. Nine 30-day-old mice were divided into three groups of three mice each (intact and two experimental). Experimental mice were intraperitoneally injected with a human albumin pharmacy drug at a dose of 0.9 ml every other day for 30 days. The animals of the intact and first experimental groups were removed from the experiment by decapitation on the 30th day from the beginning of the experiment, the second experimental group – 1 month after the last injection (60 days from the beginning of the experiment).

Results. Albumin increased the wet mass of the spleen from 0.74±0.01 g (control) to 1.40±0.02 and 1.20±0.04 g in groups 1 and 2, respectively; induced amyloidosis, determined by the relative area of the congophilic substance in 4 microns of paraffin sections of the spleen – 13.32±1.31 and 23.47±0.84% in groups 1 and 2, respectively; reduced the average area of lymphoid nodules from 1.34±0.10•105 mkm2 (control) to 1.18±0.08•105 and 0.91±0.04•105 mkm2 in groups 1 and 2, respectively; increased the relative area of white pulp – from 22.65±2.30% (control) to 34.99±2.34 and 31.87±1.68% in groups 1 and 2, respectively, and reduced the relative area of the red pulp – from 77.35±4.20% (control) to 65.01±2.34 and 68.13±1.68% in groups 1 and 2, respectively, which led to a significant decrease in the index (red pulp) / (white pulp) from 6.67±1.39 (control) to 2.79±0.42 and 2.65±0.29 in groups 1 and 2, respectively. In all cases, the differences are statistically significant.

Conclusions. Parenteral administration of human albumin to mice at a dose of 0.9 ml of a pharmacy drug causes amyloid damage to the spleen. Discontinuation of the introduction of amyloidogen protein does not prevent the development of the amyloid process that has begun.

A personalized approach to discharge and follow-up of patients with COVID-19

Abstract

Criteria for laboratory screening of patients with COVID-19 in hospital settings are proposed to justify treatment, discharge and terms of dispensary follow-up. Optimal approaches are based on the determination of viral load, SARS-CoV-2 viability and serum ferritin level in the dynamics of the disease. High viral load is a criterion for an unfavorable outcome, ferritin level >200 mg/l indicates possible virus persistence. Assessment of SARS-CoV-2 virus viability substantiates a period of infectiousness in COVID-19 patients. At the same time, a longer excretion of the virus with feces is not excluded, which requires further studies to establish or exclude an additional fecal-oral transmission mechanism.

Non-alcoholic fatty liver disease and its role in the development of cardiovascular diseases

Abstract

Objective. To evaluate the contribution of non-alcoholic fatty liver disease (NAFLD) to the pathogenesis of cardiovascular complications, as well as to highlight the main approaches to the diagnosis and treatment of NAFLD based on modern literature data.

Key points. NAFLD is the most common chronic liver disease (in developed countries – up to 25–30% of the population). NAFLD is a significant risk factor for insulin resistance (IR), carbohydrate metabolism disorders, dyslipidemia, and adverse cardiovascular outcomes. Treatment of NAFLD includes an impact on the main links of pathogenesis, therefore the main therapeutic strategy is lifestyle changes, weight loss and elimination of IR, the prescription of hepatoprotectors and herbal medicines.

Conclusion. Being a multifactorial disease leading to increased cardiovascular risk, NAFLD should be detected in a timely manner. Treatment of NAFLD can slow the progression of liver disease and reduce the risks of cardiometabolic complications.

The function of right cardiac chambers in patients with asthma according to doppler echocardiography

Abstract

Objective. To determine the functional state of right cardiac chambers in patients with asthma of varying severity according to Doppler echocardiography.

Subjects and methods. Cardiopulmonary hemodynamics was assessed in 56 asthmatic patients, by using ultrasound. The examinees were divided into 3 groups by the severity of the disease.

Results. With an increase in the severity of asthma, diastolic dysfunction of the right ventricle (RV) progressed with the development of its dysfunction that further plays a dominant role in developing chronic heart failure. As diastolic dysfunction of the RV progressed in the asthmatic patients, there was an increase in the contractile activity of the right atrium (RA) simultaneously with a decrease in the Е/А ratio.

Conclusion. In asthmatic patients, diastolic dysfunction in the right cardiac chambers frequently precedes systolic dysfunction, which manifests itself in a change in the ratio of various phases of RV filling. During RV diastolic dysfunction, there is a relative increase in the contractility of RA.

Therapy of dysrhythmias in young women with estrogen deficiency, arterial hypertension, and "metabolically healthy" abdominal obesity

Abstract

Objective. To study the frequency and type of dysrhythmias in young women with arterial hypertension (AH), hypoestrogenemia and «metabolically healthy» abdominal obesity (MHAO), as well as the efficacy of antiarrhythmic drug Allaforte in this cohort of patients.

Material and methods. The study included 29 patients with AH, hypoestrogenemia and MHAO, who received the antiarrhythmic drug Allaforte in standard therapy. Patients were comparable in age, severity of course and duration of AH, and therapy. At the beginning of the study and after 4 months of therapy with Allaforte the patients underwent 24-hour ECG Holter monitoring.

Results and conclusion. When analyzing the nature and structure of dysrhythmias, it was found that supraventricular extrasystoles, including frequent and allorhythmic ones, are quite often registered in women with AH, estradiol deficiency and MHAO. Similar trends can be traced with respect to the frequency of ventricular arrhythmias. The inclusion of Allaforte in the therapy contributes to the decrease in the number of extrasystoles, normalization of heart rhythm and improvement of general well-being of the patients.

Correlations of systemic blood cytokines in patients with coronary heart disease and experienced COVID-19

Abstract

Coronavirus infection brings an extreme threat to older people and particularly to patients with severe cardiovascular pathology, including coronary heart disease (CHD). Recovery in these patients largely depends on the severity of cytokine disorders in the acute phase of the disease, since they negatively affect cardiomyocytes, including those through the development of complications in other organs and systems. However, changes in the plasma cytokines of elderly patients with CHD who have experienced COVID-19 have not been studied in practice.

Objective. To study the correlations of plasma cytokines in older patients with CHD in the early recovery stages after COVID-19.

Subjects and methods. The investigation enrolled 40 elderly patients with CHD at weeks 3-4 following recovery from COVID-19 and 38 patients of the same age with CHD without COVID-19 in their history. The fasting plasma cytokine level was determined using a Becton Dickinson FACS Canto 2 apparatus (USA) in the morning. The relations between the cytokines were studied by the correlation method.

Results. Interleukin (IL)-6 showed the greatest number of significant correlations (direct weak and medium associations with other cytokines), including a direct statistically significant average correlation with IL-17 (r=0.63), tumor necrosis factor-α (TNF-α) (r=0.42), interferon-γ (IFN-γ) (r=0.39), IL-2 (r=0.35), IL-3 (r=0.33), as well as a weak significant relationship to IL-7 (r=0.29). IFN-γ had three direct significant correlations with IL-6 (r=0.39), IL-7 (r=0.36), and TNF-α (r=0.29). The latter showed two direct average significant correlations with IL-6 and IL-17.

Conclusion. In the early stages of recovery, in 60–74-year old patients with CHD after experienced COVID-19, IL-6, IFN-γ, and TNF-α cytokines had the greatest conjugacy, which can be used as markers of recovery in this patient cohort.

Musculoskeletal pain in adolescents with recurrent cephalgia: age and sex features, treatment and prevention

Abstract

A review of information about musculoskeletal pain in adolescents is presented: the causes of their development, prevalence, classification, clinical manifestations, treatment and prevention. Included are the results of our own research, which showed the peculiarities of the relationship between the frequency of occurrence and the structure of these types of pathology in adolescents with different variants of the course of recurrent cephalalgia – episodic (rare) and chronic (frequent). Frequent dorsalgia in adolescents is significantly more often associated with chronic cephalgia (32.0%), and rare ones – with episodic (46.9%). Girls have a higher incidence of frequent dorsalgia associated with chronic (37.1 and 14.3%) and episodic (22.2 and 16.4%) cephalgia. The authors come to the conclusion about the need for early diagnosis, prevention and timely treatment of dorsalgia associated in adolescents with chronic and episodic cephalgia, and highlight approaches to the treatment and prevention of musculoskeletal pain in children and adolescents.

Assessment of comorbid pathology in patients with breast cancer

Abstract

The incidence of cardiovascular diseases associated with the treatment of malignancies varies widely depending on the antitumor therapy, its duration, and concomitant diseases of a patient.

Objective. To assess comorbid pathology in patients with breast cancer (BC).

Material and methods. The study included 100 women with breast cancer aged 30 to 80 years. The patients were divided into 5 groups according to their age. The following were assessed: primary and concomitant diseases, history of the primary disease, results of instrumental methods of investigation – ECG, chemotherapy, radiotherapy, and metabolic therapy. The Charlson comorbidity index was calculated, and the one-year death risk and 10-year survival rate in patients with BC were assessed.

Results. The following comorbid pathologies were the most common in the study group patients: hypertension, angina pectoris, chronic gastritis, and myocardiodystrophy. Increasing prevalence of comorbid pathology, significant growth of Charlson comorbidity index score, and deterioration of prognostic indexes were revealed in patients older than 40.

Conclusion. In patients with BC aged 40 and older, cardiovascular pathology, particularly hypertension, angina pectoris, myocardiodystrophy, prevails in the comorbidity structure. Patients most often had a combination of cancer with two or more comorbidities. A significant increase in the Charlson comorbidity index score was observed with age in all patients with BC. According to the prognostic value of the Charlson comorbidity index, 56% of the studied patients with BC have a high risk of one-year mortality and a low 10-year survival rate.

Cardiotoxic effects of antitumor therapy for colorectal cancer

Abstract

Fluoropyrimidines (5-fluorouracil, capecitabine) are the main chemotherapeutic agents in the treatment of gastrointestinal oncologic pathology. However, therapy with these drugs may be associated with the development of cardiotoxic effects. The article discusses risk factors, the main proven mechanisms of cardiotoxic effects development; possible measures of primary and secondary prevention, peculiarities of fluoropyrimidines cardiotoxicity treatment are outlined.

Clinical cases are presented that demonstrate the complexity of the pathophysiological mechanisms of development of fluoropyrimidine (capecitabine) cardiotoxicity and the difficulties of its prediction. In this case, drugs with a vasodilating effect can be effectively used as part of secondary prevention.

Silverman–andersen respiratory scale in a comprehensive assessment of respiratory distress syndrome of premature newborns: clinical and prognostic role in the early neonatal period

Thrombotic and thromboembolic complications of a new coronavirus infection COVID-19 a medical worker (a clinical case with an unfavorable outcome)

Abstract

The presented clinical case describes severe complications of a new coronavirus infection (COVID-19) in a comorbid patient with obliterating atherosclerosis of the arteries of the lower extremities, varicose veins of the lower extremities, hypertension, in the form of late thrombosis of the arteries of the lower extremities with the development of gangrene, which was the basis for amputation of the limb. The development, in the future, of complications such as: thromboembolism of the branches of the pulmonary artery and acute left ventricular failure, which caused a fatal outcome.

The principles and results of the medical examination conducted in order to establish a causal relationship of the disease with the professional activity of a medical worker are presented. The need for a multidisciplinary approach to the early diagnosis, prevention and treatment of COVID-19 and its complications, improving the mechanisms of admission to the professional activities of medical workers taking into account age and comorbid conditions in conditions of high risk of infection with SARS-CoV-2 is emphasized.

Review of dissertation research on the effectiveness of complex physiotherapy

Abstract

Today, natural and preformed physical factors occupy a special place in medical rehabilitation. For many years, a large number of experimental and clinical studies have been carried out, including dissertations, which have proved the high effectiveness of the use of the above factors in the treatment of patients with various diseases.

Objective. To review dissertations that are devoted to the study of the effect of the complex application of physical factors in patients with diseases of the musculoskeletal system, neurological, and urological diseases for the period 2000–2020.

Materials and methods. We have analyzed publications in the disserCat scientific electronic library of dissertations and abstracts over the past 20 years, which are devoted to the study of the use of complex physiotherapy in the medical rehabilitation of patients with diseases of the musculoskeletal system, neurological and urological diseases.

Conclusion. Currently, there is a fairly serious evidence base confirming the effectiveness of complex physiotherapy in the medical rehabilitation of patients with diseases of the musculoskeletal system, neurological and urological profile.
