PHARMACOTHERAPY IN PATIENTS WITH COMORBIDITY: THE POSSIBILITY OF DRUG INTERACTIONS AT THE LEVEL OF METABOLISM

Abstract

Affective disorders are implicated in the pathogenesis of somatic diseases, such as hypertension, angina pectoris, myocardial infarction, asthma, etc., or aggravate their course. Thus, combination pharmacotherapy with drugs from different groups is used in patients with comorbidity. The CYP3A4 isoenzyme is known to metabolize about 70% of all known drugs. There are many CYP3A4 inhibitors among antidepressants. If a drug substrate is co-administered with a CYP3A4 inhibitor, the metabolism of the substrate slows down and it can be accumulated in the body; this increases the risk of adverse reactions. When comorbidities are treated, drugs should be prescribed with regard to metabolic pathways, which will make it possible to predict a priori undesirable interactions and to enhance the safety of performed combination therapy.

COMPLIANCE IN THE THERAPY OF BABIES WITH TRANSIENT LACTOSE INTOLERANCE

Abstract

Whether dietary guidelines are followed has been studied by observing 22 breastfed babies aged 1 to 6 months with transient lactose intolerance (TLI). Data on the patterns of their feeding and care and the quality of treatment compliance are analyzed. TLI is shown to be a disease that requires that dietary guidelines and replacement therapy should be sequentially and long complied with. 63.6% of cases could achieve complete compliance, which has allowed the quality of life to be improved in not only a baby, but also in his/her entire family. The negative results of treatment are associated with both subjective and iatrogenic factors.

The incidence of mycotic infection was studied in 77 children with cancers after polychemotherapy (PCT). Multiple courses of high-dose PCT, antibacterial therapy, long-term bone marrow aplasia, frequent forced corticosteroid use and, possibly, untimely diagnosis of the infection have a considerable impact on the development of mycotic infection. Abstract

During a 5-year follow-up of elderly patients, the likelihood of transformation of nonerosive reflux disease (NERD) to erosive reflux disease (ERD) or Barretts esophagus (BE) was 46,4 and 2,5%, respectively. ERD was a stable form in 66,8% of the patients, it transformed to NERD or BE in 26,0 and 6,4%, respectively. Lower third esophageal dysplasia occurred in 8,1% of the patients with BE during the follow-up. In the elderly patients, the leading risk factors of progressive gastroesophageal reflux disease were the absence of maintenance therapy with proton pump inhibitors (odds ratio (OR) = 6,2%; confidence interval (CI), 1,8-8,8), abdominal obesity (OR 3,1; CI, 2,3-3,9), tobacco smoking (OR 2,3; CI, 1,5-3,1); regular alcohol use (OR, 2,0; CI, 1,1-2,7).
