Sagittal morphology of the spine and its predictive role in the pathogenesis of degenerative pathology. Historical aspects and modern view of the problem

Human upright posture is associated with constant maintaining of horizontal sight and ergonomic vertical posture. These physiology characteristics of Homo sapiens are feasible thanks to three reciprocal sagittal curvatures of the spine. The studies revealed that their profiles feature great variability in healthy individuals. The problem of systematizing the sagittal morphology of spinopelvic complex, given its constitutional ambiguity, has been the subject of scientists’ close attention since the mid-1970s. The classification suggested by P. Roussouly in 2005 can be considered the quintessence of the accumulated world experience. The author systematized the profile configuration of the spine, but also paid significant attention to the biomechanical specificity of its morphotypes and their predictive role in the pathogenesis of degenerative diseases. The problem of variability of sagittal morphology and its morphotyping has not lost its relevance, and to this day remains the subject of numerous scientific discussions. However, it is difficult to draw an overall view to this problem due to the narrow focus of the conclusions and the lack of systematizing works. This review is analytical in nature and used the studies retrieved from resources PubMed and eLibrary. The research investigates the historical aspects of the formation of spinopelvic complex morphotyping and analyzes the predictive role of the spine sagittal biomechanics in the pathogenesis of its degenerative pathology.
