DYNAMICS OF START OF CONDUCT DISORDERS IN ADOLESCENTS: CONNECTION WITH PSYCHOACTIVE SUBSTANCE USE

Abstract

293 adolescents with conduct disorder and the comorbid use of opioids, volatile solvents, alcohol and the associated use of stimulants and cannabinoids were studied. It was found that in patients with the early onset of the disease medical and psychiatric consequences of substance use have dominated and them there were more significant quantitative characteristics of this abuse, while the later substance onset has shown more pronounced social and legal consequences with larger meanings of alcohol use.

LEADING COPING MECHANISMS IN PATIENTS WITH THE TEMPOROMANDIBULAR JOINT PAIN DYSFUNCTION SYNDROME

Abstract

There have been determined the leading coping mechanisms and their interaction with neurotic symptoms and with the quality of life of patients with the TMJ pain dysfunction syndrome. The results have shown that various facial pains, increased tone of chewing muscles, depressive complaints and anxiety hamper dramatically the formation of successful therapeutic alliance and adjustment. It is necessary to develop a diagnostic algorithm for psycho-emotional disorders in patients with TMJ and muscle dysfunctions and to carry out a timely and adequate correction of the disorders according to their intensity.

Р 300 IN THE DIAGNOSIS OF COGNITIVE IMPAIRMENT AT MULTIPLE SCLEROSIS

Abstract

With the aim of early diagnosis of multiple sclerosis there were studied neurophysiological peculiarities of cognitive disorders. Clinical- neurophysiological examination of patients with recurrent multiple sclerosis revealed mild disability according to the EDSS scale. The number of patients with an average degree of gravity prevailed in secondary progressive MS, with an increase in 3 times, and with a heavy degree of 5.5 times. Neurophysiological study revealed that at different types of the disease course the reduction of the cortical response and longer latency (PL) period of cognitive wave P300 occur. Decrease in the amplitude of the response wave P 300 indicates reduced activation of neurons involved in the processing of the signal in consequence of the processes of demyelination.

THE HAND DYSFUNCTION IN PATIENTS WITH ACUTE NON-TRAUMATIC INTRACEREBRAL HEMORRHAGE

Abstract

We investigated frequency and causes of the hand dysfunction in patients with intracerebral hemorrhage (ICH) in the acute period. The most frequent cause of the hand dysfunction (more than 60 %) was paresises and paralysises, both isolated and complex. They provoked the most severe case of reduced mobility of the hand. Apraxia was the second cause of the hand dysfunction in the patients with acute intracerebral hemorrhage. The depth localization of ICH was connected with more pronounced functional impairments than superficial ICH.

THE USE OF SUCCINATES FOR THE CORRECTION OF METABOLIC DISORDERS IN THE PENUMBRA IN PATIENTS WITH STROKE

Abstract

The aim of the study was to determine the quantitative content and correlation between metabolites in the core ischemic zone, the ischemic penumbra and the intact tissue of the opposite hemisphere in the acute stage of ischemic stroke using MR-spectroscopy. We studied 72 patients, including 37 patients (the main group) treated with the antioxidant mexidol (2-ethyl-6-methyl-3-hydroxypiridine succinate) in addition to standard therapy and 35 patients who received standard therapy only. The severity of stroke was assessed using the NIHSS. The Barthel index and Rankin scale were used to evaluate functional outcome in the 30th day, hydrogen MR-spectroscopy was used twice: in the first 24 h from disease onset and in the 5th day. Mexidol used in dose 500 mg intravenously during 14 days reduced significantly the content of lactate (p=0.002) and inositol (p=0.005) in the cells compared to the control group that enhanced the restoration of the balance between aerobic and anaerobic mechanisms of oxidation and positively influenced on the rehabilitation of patients. Positive correlations between the content of lactate in the ischemic penumbra and NIHSS scores (r=0.5786; p=0.049) and the negative correlation between the content of lactate in the ischemic penumbra and the degree of functional restoration assessed by the Barthel index (r= -0.6305; p=0.028) were found. These findings support the evidence for the interaction between the disturbance of glucose metabolism in conditions of hypoxia and the degree of nervous tissue damage.

Therapy strategies of treating withdrawal syndrome (pharmacotherapy, duration of hospitalisation) and recommendations after that were totally different in Russian and in Germany.
