复杂性十二指肠溃疡：临床表现、诊断和治疗的特殊性

根据国内外文献资料，对复杂性十二指肠溃疡患者的临床表现、诊断和手术治疗特点进行了研究和总结。在科学电子图书馆 eLibrary.Ru、俄罗斯国家图书馆和 PubMed 电子搜索系统等数据库中搜索并分析了 1926 年至 2023 年期间的科学医学文献。重点是在循证医学概念框架内获得的文献数据。研究发现，在十二指肠溃疡病患者中，消化性溃疡的发生率为 3.6%-17%。消化性溃疡的临床病程往往不典型，容易出现严重的并发症，如溃疡出血、 失代偿性十二指肠狭窄、溃疡穿孔等严重并发症。手术干预技术难度大。尽管诊断能力不断提高，但在大多数情况下，溃疡在球部后区域的确切位置只有在十二指肠术中修整时才能发现。复杂溃疡患者的手术治疗与重要解剖结构损伤、术后并发症和高死亡率的高风险相关。复杂性胰腺溃疡的现有手术治疗方法伴随着术中和术后并发症风险的增加，如：胆管和胰管、大血管的损伤，术后急性胰腺炎的发展、十二指肠残端衰竭，从而导致经常出现不利的近期和远期效果以及高死亡率。选择最佳技术进行复杂宫颈溃疡手术治疗的问题至今仍未得到明确解决。大量技术的存在说明每种技术都不完美。总的来说，消化性溃疡病及其并发症问题在现代外科手术中仍然具有现实意义。与胃溃疡和十二指肠溃疡相比，关于复杂球部溃疡、其临床表现特征、诊断和治疗的著作很少。
