Abstract

The article deals with problems of using the principles of evidence-based medicine in radiology; it discusses the main problems which a doctor faces when searching for the most effective diagnostic method. Primary criteria for evaluating the efficiency of visualization methods and levels of evidence are discussed.

Abstract

The correspondence of basic principles of laboratory diagnostics to evidence-based practice is discussed. The basis of any test used in practice is a shift of the measured value distribution in comparison with the healthy population. Temporary diagnostic procedures are limited by "threshold" concept for individual parameters. Significant improvement of the model can be achieved by taking into account changes in correlations between parameters.

Evidence for anticytokine therapy of rheumatoidarthritis

Abstract

Medical management of rheumatoid arthritis (RA) is aimed at controlling or reducing the inflammatory process in order to prevent or slow down damage to the joint and to maintain or improve the functional status. However, it is well established that inflammation indicators may be stable or improved, whereas radiographic evidence shows that joint damage progresses. A new class of agents termed "biological agents" have been approved for use in RA according to the data of the evidence-based medicine. Until more is known about the long-term safety and efficacy of these drugs, they should be reserved for patients with severe conditions who are progressing despite adequate doses of methotrexate or other DMARD.

Abstract

Comparative clinico-economic characteristics of application of the modern antiviral medications arabidol, anaferon and immunal in treatment of children is provided in the article. Index of the economic efficacy was evaluated for these medications taking into consideration total expenses for the period of average duration of every insurance case. Application of interferons in the complex therapy of acute respiratory viral infections was recommended from medical and socio-economic positions.

Abstract

122 patients with acute myocardial infarction participated in the study. They received standard therapy and Asparcam-L or Panangin, or potassium and magnesium asparaginate. Efficiency and safety was studied with ECG, echocardiogram, quality of life questionnaires and laboratory methods. Asparcam-L presented good tolerance, safety and efficiency during acute myocardial infarction treatment, as well as a better prognosis.

Abstract

We studied pathomorphological and homeostatic changes taking place in the macroorganism in experimental pulmonary melioidosis when bioassayed animals were infected with strains of varying virulence. As a result, animals infected with melioidosis causative agent with different antigenic composition show different clinical and pathomorphological characteristics of the course of infectious process according to homeostasis indices.

Chronophysiological peculiarities of pain and tactilesensations of stomatological patients

Abstract

Peculiarities of chronophysiological rhythms of temperature, electrical and mechanical pain thresholds and two-point discriminative tactile thresholds of oral mucosa were evaluated. Comparative analysis showed a similarity between chronophysiological rhythms of pain perception and tactile sensation from 8 a.m. to 8 p.m. It was established that the extremes of pain and two-point discriminative tactile perception develop from 12 a.m. to 2 p.m., and minimum perception is at 8 a.m. and from 6 p.m. to 8 p.m. Correlations between temperature, electrical and mechanical pain thresholds were revealed, as well as correlations between these types of pain sensations and tactile perception of oral mucosa. These results allow a prediction of individual level of pain sensation during dental treatment.

Abstract

Examination of the 32 healthy parturient women and 57 cases with infectious complications of the postpartum period demonstrated a correlation between the increased level of pro-inflammatory cytokines, especially tumor necrosis factor, and endotoxin in the serum of the patients with a severity of infectious complications. The sensitive markers of severity of infectious complications were discovered in this research.

Postpartum infectious complications from the positionsof the systemic inflammatory response

Abstract

High frequency of postpartum infectious complications makes them one of the most actual problems in modern obstetrics. Investigation of 186 parturient women with post-parturition infectious complications revealed elevated level of cytokines ((IL-1, IL-6, TNF) and endotoxin in the serum of the patients associated with the changes in the albumin metabolism and neuro-humoral regulation manifested by the changed level of Cortisol and prolactin. Inclusion of trascranial electrostimulation and medication dibicor in the complex treatment showed marked clinical effect and provided fast restoration of physiological level of regulation. The results of the research were evaluated from the positions of the systemic inflammatory response development.

Abstract

Health care reform presupposes its structural reorganization with incorporation into the medical establishment of specialties that are new for domestic medicine: social work and medical psychology. The changes of traditional Russian culture of healing embodied in the image of a perfect zemsky physician are taking the shape of dramatic overwork for practicing physicians, including some functions non-medical in essence and requiring specialist training. Further labour division taking place within the medical speciality expands the whole health care system by means of introducing professions with non-medical educational background, which, however, contribute greatly to the efficacy of medicine and improvement of health and life quality.

Serum levels of interleukins, tumor necrosis factor, markers of endotoxycosis and peritoneal exudates were assessed in 36 patients with acute purulent diseases of pelvis minor with intra-abdominal perfusion of perftoran and 25 controls. The evaluation of laboratory and clinical findings in two groups demonstrated the effectiveness of perftoran administration by decrease in studied indices, extent of complications and duration of hospital stay.

In this study we analyzed the CVH infection epidemiology and efficacy of CVH therapy with "Peginterferon-Alpha 2a" and "Ribavirin". "Pegasys" and "Ribavirin" therapy was more effective than "Roferon" and "Ribavirin" therapy in patients with CHV type 1 in our region. Predictors of efficacy endpoint (CVH RNA reduction) were determined partially.
