SCIENTIFIC APPROACHES TO COMBINATION THERAPY FOR TYPE 2 DIABETES MELLITUS

Abstract

Type 2 diabetes mellitus is a heterogeneous disorder characterized by insulin resistance and progressive disturbance of insulin secretion starting with pancreatic beta cell dysfunction to full atrophy. Therefore a rational influence on all chains of pathogenesis of the disease results in compensation of carbohydrate metabolism. Monotherapy of type 2 diabetes mellitus is effective only for a short period. Therefore the treatment of type 2 diabetes mellitus with a combination of oral hypoglycemic agents leads to appropriate glycemic control. The review throws light on optimal combinations of oral hypoglycemic drugs taking into consideration their molecular mechanism of action.

The influence of sauna bath procedure in the morning (8 AM) on the external respiration, gas-energy exchange and cardiovascular system of 10 judo sportspersons performed on the next day after two incremental maximal cycle ergometer tests has been studied. It has been established that after the morning sauna bath procedure the maximal O2 uptake significantly increased the on the next morning.

Abstract

An advantage of heart bypass in patients with ischemic heart disease with hemodynamically significant lesion of coronary arteries from the point of view of clinical effectiveness and expenses per year of life considering the life quality is shown. When planning a treatment of ischemic heart disease in patients with diabetes mellitus one should consider the findings of clinicoeconomical analysis. According to our study, patients who had heart bypass showed the best clinical outcomes and a lesser cost of surgery and both expenses in the early postoperative period and the cost of medicamentous therapy and repeat surgery over 3 years of observation.

The paper provides a retrospective estimation of primary medical records (case records and medication administration records) for 360 patients with community-acquired pneumonia admitted to health care institutions of Volgograd in 2009-2010. a study showed a high variability in the quality of antibacterial therapy of patients admitted with community-acquired pneumonia. The main problems areas are inadequate administration of antibacterial agents for initial treatment of community-acquired pneumonia and a low rate of sequential therapy administration.

Having carried out a complex technological and physicochemical study, we developed a technology of obtaining bischofite solution with glycyrrhizinic acid in the form of ophthalmological spray that can be used as an anti-inflammatory agent with pronounced anti-viral, immunomodulatory and reparative activity. The osmotic activity of two different solutions was studied and we chose the optimal composition, which may be recommended for treatment of purulent eye infection.

Abstract

A complex histologic and electronic-microscopic analysis of bronchial biopsy specimens was performed in various types of respiratory tract inflammation. Allergic inflammation of bronchial mucosa is accompanied by development of perivascular edema with malfunction of endothelial cellular activity, epithelial destructions and desquamations that are significant pathogenetic factors of bronchial hyperreactivity. Nonallergic inflammatory process is accompanied by subepithelial and perivascular fibrosis, which leads to irreversible bronchial obstruction. Atrophy sites of epithelial layer alternate with squamous cell metaplasia, which reflects dysregeneration processes developing in the respiratory tract.

Abstract

Results of medico-social evaluation of patients with proximal humerus fractures are presented. Such characteristics as age, social status and concomitant pathology were investigated. An analysis of peculiarities and causes of traumatic injury of upper extremity was conducted. It was shown that these characteristics influence the duration of hospital treatment period. A conclusion was made that during management of patients with proximal humerus fractures it is necessary to take these characteristics into consideration.

Abstract

The clinical efficacy of course therapy of bronchial asthma with low doses of fluticasone propionate with sodium cromoglycate and sodium montelukast in children under 6, including an assessment of clinical symptoms and indicators of airway resistance were compared. In children under 6 with a mild form of asthma the therapy with fluticasone propionate at low doses (Flixotid) and leukotriene receptor antagonists (singular) is superior to therapy using chromohexal in the most significant effects: the severity of daytime and nighttime symptoms, the number of asymptomatic days, the need for ß2-agonists of short duration, a decrease in airway resistance.

Abstract

The research is based on analysis of treatment results of 127 patients with benign thyroid neoplasm. 58 patients (45,67 %) were treated by method of interstitial laser photocoagulation; 69 patients (54,33 %) were operated by standard surgical methods. Application of interstitial laser photocoagulation resulted in two-fold decrease of pain syndrome intensity in post-surgery period and the time of patients return to activity; the rate of complications decreased 2,3 times. The analysis proved that, as compared to standard treatment, interstitial laser photocoagulation method shows a high effectiveness, clinical & social advantages in treatment of patients with benign thyroid neoplasm.

Abstract

The article is devoted to a study of seasonal variations in blood hormone levels in healthy women. Is was established that leptin, insulin, C peptide, aldosterone and progesterone peaks are noted in winter, adiponectin, testosterone, estradiol and follicle-stimulating hormone - in summer, cortisole - in autumn, prolactin and luteinizing hormone - in spring. Thus statistically significant seasonal variations of hormone levels in healthy women in different phase of the menstrual cycle are revealed.

COMPARATIVE ASSESSMENT OF EFFECTIVENESS OF LAPAROSCOPIC AND LAPAROSCOPY-ASSISTED APPENDECTOMY

Abstract

Results of treating 367 patients with acute appendicitis were analysed in a cohort study: 151 laparoscopic appendectomy with clipping the stump of appendix, and 216 laparoscopy-assisted appendectomies with miniaccess. The laparoscopic variant of appendectomy is more preferable due to the absence of wound complications. In patients who underwent laparoscopy-assisted appendectomy wound complications developed in 5 % of observed cases. After laparoscopy in 3,9 % of cases intraabdominal complications were noted. Relaparoscopy was performed in 1,3 % of patients after laparoscopy. Incompetence of appendix stump after laparoscopy was noted in 1,3 % of cases, which required repeat surgery.

EXPERIMENTAL STUDY OF LIPID PEROXIDATION IN COMBINATION OF DOXORUBICIN AND MEXIDOL

Abstract

The influence of a derivative of 3-oxyperidine of mexidol on animal lipid peroxidation in chemotherapy with doxorybicin was studied. It is shown that mexidol eliminates induced lipid peroxidation by doxorybicin, decreases the MDA lever, increases enzymatic activity of antioxidative protection both in the blood and in tissues and erythrocytes. This substantiates the use of mexidol in complex tumor treatment with lipid peroxidation activation by chemotherapy.

Abstract

Having analyzed 6933 cases of death in hospital from cardiovascular diseases with and without AH we established that AH makes the biggest contribution to death rate from cardiovascular diseases. Most cases of death with AH complications among females occur in the age group 70-79, among males - in the group of 60-69 years old. The most vulnerable age related to death from AH complications in hospital for females was under 59 years, for men under 49 years.
