Abstract

It is shown that long-standing type II diabetes mellitus leads to the reduced CYP2C9 capacity. There has been obtained strong evidence that long term administration of sulphonylurea derivatives reduces the CYP2 C9 capacity by significantly altering pharmacokinetics of the drugs. This requires a change in their dosing regimen . Mandatory pharmacokinetic typing of CYP2C9 activity should be recommended to improve the efficacy of pharmacotherapy for type II diabetes mellitus, to prevent the undesirable adverse reactions when taking the drugs and to reduce the treatment cost for this group of patients.

Abstract

An increase in morbidity of newborns is observed over recent years. In most cases metabolic disorders, levocarnitin deficiency in particular as it is hardly produced in late neonatal period, underlie their pathogenesis. The structure of cardiovascular diseases in the neonatal period is dominated by an increase in the rate of consequences of perinatal hypoxic lesions of the myocardium in the form of transient ischemia. This study proposes modern approaches to diagnostics and treatment of transient myocardial ischemia in newborns, to estimation of the level of arterial p ressure in preterm newborns using the AMBP method.

Abstract

Immunoreactivity in patients with chronic prostatitis and prostate cancer was studied; possibility of applying blood leukocytic formula parameters for the purpose of revealing secondary immunodeficiency state are considered. The absence of reaction in the form of IgM level increase in patients with chronic prostatitis indicates the presence of immune deficiency in formation of antibacterial or antiviral antibodies, which can be the direct cause of chronization supporting the chronic inflammation process.

Abstract

Investigation and treatment of 22 patients with atypical phlegmon of maxillofacial area was carried out. The immune status of patients was characterised by a high activity of the local immunity link and somewhat weak activation of general immunity. Development of postinfectious immune insufficiency primarily involving the phagocytic compartment of local immunity was the feature of immunity in torpid maxillofacial phlegmon. There is a correlation dependence of all links of the immune status parametres changes of the body.

Abstract

Results of studying a possible pathogenic correlation of tension headache and functional status of h ypophysialovarian system in women of reproductive age have been presented. 100 women with chronic tension headache and 30 women with episodic tension headache have been examined. It has been demonstrated that in case of chronic tension headache a marked imbalance of sex hormones and cortisol dependent on the phase of menstrual cycle is present. On the basis of the obtained data pathogenic correlation of chronic tension headache and hormonal deviations is considered.

Abstract

For optimization of management of pregnant women group of high risk for placental insufficiency a clinical algorithm is proposed that was developed for a modified scale of diagnostics of placental insufficiency. For d evelopment of the algorithm and its approbation in clinical conditions, 1192 pregnant women and their newborns were observed. The analysis of the condition of newborns has shown favorable perinatal outcomes upon managing pregnant women according to the developed clinical algorithm.

SURGICAL TREATMENT OF EXTRASPHINCTERIC RECTUM FISTULAS

Abstract

The purpose of this study is to evaluate a new method of fistula excision by means of U-shaped ligature loop running on a needle along the sphincter fibers. 130 patients with extrasphincteric rectum fistulas were operated, 61,5 % of them with a fistula of 3-4 degree of complication. The method was worked out and applied to 38 patients. In comparison with a control group the number of recurrence was two times lower, and the treatment period was twice shortened in the absolute absence of insufficiency rectum sphincter.

PROCESSING OF CONTAMINATED SURGICAL WOUND WITH ANTISEPTICSOLUTION IN APPENDECTOMY THROUGH MINI-ACCESS

Abstract

Microbial contamination of abdominal cavity and wound before and after processing it with antiseptics was studied. Microorganisms were isolated from a surgical wound and identified for their morphological, tinctoria l, enzymatic and antigen properties with a bacteriological method. Efficiency of processing of a surgical wound with various antiseptics is estimated. Upon processing of a surgical wound with 0,5 % water-alcohol solution, 70 % solution of chlorhexidine digluconate with a 1-2 minute exposition the results were negative in 19 patients. The results of microbiological research were negative in 100 % cases.

In conservative treatment and postoperative applications of dermal fibroblasts, culture of wound covering «Voskopran» was used. This method is efficacious and greatly promotes the healing of trophic ulcers.

CALCIUM АNTAGONISTS AND UROLITHIASIS: NEW PERSPECTIVES

Abstract

In view of the exclusive role of ions of calcium in regulation of numerous functions of tissue and cells of the body, great importance is assigned to preparations of calcium antagonists (СА). CA were first applied exclusively in cardiology, but owing to their properties they have found application in obstetrics, gynecology, neurology, immunology, surgery. Despite numerous experimental studies testifying to the expressed cardiovascular properties of CA, indications for their administration constantly expand, which is associated with the physiological role of ions of calcium in providing for vital activity of cells and the whole body. Preliminary results indicate that СА can be administered in new ways - as modulators of apoptosis in renal tissue in urolithiasis and chronic pyelonephritis.

Abstract

Results of blood and urine cytokines (IL-1betta, 6, 8, the FNO-alpha, 4,10) study in 40 children with vesicoureteral reflux in perioperative period are described in this article. It is revealed that even in the latent phase of chronic obstructive pyelonephritis there are expressed changes of cytokines profile in urine. It is shown that a change in the level of proinflammatory IL 8 and 6 is of the greatest prognostic value, which has a strong positive correlation with severity of inflammatory process in urinary tracts. We assume that a study of urine cytokines profile provides a noninvasive method for estimation of the severity of inflammatory process in urinary tracts, efficiency of administered conservative therapy, as well as a correction in the terms of carrying out operative interventions, which is especially important in young children.

Abstract

The level of leukocytes and leukocyte formula both in a disease onset, and upon admission to hospital were studied. It is revealed that the level of leukocytes in patients with systemic lupus erythematosus is closely interconnected with the activity of the disease, exudative nature of internal lesion, extent of haemolysis. However, no interrelations with a total dose of cyclophosphan was established, which testifies to a mostly immune genesis of leukopenia. We should underline the prognostic value of leukocytes decrease in the onset of a disease, which is associated with severe clinical manifestations of systemic lupus erythematosus.

Abstract

The present work is devoted to investigation of the role of intelligence in effective work of operators of sensomotor profile in a man-machine system. A short characteristic of the intellectual processes providing for operators activity is proposed. An analysis of mutual influences of indicators of emotional and psychometric intelligence is done. A comparison of isolated types of emotional and psychometric intelligence is made. Specifics of operators intellectual sphere are shown. It is demonstrated that a high level of emotional and psychometric intelligence is the integral component of an effective operators activity.
