Clinical significance of antibiotics modified release oral formulations

Abstract

The authors studied clinical advantages of antibiotics modified release oral formulation, which are very important for clinical practice. Such antibiotics include azithromycin ER and clarithromycin ER. These formulations have special pharmacokinetic feature - delayed absorption in distal areas of gastrointestinal tract, what decreases the contact with motilin receptors and improves antibiotics tolerability. Because of the high tissue penetration optimal tissue concentration of such antibiotics are preserved for a long time. Modified release oral formulations allows to achieve better clinical results, increase tolerability and decrease the frequency of antibiotis dosing, that result in better patients compliance.

Influence of renin-angiotensin-aldosterone system blockade on atrial fibrillation course

Abstract

Despite the great success in understanding of many electrophysiological mechanisms of atrial fibrillation (AF) developing and maintaining, until know there is no universal method of its treatment. Using of antiarrhythmic drugs for sinus rhythm maintaining is limited by its low effectiveness. It is reasonable to suppose that the blockage of renin-angiotensin-aldosterone system can decrease the number of the first episodes of AF and prevent the relapses. The article includes the results of the studies evaluating the effects of inhibitors of angiotensin-converting enzyme and angiotensin II receptors blockers on the development and progress of AF. These drugs can prevent AF and its complications, probably by means of regress of structural and functional remodeling of myocardium.

Abstract

Peculiarities of antihypertensive therapy in metabolic syndrome and diabetes mellitus The prevalence of hypertension (HP) and associated with it cardiovascular risk, disability and premature death of the patients increasing. It is known that HP is frequently associated with such conditions as metabolic syndrome (MS) and type II diabetes. In patients with MS and diabetes prevalence of HP then in general population. In 80% of cases development of HP precedes to development of diabetes. Treatment of HP have in these patients have some specifics. First of all, it is necessary to achieve blood pressure goals, metabolic control and prevention of complications.

Abstract

Movalis efficacy and safety in the treatment of locomotor apparatus diseases Treatment of chronic locomotor diseases is a complex task. Antiinflammatory and analgesic therapy are complicated by advanced age, concomitant diseases and necessity of its treatment. Frequently used classic nonsteroidal antiinflammatory drugs besides positive effects often cause adverse effects from gastrointestinal tract, liver, kidneys, bronchus, what limits the providing of adequate therapy. The cyclo-oxygenase-2 selective inhibitors meloxicam (Movalis) have the same effectiveness as classic nonsteroidal antiinflammatory drugs, but much better tolerability, including elderly people. The paper includes the results of research and clinical practice, confirming the efficiency and safety of Movalis.

Ferro-Folgamma provides increasing of hemoglobin, erythrocytes, and content of hemoglobin in erythrocytes, and characterizes by good tolerance.

Abstract

Gastroesophageal reflux disease (GERD) is a chronic, relapsing disease, caused by the disturbances of motor and evacuator functions of gastroesophageal zone, characterized by spontaneous and regularly regurgitation of gastric and/or duodenum content to esophagus, with specific symptoms which not depends on morphologic changes of esophagus mucosa. According to Genvald guidelines GERD is divided on endoscopic-negative GERD, endoscopic-positive GERD and Barrett esophagus. Treatment depends on the disease form. Artile includes the modern algorithms of GERD management. The most effective drugs for GERD treatment are proton pomp inhibitors.

Rational approach to diagnostics and treatment of chronic viral hepatitis C

Abstract

Virus of hepatitis C is the only virus of Hepadnovirus line in the Flaviviridae family which includes RNA. It was found in 199o as a etiologic agent of posttransfusion non-A, non-B hepatitis. Chronic hepatitis C is the main etiologic factor of chronic hepatitis, cirrhosis and hepatocellular carcinoma. In 1999 in Russia the morbidity of hepatitis C was 19,3 on 100 thousand of population. The course of hepatitis is often asymptomatic. In 20-30% of patients with chronic infection it transforms in cirrhosis during the 20 years. The article includes the epidemiology and pathogenesis of chronic hepatitis C and modem algorithms of its diagnostics and treatment.

Antihypertensive therapy in patients with paroxysmal form of atrial fibrillation: is drug selection important?

Abstract

The study included 84 patients with paroxysmal atrial fibrillation (AF) and hypertension of different degrees. We evaluate the influence of antihypertensive therapy by calcium antagonists (CA) and inhibitors of angiotensin-converting enzyme (ACE) on the clinical course of paroxysmal AF in patients with hypertension and structural and functional remodeling of the myocardium. It was shown that the efficiency of blood pressure (BP) lowering by CA and ACE inhibitors is similar. BP control leads to significant decreasing of the frequency of AF paroxysm compared this using only classic ant arrhythmic drug. ACE inhibitors characterized by stable anti-relapsing activity, but protective effect CA decreased on 7,9% after 12 months. Compared with CA therapy by ACE inhibitors led to decreasing of structural and functional remodeling of the myocardium and reduction of AF paroxysm frequency on 61,5%, what can be caused by specific blockage of renin-angiotensin-aldosterone system.

Evaluation of efficacy of the thiazide diuretic hydrochlorothiazide in mono- and combined therapy in patients with arterial hypertension distributed according to blood rennin-aldosterone profile

Abstract

We conducted the evaluation of efficacy and the role of the thiazide diuretic hydrochlorothiazide in mono- and combined therapy in patients with arterial hypertension distributed according to blood renin-aldosterone profile. The study included 340 patients with essential hypertension. The plasma renin activity (PRA, range 1,0-3,0 ng/ml/h) and aldosterone plasma levels (range 0,18-0,83 nmol/l or 5-23 ng/ml/h) were evaluated by radioimmunoassay method. Normal range of ratio aldosterone plasma levels to PRA is 5-23. The conclusion of the study is that it possible to give the patients with PRA 0,22-1,0 ng/ml/h and ratio aldosterone plasma levels to PRA 5-50 hydrochlorothiazide in metabolic neutral dose (less then 12,5 mg/day) in monotherapy or in combination with enalapril or losartan in low doses with the titration to achieve blood pressure levels.
