Place of antibiotics in the schemes of inflammatory intestinal diseases treatment (review of literature)

Abstract

We discussed the problem of the use of antibacterial drugs in the treatment of inflammatory bowel disease (IBD). The results of numerous studies on the management of combination antibiotic therapy in addition to treatment by glucocorticosteroids, probiotics, chemotherapeutic agents in various forms of IBD are presented. These experimental and clinical data show that not only antibiotics, as follows a complex of several "technical" methods in patients with IBD will allow to control such a complex system as microflora of gastro-intestinal tract with projected effect.

Mode of bacteria-commensales interaction with immunity in certain syndromes of chronic intestinal inflammation

Abstract

This review covers the molecular mechanisms that ensure the integrity of the intestinal epithelium and the nature of the interaction of normal microflora with immunity factors in some chronic inflammatory syndromes. We highlight the role of nitric oxide in the pathogenesis of inflammatory bowel disease, and the role of transcription factor NF-kB, involved in control of expression of genes coding for the synthesis of various antimicrobial products, anti-inflammatory and anti-inflammatory cytokines, and various enzymes and other. Data presented for the molecular genetic basis of pathogenesis of inflammatory diseases of digestive tract are very important to develop more effective methods of their therapy.

Abstract

Anisakidosis is an important medical and social problem for Russian Far East region. Anisakidosis causative agents are nematode larvae of Anisakis, Contracaecum, Pseudoterranova, Hysterothylacium genus, ordinal to Ascaridida, Anisacidae family. Most cases are usually not diagnosed. The article discusses the possibilities of modern diagnosis of the disease, problems of its differential diagnostics with Crohn's disease, treatment and prevention.

Potential for pantoprazole use in the therapy of gastroesophageal reflux disease

Abstract

Erosive esophagitis is one of the most severe clinical forms of gastroesophageal reflux disease (GERD), a lifelong health condition, which occurs as a result of gastric juice reflux into the esophagus. More than 40% of the adult population has GERD symptoms, expressed in varying degrees and the registered at least 2 times a week. GERD-induced esophagus damage can cause more serious complications, such as stricture, bleeding, and the precancer formation - Barrett's esophagus. In GERD treatment the drugs of choice are the modern acid production blockers (proton pump inhibitors, PPI). The analysis of the literature data for optimal choice of PPIs for the GERD treatment highlights the advantages of pantoprazole. We consider the metabolism characteristics of this drug administration in standard doses in comparison with other PPIs.

The drug Hepabene in the therapy of hepatobiliary system diseases

Abstract

We presented data for many studies on the therapeutic efficacy of Hepabene having hepatoprotective, choleretic and spasmolytic properties, in patients with hepatobiliary system diseases. Positive properties of the drug are determined by the qualities of its component plants - Silybum marianum fruit extract and Fumaria officinalis herbs extract. The clinical trials data analyses confirmed the Hepabene effectiveness in the treatment of hepatobiliary system diseases and allowed us to formulate a list of indications for this drug administration. They are all kinds of biliary dyskinesia, chronic liver disease, including steatosis and toxic injury. We highlight the importance of Gepabene administration in patients with liver steatosis caused by metabolic syndrome and type 2 diabetes mellitus.

Possibilities of hyoscine butylbromide (Вuscopan) use in the treatment of patients with chronic pancreatitis

Abstract

We present the results of clinical assessment for applicability of Buscopan (Hyoscine butilbromide) in the treatment of chronic pancreatitis with pain syndrome. In the study, including 52 patients, Buscopan significantly reduced the pain intensity in a period of 2 weeks management. The optimal dose was 20 mg 3 times daily, in this case the pain index decreased by 4 times compared with baseline. The frequency of the typical cholinolytic side effects of Buscopan is about 14% regardless of dose.

Efficacy of Ganaton (itopride hydrochloride) use in the treatment of patients with functional dyspepsia

Abstract

We assessed the Ganaton (itopride hydrochloride) efficacy in the treatment of 509 patients with functional dyspepsia (FD). We estimated the symptoms severity according to a points system. It noted the frequent combination of PD with heartburn, nausea, bloating, and bowel function disorders. Ganaton administration at a dose of 150 mg daily for 4 weeks contributed to the complete disappearance of complaints in 46.6% of patients and significant decrease in its severity in 47.8% of patients. The drug administration was well tolerated; serious side effects were not observed.

Efficacy of Ganaton (itopride hydrochloride) use in the treatment of patients with functional dyspepsia

Abstract

We presents the results of study on the efficacy and safety of tricomponent therapy with omeprazole, amoxicillin and nifuratel use based on the results of HP-antigen identification in coprofiltrates, which included 32 children and 35 adults with chronic inflammatory diseases of the upper gastrointestinal tract, associated with Helicobacter pylori (HP). Therapy-induced clinical symptoms improvement and erosive and ulcerative mucosal defects epithelization were noted in all patients. Eradication was achieved in 93.8% of children and 88.6% of adults based on the results of histological examination of biopsy samples and immunochromatographic analysis of fecal specimen. Treatment regimen was well torelated, side effects such as nausea and unformed stool was registered only in 6.3% of children and 8.6% of adults. Thus, the nifuratel-based scheme in combination with omeprazole and amoxicillin is effective and safe for the eradication of HP infection in children and adults. Noninvasive immunochromatographic short-term test based on monoclonal antibodies for the HP-antigens identification in fecal specimen can be used for the diagnosis of HP infection and assessment the efficacy of the therapy.

Influence of different variants of eradication therapy on indices of non-specific colon immunity and microbiocenosis in patients with peptic ulcer disease

Abstract

According to the study, including 63 patients, all patients with Helicobacter pylori (HP) associated peptic ulcer disease have moderate cellular and humoral immunity disorders. HP eradication leads to nonspecific immunological resistance normalization. Intestinal dysbiosis is detected in all patients with exacerbation of HP-associated peptic ulcer disease. The number of bifidus and lactobacillus was increased and enzimic poor E. coli growth was inhibited after HP eradication by the use of clarithromycin, amoxicillin and omeprazole.

Interferon therapy of chronic viral hepatitis in children

Abstract

We presented the experience of Roferon-A (recombinant alfa-2a interferon) administration in 69 children with chronic hepatitis C. The data analysis shows the initial remission occur in 49-61% of children at the end of therapy. Remission in 12 months after completion of treatment remained at 22.8-29.6% of patients. It is emphasized that the hitting probability of prolonged remission significantly increases in case of HCV RNA elimination in early stages of interferon therapy.

Efficacy of Odeston in the treatment of patients with chronic pancreatitis

Abstract

The results of investigations of the myotropic spasmolytic Odeston (hymecromone) influence in the pancreas exocrine function in 11 patients with different genesis chronic pancreatitis (CP) are presented. Odeston administration in the treatment of patients with CP leads to pain syndrome relief, severity of diarrhea and flatulence decrease, basic indicators of pancreas exocrine function improvement: time-voltage pancreatin extension, mean concentration and discharge of amylase and bicarbonates one hour after stimulation. The drug administration was well tolerated, we did not reveal any complications or negative side effects when it is receiving in stated dosages. Thus, Odeston is a highly effective medicine for maldigestive syndromes relief, and it promotes regression of duodenum dyskinesia and symptoms of pancreatitis.

Probiotics: prospects for therapeutic use in inflammatory intestinal diseases

Abstract

The paper discusses the prospects for probiotics use for the prevention of exacerbations and treatment of inflammatory bowel disease (IBD) - ulcerative colitis and Crohn's disease. We present current data on the intestinal microflora and its physiological functions, as well as the role of intestinal flora in the genesis of inflammation in IBD. We give a probiotics characteristics, discuses the results of probiotics clinical trials in IBD. It is concluded that management of IBD by probiotics is a simple, natural and safe therapeutic intervention. These clinical studies suggest that the probiotics use should be long-term - one year continuously, or in the form of repeated courses, required in large doses.
