Abstract

The article is dedicated to the problem of cognitive impairments occurring in arterial hypertension. The evidences of relationship between increased blood pressure with high stroke risk and cognitive impairments, as well as effect of antihypertensive therapy on decrease of risk of development of cognitive impairments are presented. The solution algorithm for pharmacotherapeutic goals in these impairments is discussed. Particular attention is paid to the use of neuroprotectors, in particular, Mildronate for correction of cognitive impairments. Mildronate has a pronounced positive effect on cognitive functions in elderly patients with arterial hypertension.

OPTIMIZATION OF ANTIHYPERTENSIVE THERAPY IN PATIENTS WITH NON-ALCOHOLIC FATTY LIVER DISEASE

Abstract

The numerous studies have shown that cardiovascular disease and non-alcoholic fatty liver disease (NAFLD) are widespread in the general population and are linked by common pathogenetic mechanisms. Therefore their combination is typical for obesity and metabolic syndrome. The results of the study, whose goal was to study the clinical course, diagnosis and treatment of arterial hypertension (AH) in patients with non-alcoholic steatohepatitis (NASH - NAFLD stage) are presented. It was found that concomitant NASH leads to more severe course of arterial hypertension. It is emphasized that in patients with arterial hypertension and NAFLD comprehensive approach to treatment, aimed at the various components of metabolic syndrome is justified. In addition to antihypertensive treatment, drugs improving liver function (essential phospholipids, ursodeoxycholic acid), lipid-lowering drugs (statins), and, if necessary, hypoglycemic agents (metformin), should be used.

Abstract

The open randomized 4-month study was aimed to assessment of efficacy and safety of Piascledine compared with Ibuprofen in patients with osteoarthrosis (OA) of hand joints. The study included 40 patients of both sexes (mean age, 65.3 ± 6.9 years), 20 patients (study group) have received Piascledine 300 mg/day + Ibuprofen 1200 mg/day, while the remaining 20 patients (control group) - Ibuprofen 1200 mg/day. Treatment effectiveness was evaluated by the functional AUSCAN index, the modified functional Dreiser's index (FIHOA), and assessment of therapy effectiveness by the doctor and patient monthly. After 2 months of therapy, the decrease of joint pain was observed in all patients (p<0.05), but indexes, against the background of Piascledine administration, were lower (97.6±63.9 and 131.5±89.4, respectively). Significant reduction in joint stiffness was observed only in the study group in a month later and lasted until the end of treatment. There was a significant improvement of hand joints function at the end of the study in patients who have received Piascledine + Ibuprofen, compared with Ibuprofen only (235.5±116.8 vs 318.3±131.0, p <0.05). By the end of treatment, 100% of patients of study group have estimated their condition as improvement in contrast with only 65% of patients of control group. Thus, Piascledine demonstrates efficacy in OA of hand joints (reduction of pain and joint stiffness, and improvement of joint function), having a good tolerability.

OUTPATIENTS WITH ARTERIAL HYPERTENSION: IS IT IMPORTANT TO TAKE INTO ACCOUNT FEELING-ACTIVITY-MOOD?

Abstract

Clinical experience shows that achievement of target blood pressure (BP) in patients with arterial hypertension (AH) is probably only in case of full mutual understanding with the doctor. The assessment of levels of feeling, activity and mood (FAM) in patients allows not only initiating the trust relations with patient but also predicting the success of his treatment. The study involving 119 hypertensive patients showed that active management of complex patients, taking into account their psychological status, helps to achieve not only the target BP, but also improve psychological well-being of patients in most cases. In the case of low rating in FAM test, patient should revisit doctor in 10-14 days, and, as possible, visit psychologist or psychotherapeutist. Careful attention to these patients and their problems may be the missing link in the successful treatment and the early achievement of target BP levels.

Abstract

The data for screening examination of iron metabolism in 289 patients with myelodysplastic syndrome (MDS) are presented. Screening was aimed to identify the category of patients who may benefit from chelator therapy. The data analysis suggests that iron overload is one of the characteristic symptoms in patients with low risk MDS, especially in the case of donor red blood cells transfusion. Thereby, chelator therapy should be an integral part of treatment in patients with MDS. Administration of iron chelators for patients with MDS should be preceded by analyses of the clinical and laboratory indices, including the number of packed red blood cell unit transfusions, serum ferritin level, and transferrin saturation level.

Abstract

In recent years, halenic forms of enzyme preparations were developed. These drugs substantively improve delivery, incorporation and interaction of proteases and lipases and their substrates, and significantly improve the quality of supplementation treatment of pancreatic deficiency. Constraining factor for widespread use of these drugs is relatively high cost compared with pelleted enteric-coated pancreatine; but the use of such pancreatine is substantially ineffective. The retrospective comparative study on model of complete inpatient event insurance for the diagnosis of chronic pancreatitis was conducted. The use of two different preparations of pancreatine with equivalent doses of lipase (Creon® and Mezym forte 10000) - not less than 100 000 units of lipase per day - was compared. Against the background of Creon administration, duration of the treatment period has decreased by 7 days that led to reduction of cost of inpatient treatment by one third. The cost ratio for pain relief and reduction of exocrine pancreatic deficiency with Creon showed savings in 542,21 and 355,82 rubles per patient, respectively. Thus, despite the higher cost of equivalent dose of lipase, supplementation treatment with Creon in patients with chronic pancreatitis with exocrine deficiency was proved to be more beneficial.

Abstract

The present article is concerned with the questions of pathogenesis and treatment of intestinal dyspepsia. The results of the use of probiotic Lactiv-ratiofarm in 40 patients with symptoms of intestinal dyspepsia and suffering from diseases of the digestive system are presented. After 2-week course of the use of Lactiv-ratiofarm, positive therapeutic effect was observed in all 40 patients enrolled in the study (discomfort in the abdomen, borborygmus and bloating, and abdominal wall pain tenderness were significantly decreased or disappeared). The use of Lactiv-ratiofarm was not accompanied by the development of any adverse events. As a result, all 40 patients have expressed satisfaction with the therapy.
