Abstract

The contribution of E.M. Tareev in the development of problem of arterial hypertension (AH), including renal and malignant AH, is presented. The fundamental works of E.M. Tareev devoted to the problem of arterial hypertension are discussed. Many of the concepts of AH stated by E.M. Tareev today are justified not only in the experimental and clinical studies, but they also confirmed by the data obtained during a long-term observation of very large groups of patients.

Abstract

The present article is concerned with causes, development and approaches to the coping of diuretic resistance (DR). DR is common presentation sign in patients with severe diseases, for example, in patients with more extreme degrees of nephrotic syndrome or decompensated chronic heart failure. DR always considerably complicates treatment and often leads to worsening of prognosis. It is clear that solution of this problem requires a multidisciplinary approach - the replacement of diuretic and/or increasing the doses only is often ineffective.

Abstract

The article discusses the problem of chronic pancreatitis (CP), the incidence of which is currently increasing. The data for the etiology and pathogenesis of pancreatitis, modern classification, and diagnosis and treatment tactics of CP are presented. The article presents review of the results of numerous studies of the pharmacological treatment of abdominal pain and exocrine pancreatic insufficiency with pancreatic enzyme preparations. To date, all published studies proved that only generation IV medications (Creon and analogues) are most effective in the treatment of pancreatic insufficiency that largely determines the usefulness of Creon for reduction of two major syndromes of CP - exocrine pancreatic insufficiency and abdominal pain syndrome.

Abstract

Chronic neuropathic syndromes are relevant medical problem because they are widespread in clinical practice and lead to significant maladjustment of patients. Pathophysiological processes in the formation and development of neuropathic syndrome are clearly correlated with the clinical picture of neuropathic pain, therefore, detailed analysis of the clinical presentation is extremely important for the selection of pathogenetically justified treatment. The article is concerned with analysis of clinical manifestations and mechanisms of development of symptom in neuropathic pain, which is the basis for optimization of treatment with the use of pathogenetic therapy.

Abstract

Among the variety of neurological symptoms developing as a result of vascular brain lesions, cognitive impairments have a special place, and they significantly affect the quality of life of patients. On the one hand, endothelial dysfunction (ED) is one of the important pathogenetic mechanisms of vascular diseases of the brain; on the other hand- an independent risk factor for development of this pathology. Identification and evaluation of ED in the early stages of vascular cognitive impairment allow developing the correct strategy of therapy, which capable to slowing or regression of the resulting symptoms. Taking into account modern views on the pathogenesis of these disorders, antioxidant medications restoring endothelial function should be considered as important part of complex treatment of cerebrovascular disease.

Abstract

The article is dedicated to the problem of autonomic neuropathy, which may be caused by variety of etiological factors. The most common cause of autonomic neuropathy (AN) is diabetes mellitus, and disability and death in diabetic patients are mainly due to cardiovascular complications of AN. The clinical characteristics of cardiovascular AN, methods of diagnosis and principles of treatment of autonomic dysfunction are presented. The treatment of AN is a complex medico-social problem that requires maximum participation of the patient in the treatment, as well as the creation of necessary conditions for rehabilitation against the background of the pathogenetic (tioctic acid) and symptomatic (mineralocorticoides, sympathomimetics) therapy.

Abstract

The article is dedicated to the importance of motor-tonic disorders in the pathogenesis of cholelithiasis (CL) and other pathology of biliary tract (BT), as well as the effectiveness of their correction by selective miotropic spasmolytic Mebeverine (Duspatalin) using large number of case histories. The basic mechanisms of the pathogenesis of cholelithiasis are discussed, and the approaches to treatment are described. Much attention is paid to the mechanism of action and benefits of the drug Duspatalin in correction of motor-tonic disorders of BT. Use of selective miotropic spasmolytic Duspatalin in patients with biliary pathology, including cholelithiasis, leads to the reduction of clinical symptoms (pain and dyspeptic syndromes), as well as normalization of cholepoiesisand choleresis, in a relatively short time .

Hypertensive encephalopathy (HE) is a slow-progressive diffuse focal brain damage due to chronic circulatory disturbance associated with long-term uncontrolled hypertension. The article is dedicated to the pathogenesis of HE, contributing factors; prevention measures are discussed. The role of prevention of brain lesions in arterial hypertension, which should include not only control of blood pressure, but additional treatment aimed to improving the metabolic processes of the brain is emphasized. Abstract

The study was aimed to evaluation of the features of autonomic responses in patients with arterial hypertension (AH) with different types of diurnal profile of blood pressure (BP) within the range of Na+-Li+- exchange transport velocity in erythrocyte membranes. 24-hour BP monitoring, evaluation of the frequency spectrum of heart rate variability at rest and orthostasis, determination of Na+-Li+- exchange transport velocity in erythrocyte membranes were performed in ninety-five patients with I-II stages AH. The results showed a significant effect of the functional state of cell membranes to formation of specific vegetative status in hypertensive patients with appropriate diurnal BP profile, including features of the sympathetic and parasympathetic activity and the nature of autonomic responses during the functional test. Correlation of initial level of sympathetic activity with the transmembrane ion transport velocity in hypertensive patients with Dipper type of diurnal BP profile is revealed.

EXPERIENCE OF USE OF HEPABENE IN THE TREATMENT OF NON-ALCOHOLIC STEATOHEPATITIS

Abstract

Non-alcoholic steatohepatitis (NASH) is a chronic liver disease developing in patients who do not consume alcohol in hepatotoxic doses, and is characterized by the development macrovesicular steatosis and lobular inflammation of the liver. NASH is considered as a disease of the liver in the metabolic syndrome, the pathogenetic basis of which is the development of insulin resistance. The results of open clinical study of efficacy and safety of herbal medicine Hepabene (milk thistle and fumitory extracts) in the treatment of 43 patients with NASH are presented. It was shown that treatment of NASH with the drug Hepabene due to restoration of hepatocyte function and normalization of bile outflow leads to rapid improvement of general state of patients, reduction of severity of clinical symptoms, laboratory and instrumental parameters. The drug effectively restores motor-evacuatory function of the biliary tract; Hepabene is well tolerated by patients and can be recommended for use in the treatment of patients with NASH, especially with concomitant biliary tract disease.

EXPERIENCE OF USE OF NATIVE HEPATOPROTECTOR HEPTOR (ADEMETIONINE) IN PATIENTS WITH ALCOHOLIC LIVER DISEASE

Abstract

Ademetionine plays an important role in the treatment of alcoholic liver disease (ALD) due to its detoxication, antioxidant, chloretic, cytoprotective (hepatoprotective), antifibrosing and neuroprotective (antidepresant) effects. The article presents results of study aimed to estimation of efficacy and safety of drug Heptor (tablets Ademetionine) manufactured by Joint-Stock Company "Veropharm", Russia, in a combination with parenteral form of Ademetionine (drug Heptral manufactured by "Knoll", Germany) in the treatment of 60 patients with ALD(chronic hepatitis or cirrhosis) in comparison with monotherapy by Heptral (lyophilizate + tablets). 18-day courses of treatment by tablets Heptor and Heptral have shown similar results in regard to time of development of a positive effect and influence for biochemical activity of disease. The profile of safety of Heptor (tablet) is similar to that Heptral (tablet). Thus, Heptor (tablets Ademetionine) can be recommended for treatment of patients with ALD at stages of chronic hepatitis and cirrhosis.

Abstract

The present article is dedicated to the results of evaluation of epidemiological (preventive) efficacy of subunit purified inactivated vaccine Agrippal S1 in the period of seasonal increase of incidence of influenza and acute respiratory viral infection - ARVI (2008-2009) among 17,102 high risk children and adults (health professionals). It was shown that vaccine Agrippal S1 has high epidemiological efficiency; the incidence of influenza and ARVI in once vaccinated group was less by 4.32 times compared with not vaccinated group. Agrippal S1 was well-tolerated in children and adults: within a month after vaccination, there were no seeking medical advices in vaccinated group. The calculations have shown that immunization by Agrippal S1 in 17,102 individuals allowed to prevent the loss of 49,918.32 thousand rubles (2.92 rubles per vaccine).

The article presents history of development, the procedure and diagnostic value of evaluation of endogenous creatinine clearance by Rehberg-Tareev test used for determining the glomerular filtration rate.
