Abstract

Estrogens are involved in the development of brain structures that define a woman's behavior. They provide genomic and negenomic effects on the brain through binding to estrogen receptors. Neuronal and glial cells contain the enzyme aromatase for local estrogen synthesis. The article summarizes the protective effects of estrogen in degenerative processes of the central nervous system (CNS). The data on the use of estrogen in schizophrenia, migraine, and other pathological processes in the CNS are presented.

Abstract

During pregnancy, folate deficiency leads to a significant increase in the frequency of congenital abnormalities due to violations of DNA methylation. Compensation of folate deficiency only with synthetic folic acid (FA) is not always successful, because it has to be transformed to the active form of folate (tetrahydrofolate) through specific enzyme systems. In many women, these systems may be lost due to genetic defects, drugs that block these enzymes, and other reasons. This necessitates the use of active folate (AF) for the improvement of efficiency and safety of compensation of folate deficiency, especially in the relevant risk groups. This review presents the results of a systematic analysis of fundamental and clinical medicine, indicating the perspectiveness of use of AF for the nutrient supplementation in pregnancy and for other indications in the field of obstetrics, and gynecology. It is shown that AF is characterized by pronounced antiteratogenic effect and significantly neutralizes the negative effects of retinoids, nitrogen oxides, valproic acid and methotrexate on the growing embryo. In contrast to the inactive FA, the excess of which leads to blockage of folate metabolism, excess of AF has no such effect. Therefore AF has no side effects typical of excessive consumption of FA, and has a positive impact in any genotype of folate biotransformation enzyme systems.

Abstract

The article presents the results of assessment of clinical and hematological efficacy of Ferro-Folgamma at a dose of 1 capsule 2 times a day for 6 weeks as a treatment of mild iron deficiency anemia (IDA) in 115 pregnant women (Group 1) and 140 postpartum women (Group 2). Ferro-Folgamma significantly reduced the severity of clinical signs of IDA both in pregnant and postpartum women (p < 0,05). Normalization of hemoglobin level and ferrokinetic indicators were observed after 4 weeks of treatment in 85 (73,9 %) patients of Group 1 and in 106 (75,7 %) in Group 2. After 6 weeks of treatment, in pregnant and postpartum women significant increase in hematologic, ferrokinetic indicators and the serum levels of folic acid and cyanocobalamin was found as compared to baseline levels. Subjective and objective parametres indicate good antianemic effect of Ferro-Folgamma. A course of therapy for 6 weeks provides fast recovery in hemoglobin, serum iron and ferritin levels both in pregnant and postpartum women with mild IDA. Safety, high therapeutic activity, good tolerability of drug for the treatment of IDA are demonstrated.

Abstract

The results of the evaluation of 396 patients with complaints of erectile dysfunction (ED) are presented. Alprostadil was used for the pharmacological Doppler sonography of cavernous arteries. The test was assessed 15 minutes after intracavernosal injection of drug. The degree of erectile dysfunction was defined in accordance with the degree of penile tumescence and rigidity of the penis. Along with assessment of the effectiveness of alprostadil, the spectral characteristics of the cavernous artery blood flow were analyzed. Clinical result of intracavernous administration of alprostadil (degree of filling and rigidity of the penis) was most pronounced in young patients. The presence and type of risk factors did not significantly influence the clinical result of test. Clinical result of intracavernous administration of drug in patients with preserved sexual activity was significantly higher. Thus, application of 5 μg of alprostadil is an effective and safe method for evaluation of patients with ED of various origins and severity.

Abstract

The results of the evaluation of quality of ejaculate in 20 men after surgical treatment of varicocele, and definition of the role of metalligand homeostasis, depending on the GSTM1 and GSTT1 genotypes in the development of pathozoospermia, are presented. In patients with GSTM1 (-) compared with GSTM1 (+), a lower quality of ejaculate was observed in all aspects. It was found that the metal-ligand homeostasis has a genetic determinancy and reflects the activity of the metabolic processes in the organs of the scrotum.

Abstract

The present study aimed to compare the effectiveness of extracorporeal shock wave lithotripsy (ESWL) and contact ureterolithotripsy (CULT) included the results of treatment 481 patients with ureteral calculi: ESWL was performed in 224 patients, and CULT - in 257 patients. Complete destruction of the calculi using ESWL was observed in 94,7 %, and using CULT - in 86,4 % of cases. Postoperative complications rate in the application of ESWL was 4,0 %, in the application of CULT - 10,9 %. The average number of sessions of ESWL for 1 patient was 1.28 ± 0.05, the average duration of DLT session - 49,48 ± 2,16 minutes. CULT required one session for litholisis and removal of calculus, and the average duration of one session was 68,54 ± 3,14 minutes. The choice of method of treatment for patients with ureteral calculi (ESWL, CULT) depends on the location, size, density of calculus, state of the upper urinary tract, as well as the technical equipment of the clinic and the surgeon's experience.

Abstract

The efficacy of the citrate drug blemaren in 158 patients with various forms of urate nephrolithiasis was evaluated. It is shown that adequate use of blemaren controlled by urologist allows to achieve complete dissolution of uric acid stones for 1,5-2 months in 83,3 % of cases. It is emphasized that the citrate mixtures should be administered only after evaluation of metabolic changes in blood and urine of patients with urolithiasis, and litholytic therapy should be implemented and adjusted under control of these parameters. The use of citrate mixtures significantly reduces the density of existing stones in the urinary system, and facilitates their subsequent remote and contact dissolution.

Abstract

The article presents the results of the evaluation of the efficacy and tolerability of dienogest in the combined treatment of 176 patients with external genital endometriosis and patients with associated hyperplastic processes in other organs of reproductive system. It is shown that dienogest at a dose of 2 mg is effective in the treatment of endometriosis, significantly reduces the severity of pain, and can be used for a long time; dienogest should be considered as the drug of choice for patients with recurrence of disease.

Abstract

Adverse effects of androgen deficiency on the male body determine the need for mandatory screening for androgen in men aged 50 years and older. Testosterone level were evaluated in a group of 521 men aged 18-93 years (mean age, 60,94 years). 235 (45,2 %) patients with late onset hypogonadism were identified. An algorithm for diagnosis of androgen deficiency in male outpatients is recommended. The predominance of prevalence of prostate diseases and a number of somatic pathologies in patients with late onset hypogonadism is observed. Abstract

The article presents the results of the study involving 77 patients with urinary incontinence (UI) after radical prostatectomy; patients underwent pelvic floor muscle training with control of biofeedback for therapeutic purposes. In order to increase the effectiveness of treatment, 16 (20,8 %) patients have received М-anticholinergic drug trospium chloride as a part of complex therapy for the reduction of frequency of involuntary contractions of the detrusor. Pelvic floor muscle training in patients with UI led to reduction of incidence of involuntary detrusor contractions, and prevention of the reduction of pressure in the urethra in response to a sudden increase in intra-abdominal pressure. The use of trospium chloride for the complex treatment of urinary incontinence has strengthened inhibitory effect of voluntary contractions of pelvic floor muscles on detrusor.

Abstract

Lower abdominal pain is the most common complaint in gynecological practice. This symptom is non-specific, because it occurs in many diseases. The main objective in the treatment of patients with pain syndrome is decrease of pain intensity. The main groups of drugs used for pain relief are divided on analgesics (narcotic and non-narcotic) and anesthetic agents (local and general). Currently, a broad arsenal of non-steroidal anti-inflammatory drugs (NSAIDs) is available. Special attention is given to the drugs with potent pharmacological profile, favorable combination of clinical efficacy and safety, which have various forms of release. Ketoprofen refers to a group of non-selective NSAIDs, which optimally balances the inhibition of cyclooxygenase-1 (COX-1) and COX-2; and this feature provides a powerful analgesic effect. Moreover, small molecular size and high lipophilicity, unlike most other NSAIDs, allows to penetrate the central nervous system (CNS), and additionally provides a central analgesic effect. NSAIDs are used in gynecological practice in the treatment of dysmenorrhea, pelvic inflammatory diseases (PID), pelvic ganglioneuritis, as well as for the premedication before various minimally invasive interventions.

Abstract

The article presents the results of observation of 216 women aged 21-72 years with pelvic floor incompetence and stress urinary incontinence who underwent surgical treatment: anterior colporrhaphy with the plastic reconstruction of the sphincter of the bladder by the standard technique modified by authors, cystourethropexy (fixation of paraurethral tissues and bladder neck on the anterior abdominal wall), which was supplemented by posterior colporrhaphy with perineolevatoroplasty and amputation of the cervix according to the standard rules as appropriate. During follow-up period of 1 year to 10 years (2000-2010), good results were observed in 188 (87 %) patients, satisfactory result - in 22 (10,2 %) patients, unsatisfactory - in 6 (2,8 %) patients. After the surgery, there was a statistically significant reduction in the maximum urine flow rate and a statistically significant increase in the time of urination. This method of treatment impacts on all components of pathogenesis and performs the tasks of operative treatment of patients with stress urinary incontinence, and sufficiently completely eliminates all the changes of anatomical and functional state of the lower urinary tract, restoring the “single sphincteric mechanism of the pelvis". Since 2005, no postoperative pyoinflammatory complications were registered in patients who received vaginal drug Hexicon.
