Abstract

На сегодняшний день установлено, что высокая суточная вариабельность гликемии значимо повышает риск внезапной сердечной смерти. При этом недавние исследования показали значительное повышение числа гипогликемий на фоне интенсивного режима сахароснижающей терапии. Ведущие специалисты в области эндокринологии признают, что достижение оптимальной цели гликемическо-го контроля у больного сахарным диабетом 2 типа - один из наиболее важных вопросов современной диабетологии, что позволит существенно как снизить смертность среди пациентов, так и повысить эффективность лечения в целом.

Abstract

Lapatinib is a new anticancer drug, dual inhibitor of tyrosine kinase receptors HER1/HER2. Now, it is widely used in combination with capecitabine in advanced and/or metastatic breast cancer with HER2 receptor overexpression in patients previously treated with trastuzumab and resistance to the latter. The intensive studies to assess the effectiveness of lapatinib in combination with various cytotoxic drugs, hormonal treatment, as well as with other targeted agents are continued. The potentials for the use of lapatinib as adjuvant and neoadjuvant therapy are discussed.

Abstract

On the example of celiac disease, modern approaches to treatment of multifactorial hereditary diseases are considered. Etiopathogenic basis of these diseases includes functionally weakened variants of certain genes («predisposition» genes), and their damaging effect is implemented on the background of unfavorable environmental factors. Celiac disease is chronic, multi-syndrome, autoimmune, genetically determined disorder characterized by persistent gluten intolerance with structural and functional changes in the intestinal mucosa and abnormal processes of membrane digestion and absorption of nutrients in genetically predisposed individuals. Modern data on the diagnosis and treatment of celiac disease are presented.

Abstract

Systemic autoimmune diseases (SAD) include a heterogeneous group of immune-mediated inflammatory diseases characterized by the development of multi-organ failure. Patients with SAD can experience interstitial lung disease (ILD). High-resolution computed tomography of the lungs and pulmonary function test play a leading role in the diagnosis of ILD. To date, despite the progress in addressing issues of pathogenesis and diagnosis of ILD in SAD, there is no effective immunomodulatory therapy for ILD-SAD. The therapy of ILD is aimed to the suppressing immune inflammatory process. This article discusses the effectiveness of the immunosuppressive therapy in some SAD.

Abstract

Treatment of patients with severe recurrent herpes infection remains one of the urgent problems of modern virology and is a difficult task. Typically, the disease is accompanied by the formation of secondary immunodeficiency manifested by disturbances in the system of natural killer cells, the development of hyporesponsiveness of systems of specific cytotoxicity, failure of interferon system. Herpes simplex virus replication always occurs against the background of violations in the interferon system; so, therapy aimed to the activation of Th1-cytokine response contributes to the reduction of virus activity and accelerates clinical recovery. Maximum therapeutic effect can be achieved with a combination of acyclic antiviral agents (acyclovir, valaciclovir, famciclovir) and the medications correcting immune cytokine response. Correction of immunologic disorders in recurrent herpes infection can be achieved by using interferons and interferon inducers as part of combined treatment of this disease.

Vascular brain lesions are one of the most frequent causes of cognitive impairment, especially in elderly and senile patients. Their severity is variable: from a minimum disorders of higher brain functions to dementia. Currently, the treatment of cognitive dysfunction in vascular cognitive impairment (VCI) includes the use of cholinesterase inhibitors and NMDA (N-methyl-D-aspartate) antagonists. Primary and secondary prevention of SKN is similar to the prevention of stroke [11]. Phenotropil is the one of the most promising drug for the treatment and prevention of the progression of cognitive impairment.

Abstract

The article presents the results of evaluation of the relationship of adiponectin, leptin, and hormonal-metabolic parameters based on the survey of 93 women with hypothyroidism. In patients with hypothyroidism, insulin resistance and compensatory hyperinsulinemia were identified. They also have pronounced atherogenic dyslipidemia, which was expressed by elevated levels of total cholesterol, triglycerides, low density lipoprotein cholesterol, and atherogenic index, and decreased level of high density lipoprotein cholesterol. In women with hypothyroidism, concentration of leptin was increased and adiponectin levels were reduced. It is obvious that changes of these adipokines play a role in the development of atherogenic dyslipidemia.

Abstract

The examination and verification of the diagnosis of 70 patients with bacterial vaginosis were performed. These patients were divided into two groups depending on the method of treatment. Main group of 35 patients has received treatment with chlorhexidine (Hexicon) and lactic acid (Femilex). Comparison group of 35 patients was treated with chlorhexidine (Hexicon) only. It was found that combination therapy of bacterial vaginosis using chlorhexidine and lactic acid is the optimal method of treatment of this disease.

Abstract

The appearance of S-1 - peroral fluorinated pyrimidine - has allowed to take a new look at the treatment of patients with metastatic stomach cancer. The combination of S-1 with cisplatin has provided improvement in overall survival against the background of absence of clinically significant toxicity. The use of S-1 as monotherapy for adjuvant purposes has led to the significant increase in the three-year disease-free survival of patients with stomach cancer (72.2 vs 60.1 %, P < 0.0001). Great interest to the S-1 was confirmed by numerous ongoing international studies which have evaluated the effectiveness of S-1 not only in combination with other cytotoxic drugs, but also with targeted drugs.

Abstract

The article presents the current guidelines on the pharmacological treatment of systemic scleroderma according to the European League Against Rheumatism. The main groups of drugs used in various clinical manifestations of systemic scleroderma - skin lesions, sclerodermic renal crisis, pulmonary hypertension, gastro-oesophageal reflux, and arthritis - are discussed. It is emphasized that modern guidelines on the treatment of systemic scleroderma have summarized and analyzed extensive experience of foreign experts, which in combination with the experience of the national rheumatology integrated in national guidelines on rheumatology, should help the practitioner to effectively treat the patients and improve disease prognosis.

The article considers the potentials for the use of a relatively new representative of calcium slow channel blockers, lercanidipine, in different variants of arterial hypertension. These data strongly suggest that it may be recommended for the wide use as an antihypertensive agent. The drug combines stable antihypertensive effect and organoprotective action, while well tolerated. The use of lercanidipine in the fixed combinations of antihypertensive drugs for the treatment of patients with renal diseases, rheumatic diseases, and diabetes is especially promising.

Abstract

Modern macrolides firmly won their place in the treatment of inflammatory diseases of the respiratory tract due to the high antibacterial activity against Gram-positive cocci, Haemophilus pertussis, Corynebacterium diphtheriae, Moraxella, Legionella, Campylobacter, Listeria, Spirochetes, Chlamydia, Mycoplasma, Ureaplasma, anaerobes and some protozoa. The combination of additional effects that are not associated with an antimicrobial effect, opens up new prospects in the application of macrolides. In this review, characteristics of roxithromycin presented in more detail; this drug has been used successfully in the clinical practice due to the optimal parameters of pharmacokinetics and good tolerability.
