INTEGRATED APPROACH TO THE TREATMENT OF PATIENTS WITH OTITIS EXTERNA

Abstract

The prevalence of inflammatory diseases of the external ear is 17 to 30% among all otological pathology. Environmental deterioration, increased level of antibiotic resistance, increase in number of people with metabolic disorders, immune status disorders, including allergo-pathology, irrational treatment of acute inflammatory diseases, delayed visiting an ENT doctor, and a number of other aspects contribute to the growth of rate of these diseases. The article provides an overview of inflammatory diseases of the external ear, and the modern classification of otitis externa. Anatomic and topographic features of the external ear contributing to inflammation, and the main infectious agents are presented. The issues of diagnosis and treatment of otitis externa, which are based on an integrated approach, are discussed. The authors also give recommendations for patients with otitis on proper care of the ears, and practical advices for the doctors.

ALGORITHM OF TREATMENT OF NON-ALCOHOLIC FATTY OF LIVER DISEASE AND THE ROLE OF MITOCHONDRIAL DYSFUNCTION IN ITS DEVELOPMENT

Abstract

The lack of a unified concept of the essence of the processes occurring in non-alcoholic fatty liver disease (NAFLD), scarcity of clinical symptoms, and the limited possibilities for timely diagnosis cause the interest of physicians of various specialties in seeking solutions to this problem. The issues related to the evaluation of etiopathogenesis, diagnosis, treatment and prevention of steatosis in patients with NAFLD still remain an urgent task for the world healthcare. Symptoms of mitochondrial dysfunction, such as changes in the lipidogram and lipid peroxidation indices, as well as in the hepatic cell during electron microscopy, arising from steatosis, confirm its role in the development and progression of the disease. This fact should be taken into account when drawing up a diagnosis plan and an algorithm for treating patients with NAFLD.

CHANGES IN ARTERIAL STIFFNESS AND ENDOTHELIAL DYSFUNCTION ON THE BACKGROUND OF ANTICOAGULANT THERAPY AFTER CARDIOEMBOLIC STROKE IN PATIENTS WITH ATRIAL FIBRILLATION

Abstract

The present study is devoted to the evaluation of the effect of anticoagulant therapy with apixaban on the parameters of arterial stiffness and endothelial dysfunction (ED) in patients with atrial fibrillation (AF) who have experienced cardioembolic stroke (CES). The study included patients with a persistent or paroxysmal form of non-valvular AF who have experienced stroke and were discharged from Samara ’s vascular centers for follow-up. Patients were divided into 2 groups: 56 patients (Group 1) has received apixaban in a daily dose of 5 mg according to the recommendations. The comparison group (Group 2) included 57patients who did not follow the recommendations for taking anticoagulants. The duration of follow-up was 182.5±5.5 days. All the examinations were carried out during the period of sinus rhythm in patients: volume sphygmography, echocardiography and tissue dopplerography, analysis of ED (von Willebrand factor, tissue plasminogen activator-1 inhibitor, antithrombin III) .The results obtained indicate that arterial stiffness and endothelial function in patients with AF who have experienced CES have a positive tendency to improve against the background of anticoagulant therapy with apixaban.

THE POSSIBILITIES OF ANXIOLITHIC THERAPY OF ANXIETY-DEPRESSIVE SYNDROME IN PATIENTS ON PROGRAM HEMODIALYSIS

Abstract

Anxiety-depressive syndrome in patients on program hemodialysis is the most frequent mental disorder in this category of patients. The article describes the results of the use of morpholinoethylthioethoxybenzimidazole dihydrochloride in such patients. It is established that this drug significantly reduces the severity of anxiety and depression, and also improves the quality of life of patients. The four-week therapy with morpholinoethylthioethoxybenzimidazole dihydrochloride is not accompanied by an impact on laboratory indicators used as a control during program hemodialysis.

The article presents the results of the comparative non-interventional clinical study in parallel groups aimed to the evaluation of the efficacy and safety of the drug containing a complex of antibodies to tumor necrosis factor a, moskomarhitectura protein S-100, histamine (cAT) compared to trimebutine in the therapy of irritable bowel syndrome (IBS). The data obtained allow to recommend cAT for widespread use in the treatment of IBS, because it will have a positive effect on the relief of the clinical manifestations of the disease, especially pain syndrome.

IVABRADIN AGAINST BISOPROLOL IN PATIENTS WITH CHRONIC HEART FAILURE AND PRESERVED LEFT VENTRICULAL EJECTION FRACTION

Abstract

In a prospective, randomized trial included 126 patients with chronic heart failure with a left ventricular ejection fraction >50% on a background of arterial hypertension and coronary artery disease, a fixed combination of perindopril/amlodipine was used in all cases, with addition of bisoprolol (n=62) or ivabradine (n=64 ). Assessment of clinical status, exercise tolerance, quality of life, blood level of the N-terminal pro-brain natriuretic peptide, and echocardiography were performed baseline and 12 months after therapy. In contrast to bisoprolol, ivabradine significantly increased the exercise tolerance, improved quality of life, reduced the blood level of the N-terminal pro-brain natriuretic peptide, improved echocardiographic indices of active relaxation and compliance of left ventricle. In comparison with bisoprolol, ivabradine has advantages in the treatment of patients with chronic heart failure with preserved left ventricular ejection fraction on background of arterial hypertension and coronary artery disease.

PATHOPHYSIOLOGICAL AND NEUROPHARMACOLOGICAL FEATURES OF THERAPY OF CHRONIC RETENTION OF URINE IN MULTIPLE SCLEROSIS

Abstract

Chronic neurogenic retention of urine (CNRU) is a very common and characteristic complication of demyelinating disease of the central nervous system - multiple sclerosis (MS). Comparison of clinical, urodynamic and neuroimaging features of CNRU in MS patients allowed to determine the spinal and cerebral components of the «sphincter» and «detrusor» urinary retention, as well as sensory and motor components. Using the method of comparing clinical, neuroimaging and urodynamic data made it possible to describe the pathogenesis of the development of urinary tract disorders in MS, and the use of method of neuropharmacologic analysis allowed to substantiate the symptomatic therapy of the various forms of CNRU occurring in this disease. The double mechanism of action (central and peripheral) of α1-adrenoblocker for its application in patients with detrusor-sphincter dissynergy (DSD) is described for the first time. The use of mono- and multicomponent symptomatic therapy based on compensation of disturbed reflex connections between the brain and spinal urination centers with the use of α1-adrenoblocker, anticholinesterase agent and GABA agonist in patients with DSD, pseudodissinergia and detrusor hypotony due to MS allowed to compensate the deficiency of impaired functions of lower urinary tract.

THE USE OF ANTIBACTERIAL DRUGS DURING ORTHOPAEDIC ALIGNMENT WITH REMOVABLE LAMINAR DENTURES IN PATIENTS WITH TYPE 2 DIABETES MELLITUS TAKING INTO ACCOUNT THE RESULTS OF PCR-BASED DIAGNOSTICS

Abstract

The article presents the results of evaluation of methods for increasing the effectiveness of orthopedic dental rehabilitation of patients with type 2 diabetes mellitus (DM2) with chronic generalized periodontitis. To diagnose parodontopathogenic microorganisms and determine their antibiotic susceptibility in 59 DM2 patients during the treatment of partial adentia using removable acrylic prostheses with a clam-type fixation, a real-time polymerase chain reaction (PCR) method was applied. Dynamics of results of PCR-based diagnostics before and after antibacterial therapy has demonstrated a decrease in the incidence and number of individual types of parodontopathogenic microorganisms in the microflora of the oral cavity in DM2 patients with moderate-to-severe chronic generalized periodontitis. There was a decrease in inflammation of the periodontal tissues, oral mucosa and prosthetic bed. The necessity of using antibacterial therapy for the treatment of exacerbation of periodontal diseases and inflammation of the oral mucosa during the orthopedic dental rehabilitation of DM2 patients is justified.

THE ROLE OF PROBIOTICS IN THE TREATMENT OF HELICOBACTER PYLORI INFECTION

Abstract

Helicobacter pylori (HP) infection is one of the most common human infections, which leads to the development of chronic gastritis, gastric and duodenal ulcers, MALT-lymphoma and gastric adenocarcinoma. Increased HP resistance to previously effective schemes of eradication therapy is a source of serious concern, and requires improvement of therapeutic strategies. The literature review shows that some probiotic strains have an inhibitory effect on the HP, and their use is recommended as an addition to the standard eradication therapy that can reduce side effects and increase its effectiveness.

Torasemide - loop diuretic, characterized by high bioavailability and favorable pharmacodynamic properties. The data on the clinical efficacy of torasemide in the treatment of patients with arterial hypertension and chronic heart failure, including in comparison with other loop diuretics, are presented. Due to unique pharmacological profile, pleiotropic properties, and neutral metabolic effect, torasemide has the advantage of other loop diuretics with regard to efficacy, safety, and treatment compliance in patients with arterial hypertension and chronic heart failure.

Abstract

The article discusses the features of the pathogenesis of mitochondrial dysfunction, which is observed in various pathological conditions accompanied by hypoxia and ischemia. A review of clinical studies and the results of authors’ own experience of use of meldonium as a corrector of secondary mitochondrial dysfunction are presented. Analysis of research confirms that the addition of meldonium to the background therapy is accompanied by a positive clinical effect in cardiovascular and neurological disorders, diabetes mellitus, metabolic syndrome and other diseases with comorbidity.

ENDOGENOUS AMMONIA AS THE REASON OF "CIRCULUS VITIOSUS" IN PROGRESSION OF LIVER DISEASES

Abstract

Ammonia is one of the most important neurotoxic metabolites in the human body. In clinical practice, hepatic encephalopathy (HE) against the background of hepatopathy is the most frequent manifestation of hyperammonemia. HE represents a wide spectrum of neuropsychiatric disorders in the presence of acute or chronic hepatic-cell insufficiency and/or porto-systemic shunt. Reduction of ammonia level is a pathogenetic approach in the treatment of chronic liver diseases, which allows not only reducing the manifestations of HE, but also reducing the activation of hepatic stellate cells, improving hepatic blood flow and inhibit the development and progression of liver fibrosis.
