EXPERIENCE OF THE USE A SHORT COURSE OF URSODEOXYCHOLIC ACID IN PATIENTS WITH CHOLESTASIS PHENOMENA

Abstract

Objective: to analyze the effectiveness of a short course of treatment with ursodeoxycholic acid (preparation Grinterol, Grindex) in cholestatic syndrome. Methods. 30 patients with clinical and laboratory manifestations of cholestasis were included in the study. All patients were prescribed a preparation Grinterol at the dose of 250 mg every 25 kg of body weight for two months, and a diet for patients with diseases of the bile ducts was also recommended. Results. The results indicate high efficacy (significant improvement of clinical and laboratory indicators) and safety (no side effects) of this drug, which allows recommending it for widespread use in patients with signs of cholestasis.

THE IMPORTANCE OF THE REFLUXATE NATURE FOR THE MANIFESTATION OF CLINICAL AND ENDOSCOPIC SYMPTOMS OF GASTROESOPHAGEAL REFLUX DISEASE

Abstract

Objective: to evaluate the possible association of the refluxate nature (RN) with the features of the leading clinical endoscopic signs of GERD. Methods. In order to support objectives, 74 patients with GERD were examined, 40 (54%) men and 34 (46%) women aged 18 to 57 years (mean age, 34±4.2 years). The diagnosis of GERD was verified based on the clinical and anamnestic, radiologic and endoscopic data using the GERD classification. Results. In cases of alkaline reflux, odynophagia, dysphagia and bitterness in the mouth were more pronounced; and with acid reflux, the significance of heartburn and eructation has increased significantly. Erosive esophagitis was more typical for acid reflux, and non-erosive esophagitis - for alkaline reflux. Conclusion. Based on the results of study, it can be concluded that RN makes a certain impact on the features of the manifestation of clinical and endoscopic signs of GERD.

RAKURS REGISTER OF PREDICTED ADHERENCE TO TREATMENT: STUDY DESIGN AND RESEARCH STRATEGY

Abstract

Background. To implement the state task of the Ministry of Health of the Russian Federation № ГР АААА-А18-118011190076-1 «Introduction of technology of patient-oriented treatment for patients with chronic internal diseases associated with metabolic syndrome», the Register of students of the Medical Faculty of the Omsk State Medical University was created to develop a scale for the assessment of potential adherence to treatment (RAKURS). Objective: is the development of design, structure and main directions of the formation and use of the register. Methods. The design of the study determined the formation of primary database on the expected adherence to treatment for students of all university classes with the ability of prospective database seeding in a format of cross-sectional study. The criteria for the formation of the register included the depersonalization (coding) of information about the participant at baseline; inclusion of data on participants in the general population who participated in paper-and-pencil interviewing during formation of continuous cross-sectional register sample, including sex, age, height, body weight, average monthly income, training course, history of chronic somatic diseases; formation of an autonomous electronic database of the register; development, creation and official registration of software products that ensure the functionality of the register in terms of storage, automated processing and use of data. Results. The processing of data included in the Respondent Register serves as a basis for quantifying the expected integral adherence to treatment and its individual components. Conclusion. Assessment of these indicators allows to form mean-sample parameters and scale of expected adherence to treatment, typical for the sample of respondents controlled by age, sex, average income and educational level.

Abstract

The article presents main ideas about the therapeutic possibilities of drugs - hepatoprotectors, created on the basis of essential phospholipids (EPLs), in the treatment of common hepatic disorders. The etiopathogenetic mechanisms of non-alcoholic fatty liver disease, metabolic syndrome, and alcoholic liver disease are considered. Disputable data on various views on the possibility of using EPLs in the treatment of these diseases are given. It is concluded that the use of EPLs in the treatment of hepatic disorders is reasonable.

INFLUENCE OF SHORT-TERM TREATMENT WITH A ZOPICLONE ON BALANCE IN CHRONIC INSOMNIA: CLINICAL CASE

Abstract

Background. The most common pharmacological treatment for insomnia is third generation hypnotics, among which zopiclone occupies one of the leading places.Despite the known negative effect of hypnotics on postural functions, sleep improvement in patients with chronic insomnia eventually leads to an improvement in the balance. We present a clinical case of effective treatment of insomnia, accompanied by an improvement in postural stability. Description of the clinical case. The results of the use of zopiclone (Somnol, 7.5 mg for 10 days 40 minutes before bedtime) in a 64-year-old patient with chronic insomnia accompanied by dizziness and walking unstability are considered. This therapy provided a significant improvement in sleep indices, accompanied by an increase in postural stability. Conclusion. There was no possible adverse effect of zopiclone on balance in the case presented. This may be due to the fact that zopiclone itself does not have such a negative effect, or the effect of normalizing sleep exceeded the negative effect of the drug. It is obvious that in many cases the meaning elderly people off hypnotics is not justified, and the doctor should independently correlate the risk and benefit from the appointment of such treatment in each case.

NON-ALCOHOLIC FATTY LIVER DISEASE AND CHOLELITHIASIS: A NEW VIEW ON OLD RELATIONSHIPS

Abstract

Currently, there is no doubt about close relationship between non-alcoholic fatty liver disease (NAFLD) and cholelithiasis. It has been established that the high prevalence and progression rate of NAFLD, as well as cholelithiasis, depend on a number of risk factors, which are predominantly common. For example, NAFLD is 3.5 times more likely to be associated with cholelithiasis than with other diseases of the gastrointestinal tract (GIT). In turn, cholelithiasis can have serious metabolic effects predisposing to the development of NAFLD, and bile acids are now considered less as «surfactants/detergents» facilitating fat absorption, as previously thought, than as signaling molecules that coordinate the regulation of metabolism and inflammation, and not only in the GIT, but also in other organs and systems. It still remains unclear what disease develops primarily, or they are developed in parallel, but their negative impact on each other is precisely known.

WHAT DIFFICULTIES DOES PRACTITIONER FACE IN TREATMENT OF IRRITABLE BOWEL SYNDROME: CLINICAL OBSERVATION

Abstract

Background. Irritable bowel syndrome (IBS) is a chronic functional disorder of the gastrointestinal tract, manifested by periodic abdominal pain associated with a change in the frequency of defecation and/or stool consistency. Description of the clinical case. The case presented is an illustration of the difficulties that often arise in the treatment of IBS. The patient visited gastroenterologists many times, but short courses of treatment by various antispasmodics and defoamers had only a short-term effects. Conclusion. The use of trimebutin, which has a normokinetic effect on the lower gastrointestinal tract motility, as well as antispastic and analgesic effects, provided the patient with a rapid and pronounced relief of the symptoms of the disease, which can be considered as a priority for treatment of IBS.
