Diagnosis of anemic syndrome in patients with active rheumatoid arthritis

Abstract

Abstract. Anemia remains one of the most common concomitant pathologies, which has an influence at the prognosis of main disease and life quality of the patient.

The aim: to develop an algorithm using hepcidin for the differential diagnosis of chronic inflammation anemia (ACI) from true iron deficiency anemia (IDA) in patients with active rheumatoid arthritis (RA).

Material and methods. The study was prospective. 78 RA patients with high activity of the inflammatory process according to DAS28-ESR were included in the analysis. Patients were recruited sequentially upon admission to hospitalization at V.A. Nasonova Research Institute of Rheumatology. Criterion for anemia was considered to be a decrease in hemoglobin levels below 120 g/l for female and below 130 g/l for male patients. Depending on hemoglobin level, participants were divided into the main (with anemia, n = 47) and control (without anemia, n = 29) groups. All patients had an assessment of DAS28 index, estimation of clinical and biochemical blood parameters (serum iron, total iron-binding serum capacity, hepcidin, interleukin 6).

Results. Among patients with active RA and anemia (n = 47), isolated ACI was diagnosed in 57.4% of cases (n = 27). Classic IDA was diagnosed in 20 patients (42.6%). Significantly higher hepcidin values (120.3 ± 56.71 pg/ml) were determined in patients with ACI comparatively to the control group (90.4 ± 36.9 pg/ml) and RA patients with IDA. Correlation between hepcidin level and interleukin 6 level (rIL-6 = 0.81) was obtained only in the subgroup with ACI. In patients with IDA and without anemia, such kind of correlation was not found. Based on the obtained data, an algorithm for diagnosing anemia in patients with active RA was developed.

Conclusion. The level of hepcidin is an informative indicator for differential diagnosis of the type of anemia during active inflammation. In RA patients with ACI, the highest values of hepcidin in blood serum are detected, while in IDA cases they are staying below the reference values. The study also demonstrates the importance of hepcidin – interleukin 6 correlation.

Experience of dapagliflozin use in elderly female patients with hyperactive urinary bladder

Abstract

Abstract. Hyperactive urinary bladder (HAUB) is common in type 2 diabetes mellitus (T2DM) and may be a factor limiting use of sodium-glucose co-transporter 2 inhibitors (SGLT2). Current studies demonstrate an increased risk of urinary tract infections (UTIs) development during SGLT2 therapy, which may be facilitated by HAUB.

The aim: to evaluate the effect of dapagliflozin therapy at HAUB clinical course in T2DM female patients and possibilities of using herbal uroseptic containing extracts of centaury herb, lovage roots and rosemary leaves for recurrent UTIs’ development prevention.

Material and methods. An outpatient examination of 18 female patients with T2DM and HAUB was carried out. The median age of the patients was 72 (68–79) years. Dapagliflozin at a dose of 10 mg was selected from SGLT2; it was added to the existing glucose-lowering therapy. Follow-up questionnaire, general urine analysis were performed after 1 and 3 months, and urination diaries were assessed after a month of SGLT-2 use.

Results. Initially, 11 patients had a mild degree of urinary disorder, and 7 had a moderate degree. A month later, during SNGLT-2 therapy, mild severity of urinary disorder took place in 8, moderate – in 4, severe – in 6 female participants of the study. There was a significantly valid increase in urination frequency, minimum and maximum urine volumes. In 7 female patients with initially moderate degree of urinary disturbances, a deterioration in HAUB clinical course was recorded, which was facilitated by urinary tract infection. Asymptomatic bacteriuria was detected in 7 out of 11 females with initially mild urinary disorder. After antibiotic therapy for complicated cystitis and asymptomatic bacteriuria, 14 patients continued taking dapagliflozin. Additionally, monthly administration of a herbal uroseptic for 10 days was recommended. Follow-up examination after 3 months showed no worsening of HAUB.

Conclusion. Taking SGLT-2 inhibitors in elderly female patients with T2DM and HAUB increases the risk of developing urinary tract infections. For patients with mild urinary disorders receiving gliflozin, it is advisable to recommend an uroseptic containing extracts of centaury herb, lovage roots and rosemary leaves for 10 days of each month. In case of moderate to severe HAUB, it is preferable to use incretins.

Estimation of the risk of cardiovascular events and adverse outcomes in patients with peripheral atherosclerosis

Abstract

Abstract. Despite the fact that patients with simultaneous atherosclerotic lesions of various vascular beds are systematically registered in clinical practice, there is insufficient reliable data on the management of such kind of patients. It could be helpful to determine management tactics for that group of patients with a direct focus on the prevention of adverse cardiovascular events.

The aim: to develop a calculator for assessing the risk of cardiovascular events and adverse outcomes in patients with peripheral atherosclerosis during a three-year prospective observation, including data based on clinical and instrumental characteristics of endothelial function.

Material and methods. The study included 519 patients with atherosclerotic lesions of various vascular territories. The patients underwent a complex of laboratory tests and a number of instrumental tests, including (if indicated) coronary angiography, angiography of the renal, brachiocephalic and lower extremities arteries. During a prospective observation (36 months), the long-term prognosis of that group of individuals was assessed.

Results. Maximal discriminant significance according to the F-criterion when dividing patients depending on the risk of cardiovascular events and adverse outcomes developing had such indexes as the presence of hemodynamically significant peripheral atherosclerosis, value of the atherogenic index, value of glycated hemoglobin, gender of patients, age, biological vascular age and augmentation index value.

Conclusion. The developed calculator for assessing the risk of cardiovascular events and adverse outcomes developing allows us to highly accurately assess the probability of unfavorable long-term prognosis development in patients with peripheral atherosclerosis.

Life quality and prognosis of patients with adverse post-infarction left ventricle remodeling

Abstract

Abstract. Chronic heart failure (CHF) is one of the most common complications of ST-segment elevation myocardial infarction (STEMI) due to pathological or adverse post-infarction remodeling of the left ventricle. At the same time, the subjective estimation of well-being and long-term prognosis of patients with different variants of such remodeling are remaining unclear.

The aim: to study the dynamics of life quality and prognosis in patients after STEMI with adverse left ventricular remodeling (ALVR) and without it.

Material and methods. 141 patients with STEMI aged 51 (45; 58) years was included into the study. Their quality of life was assessed on days 7–9 and after 24 and 48 weeks using a visual analogue scale (VAS) and Minnesota Questionnaire (MLHFQ). According to the results of echocardiography, ALVR was diagnosed in cases when the end-diastolic volume index increased by ≥ 20% and/or end-systolic volume index increased by ≥ 15% at 24 weeks comparatively to the values on days 7–9. Endpoints (cardiac surgery, recurrent MI) were registering during 96 weeks of observation.

Results. 125 patients (88.7%) have passed through observation. After 24 weeks they were divided into groups: 1st – 63 persons with ALVR (“ALVR”), 2nd – 62 persons without signs of ALVR (“No ALVR”). According to VAS data, in “ALVR” group, the subjective assessment of well-being improved by 5,2% (p0–48 = 0.045) only after 48 weeks. In group 2, by the intermediate visit the increase in this parameter reached 6.8% comparatively to the initial level (p0–24 = 0.001), and at the end of the observation it was 6.7% (p0–48 = 0.001). At the same time, VAS values differed in the comparison groups both after 24 and 48 weeks (р1–2 < 0.01). When analyzing the results of MLHFQ, a progressive worsening of CHF symptoms was found in both groups. In group 1, the end points within 96 weeks were detected in 13 (20.6%), in the 2nd group – in 2 (3.2%) patients. Odds ratio for an unfavorable outcome in ALVR was 7,8 (95% CI: 1.7–36.2) (p = 0.003) comparatively with “No ALVR” group.

Conclusion. Obtained results confirm the importance of timely diagnosis of ALVR for the purpose of adequate correction of anti-remodeling pharmacotherapy to improve the life quality and prognosis of STEMI patients.

Gender peculiarities of clinical manifestations in patients with arterial hypertension and chronic heart failure in combination with senile asthenia syndrome

Abstract

Abstract. Prevalence of comorbid pathology naturally increases with age in both men and women, with the percentage of arterial hypertension (AH) and chronic heart failure (CHF) especially increasing.

The aim: to identify gender characteristics of risk factors (RF), comorbidities, clinical symptoms and cognitive functions in elderly patients with AH and CHF depending on the presence of senile asthenia syndrome (SAS).

Material and methods. The study included 161 patients with AH and CHF aged 80 years and older. Taking into account the presence of SAS, patients were divided into 2 groups: 1st – persons with hypertension, CHF and SAS (n = 84), 2nd – persons with hypertension, CHF without SAS (n = 77). All participants were assessed for risk factors and concomitant pathologies, they underwent physical examination, laboratory and instrumental studies, and testing according to the MMSE scale.

Results. In “fragile” female patients, in comparison with “fragile” male patients and “strong” females, statistically significantly more severe asthenia was observed, atrial fibrillation, chronic kidney disease, anemia, higher systolic and lower diastolic blood pressure indexes, more a high score on the RSCS scale, lower exercise tolerance, and also dementia of mild degree of severity. “Strong” females more often had an obesity comparatively to “strong” male and “fragile” female patients.

Conclusion. The results of the study allow us to make a conclusion, on the one hand, about the influence of SAS at clinical manifestations in patients with AH and CHF, and on the other, about the gender peculiarities of clinical manifestations in patients with AH and CHF, depending on the SAS presence.

What we know about gout today

Abstract

Gout is a chronic autoinflammatory disease that develops in individuals with hyperuricemia (HU) and is caused by inflammation at sites where urate crystals are deposited. Among the causes of the development of HU and gout, in addition to dietary habits, there are genetically determined disorders in the functioning of urate transporters in kidneys and intestines. Genesis of acute gouty arthritis is associated with a two-stage activation of NLRP3 inflammasome, inducing the synthesis of active interleukin 1β, which is clinically manifested by acute, excruciating pain in peripheral joints. Along with the peculiarities of the clinical picture, also radiation diagnostic methods are important in the early diagnosis of gout. The main aim of goat therapy is stable maintenance of serum uric acid levels in blood, achieved by regular use of urate-lowering medicines (allopurinol, febuxostat), which should be taken together with low doses of colchicine, nonsteroidal anti-inflammatory drugs or glucocorticoids in the first months of the treatment.

Sarcopenia (SP) is a multi-etiological syndrome manifested by a progressive loss of muscle strength and mass. It occurs either secondary to the natural degenerative processes, or as a complication of various chronic diseases, worsening the course and prognosis of them. SP presence increases the risk of falls and fractures, leads to decrease of life quality and negatively affects its duration. Current review presents modern data on the mechanisms of the occurrence and development of muscle weakness, also a diagnostic algorithm and possible methods for SP prevention.

Modern methods of surgical treatment of patients with rheumatic diseases

Abstract

The purpose of current review is to describe the general principles and tactics of musculoskeletal system surgical treatment in patients with joint inflammatory diseases, to differentiate surgical operations depending on the severity and reversibility of the damage of joint structures and periarticular tissues. To do this, we analyzed foreign and domestic literature on the problem of surgical treatment of joints in patients with rheumatic diseases (RD), and emphasized current tendencies in rheumatic orthopedics. It could be stated that despite success in conservative treatment of autoimmune pathology, surgical treatment is also required for patients at different stages of RD. At early stages, synovectomy or tenosynovectomy is usually performed; in case of advanced erosive damage of the joint, the operation of choice is endoprosthetics; in some cases, joint arthrodesis is performed. Surgical treatment of joint pathology both in the early (organ-preserving operations) and late stages (endoprosthetics, arthrodesis) is a reliable and effective method of treating RD, allowing to avoid disability in young patients, improve their quality of life, as well as the function of the upper and lower extremities.

Clinical estimation of liver functional tests

Abstract

To assess liver condition, a wide range of biochemical tests are used, which are referred to as “liver functional tests” (LFT). They show not only the functional condition of the liver, but also the nature of damage of its main structures. Current publication provides a detailed description of screening biochemical indexes (alanine transaminase, aspartic transaminase, alkaline phosphatase, gamma-glutamyltransferase, total, conjugated and unconjugated bilirubin, prothrombin time, prothrombin index, international normalized ratio, total protein and protein fractions), as well as the main pathogenetic liver damage syndromes (cytolytic, cholestatic, mesenchymal-inflammatory, hepatocellular failure syndrome). Correct LFT interpretation allows to determine the etiology, severity, clinical course and prognosis of the disease, as well as assess the efficacy of therapy. Current review analyzes various changes in LFT taking place in clinical practice. Comorbidity of chronic obstructive pulmonary disease and anemia: Clinical course peculiarities, diagnosis, therapy

Abstract

Article is devoted to modern aspects of the clinical course of chronic obstructive pulmonary disease (COPD) and anemia of chronic diseases (ACD). Peculiarities of diseases accompanying COPD, data from studies analyzing frequency and severity of comorbid pathology, and its influence at the course of COPD are examined in details. Characteristic changes in laboratory indexes are introduced in case when a patient has COPD and ACD at the same time. The review also provides information on existing algorithms for differential diagnosis of anemia in patients with COPD, with an emphasis on the specifics of ferritin and erythropoietin metabolism in this group of patients. Special attention is paid to the results of studies indicating increased symptoms and worsening survival prognoses when COPD and anemia are combined all together. Data collected in the article emphasize the importance of timely detection and treatment of ACD in COPD patients.

Abstract

The article describes a case of gastric ulcer exacerbation in a patient with type A hemophilia and chronic viral hepatitis C (CVHC). On that specific example, the rational choice of a proton pump inhibitor and painkillers is considered, taking into account comorbid pathology, and the efficacy of modern drugs for antiviral therapy of chronic hepatitis C is analyzed, taking into account drug-drug interactions. Alternative treatment options for hemophilia A in case of concomitant pathology in a patient are also discussed.

ANCA-associated systemic vasculitis: Aspects of diagnosis

Abstract

Systemic vasculitis associated with antineutrophil cytoplasmic antibodies (ANCA-SV) are among the rare rheumatological diseases, the importance of early detection of which is due to their poor prognosis. Crucial importance in ANCA-SV diagnosis has a detailed clinical analysis of the course of the disease and a detailed examination of patients with identification of pathognomonic signs, including asymptomatic ones. The article discusses clinical peculiarities of various variants of ANCA-SV, including granulomatosis with polyangiitis, microscopic polyangiitis and eosinophilic granulomatosis with polyangiitis, illustrated by our own clinical observations. New classification criteria for this group of diseases, proposed by international experts in 2022, are discussed. Taking into account the multi-organ nature of ANCA-SV, presented data may be useful to a wide range of specialists.

Nonsteroidal anti-inflammatory drugs in therapeutical practice

Abstract

Nonsteroidal anti-inflammatory drugs (NSAIDs) occupy a central position in the modern system of multimodal analgesic therapy, based on the complex use of drug-free methodics and pharmacological agents with different mechanisms of action. The multifaceted effect of NSAIDs determines their high therapeutic potential for such common diseases of musculoskeletal system as osteoarthritis, nonspecific back pain and pathology of periarticular soft tissues. With respect to their efficacy, NSAIDs exceed the maximum doses of paracetamol and are not inferior to opioids. When using anti-inflammatory doses of NSAIDs for 7–14 days, pain relief of ≥ 50% is observed in 40–50% of patients. However, NSAIDs can cause serious complications from the gastrointestinal tract (GIT), kidneys and, most importantly, cardiovascular system. Therefore, during their prescription, one should take into account the presence of comorbid diseases and risk factors, and use drugs with best safety profile (for a specific situation) and, if necessary, prescribe medicines to prevent gastrointestinal complications (proton pump inhibitors, rebamipide).

Abstract

At the Council of experts held at the end of 2023 under RSMSIM authority, its participants considered the problem of magnesium insufficiency and deficiency in the human body, adverse consequences of such deficiency, as well as measures of their prevention and correction. Taking into account the fact that magnesium is an element necessary for the normal functioning of the body, and its deficiency is widespread, correction of such deficiency will help improve the outcomes of many diseases, quality of life, well-being and general health of patients. Due to the availability of methods for diagnosing magnesium deficiency and availability of necessary drugs, it is necessary to develop and implement into medical practice an algorithm for assessing that clinical condition by doctors of various specialties in patients of risk groups, providing its timely correction and adequate prevention. Experts expressed a consolidated opinion on the need to develop Russian clinical guidelines for diagnosis, treatment and prevention of magnesium deficiency.

ACTUAL ISSUES OF PHARMACOTHERAPY AND PREVENTIVE TREATMENT

Pharmacoeconomic analysis of apixaban use for secondary thromboprophylaxis at outpatient stage among oncological patients

Abstract

Taking into account the need for long-term secondary thromboprophylaxis in patients with cancer incidence, assessing the cost-efficacy of using various therapeutic tactics in such patients is the point of particular interest.

The aim: to make a pharmacoeconomic analysis of validity of Eliquis® (apixaban) medicine use for the secondary prevention of deep vein thrombosis (DVT) and pulmonary embolism (PE) in patients with cancer comparatively to other oral anticoagulants’ effect.

Material and methods. The economic assessment was carried out from the perspective of the interests of healthcare of the Russian Federation. The assessment was based on data from previously performed meta-analyses, the results of which were published. Total costs for patient management for each of the compared alternative thromboprophylaxis tactics were determined by calculating the cost of drug therapy, as well as the costs of managing adverse events over 12 months. The conclusion about the most preferable thromboprophylaxis option was made in accordance with obtained data concerning the correlation between efficacy and cost of patient treatment.

Results. During the study, lowest total costs for patient management were fixed when using apixaban: they amounted to 35,082 rubles per year per person. The difference in costs took place mainly due to the cost of treatment with different drugs.

Conclusion. The results of a comparison of effects of apixaban with alternative drugs from the group of direct oral anticoagulants in the prevention of DVT and PE development due to oncologic pathology indicate that the use of this drug may be more valid from the standpoint of economic efficacy, assessed by such a pharmacoeconomic index as direct costs’ minimizing.
