Assessing the importance of faith as a possible predictor of treatment adherence

In practical medical activity there is often a need to make managerial medical decisions in conditions of time deficit, which dictates the need to identify a screening marker on the basis of which it is possible to quickly and reliably predict the patient's behavior in relation to his health. The paper examined the relationship between treatment adherence and religious faith. It is hypothesized that the importance of faith may play a role as a predictor of treatment adherence. The study included 567 respondents aged 33 to 68 years (mean age = 50.13±17.71 years). Adherence to treatment was examined using a specialized questionnaire, the Quantitative Adherence Questionnaire-25 (QAQ-25). To identify the importance of religious faith, respondents' answers to question 12 of the QPQ-25, "All people believe or do not believe in God. How important is it to you to believe in God?". Non-parametric methods were used for statistical processing of data. The study established a direct relationship between the level of adherence to treatment and the importance of religious faith. Importantly, the established patterns persisted regardless of gender and age. The study discusses that assessing the level of importance of religious faith may be a simple and universal screening marker and predictor of adherence to treatment, lifestyle modification, drug therapy, and medical follow-up. Such a marker is particularly interesting when a practicing physician needs to make a managerial medical decision dependent on a patient's potential treatment adherence with minimal time commitment. Thus, the authors hypothesize that identifying the significance of religious faith may determine future patient management tactics.

Clinical manifestation of liver damage in a COVID-19 patient

A new coronavirus infection (COVID-19) caused by SARS-Cov-2 is characterized by a variety of clinical forms and unpredictable course. To date, the virus has been found to affect not only the respiratory organs but also other systems, including the digestive tract. Typical gastrointestinal symptoms of COVID-19 include nausea, vomiting, loss of appetite to the point of anorexia, abdominal pain and diarrhea. Along with this, the examination of patients often reveals changes in biochemical parameters reflecting the functional state of the liver. Among the causes that can lead to damage to the hepatobiliary system, there are direct cytopathic or immune-mediated action of the virus on the liver, ischemia of the organ, drug-induced damage, exacerbation of existing chronic, as well as the debut of previously undiagnosed liver disease. As a rule, liver function impairment is characteristic of the stage of the developed clinical picture of COVID-19, but in some patients it can be detected during the rehabilitation period, which requires dynamic monitoring even after recovery and complete elimination of the virus. At the same time, clarification of the mechanisms of SARS-Cov-2 influence on the organism will allow to single out among the whole cohort of patients the high-risk groups for the development of certain complications of infection, and will also contribute to the development of individualized approaches to curative treatment.  This article systematizes the available data on possible etiological factors of liver damage in COVID-19 and presents our own clinical observation.

Differential diagnosis of chest pain

The presented clinical case shows the diagnostic search for the cause of chest pain in a 48-year-old woman, in whom ischemic heart disease was suspected during the diagnosis at the first stage, because the chest pain corresponded to the criteria of typical angina pectoris. Negative exercise tests, absence of pathologic changes on electrocardiogram, echocardiography, chest X-ray and the presence of expiratory dyspnea in the patient allowed to suspect bronchospasm. Spirography confirmed the presence of bronchial asthma, and against the background of the prescribed baseline therapy with inhalation glucocorticosteroid and long-acting β2-agonists the chest pain was controlled.

Conclusion. The presented clinical case indicates the need to detail the complaints of typical or atypical angina pectoris to specify additional symptoms on the part of the bronchopulmonary system in the form of expiratory dyspnea or cough to exclude signs of bronchoobstruction.

Assessment of the results of the state final attestation of graduates of budget and contractual basis of education

Scientific publications discuss the issues of the relationship between the results of the USE and students' academic performance, while mainly analyzing the interdependence of the exam in school and academic performance in junior courses of study. If we extrapolate these data to the period of higher medical education, we can find interesting correlations. This problem is relevant because there is a hypothesis that students studying on a contract basis are less serious about the educational process, hence, their GIA results are lower than those of budget students. The aim of the work is to evaluate the results of the state final attestation of graduates of contract and budgetary basis of education. Materials and methods. A retrospective comparative study and analysis of the results of GIA in students of medical and preventive faculty of 2022 was carried out. Statistical analysis was carried out using the methods of descriptive statistics. Results and Discussion. 382 students - graduates of USMU, among whom there were representatives of contract and budgetary forms of education, participated in all three stages of the GIA. The obtained data confirm the hypothesis about higher rates of testing and interview on the results of GIA in students of budgetary form of education in comparison with the contract form. In addition, the absence of a contract on targeted training in 6th year graduates also contributes to higher GIA results. Traditionally, a significant part of students in USMU are students of budgetary form of education (69.9 %), of which more than 50 % are on target training. The best indicators of test and interview results were in the group of students of budgetary form of education, who are not fixed by the contract on target training. Conclusion. The absence of the contract on target training, apparently, stimulates students to get higher grades in order to be competitive in the market of graduates of medical university, and to have advantages when entering clinical residency.
