Equipment of field units of the medical service of the Armed Forces with modern medical complexes on the basis of pneumo-frame modules, and procedure of the use

Abstract

On the basis of experience of medical supply in local wars and modern armed conflicts, when liquidating medical and sanitary disaster consequences the authors revealed the main organizational problems in army echelon of the medical service. It is shown that these problems can be solved by means of deployment of medical units and subunits. Requirements for means of deployment are defined. Present organic equipment is assessed. The authors showed advantages of pneumo-framed modules, their tactic and technical characteristics, variants of the use when deploying army levels of field medical evacuation.

Emergency state of health by arterial hypertension

Abstract

According to the Burdenko Main Military clinical hospital at the present time the frequency of crisis hypertensive heart disease is 16% from the total number of all patients with this nosology. Rapid relief of hypertensive crisis should be provided for patients’ status assessment from the position of development of fatal complications. Herewith the main task when treating hypertensive crisis is not normotension, but reduction of blood pressure to safe level, taking into account potential risk of hypotonia and hypoperfusion in case of too aggressive hypotensive therapy. The main factor of hypertensive crisis development is the absence of compliance. The authors gave recommendations on treatment of complicated and non-complicated forms of hypertensive crisis, and provided indications for hospitalization.

Development of a methodology of for medical-flight examination of flight personnel under condition of hospital

Abstract

When performing an experimental study of ways of improvement of hyperbaric ascent involving 95 servicemen of special contingent determined lack of ascent security of flight crews to altitudes up to 6500 meters due to the large percentage (10%) of revealed reduced hypoxia tolerance of mild and moderate level in military personnel and potentially high risk of high-altitude decompression disease without a preparatory desaturation. The experimental testing of 5 minutes ascent technique for 5000 m and 6000 m is performed, justified criteria of hypobaric hypoxia tolerance of mild and moderate level during the examination of health state of 589 aircrew personnel (including personnel with approved diagnoses) in hyperbaric chamber in relation to hospital conditions. It is experimentally shown that the method of the survey in the chamber on hypoxia tolerance of mild and moderate level in the ascent to the height of 5000 m and 6000 m is an effective mean of determining the functional state and reserve capacity of the organism to identify hidden forms of human diseases and can be used for medical-flight examination of flight personnel in a hospital.

Experience of the use of anti-viral medication «Grippferon» for acute respiratory viral disease and chickenpox

Abstract

The authors approved an epidemiological effectiveness of «Grippferon» as a preventive mean for acute respiratory viral diseases and tonsillitis in a military collective during the seasonal morbidity, and showed its high effectiveness against chickenpox. Side reaction and complications haven’t been observed in the military personnel that is why the given medication can be recommended as a non-specific preventive mean not only for acute respiratory viral disease, but also for chickenpox.

Erectile dysfunction in young men with high body mass index and arterial hypertension

Abstract

The authors assessed the status of erectile function depending on obesity in 69 young military servicemen with arterial hypertension of 1-2 stage. It was established that patients in this group suffered from weak erectile dysfunction, which was accompanied with endothelium vasodilatation disorder of brachial and penile artery, reduction of common testosterone, and reduction of free testosterone index in proportion to increase of body weight.

Hartmann’s operation: history and present requirements

Abstract

On the 5th of October 1921 at the XXX Congress of French Surgeons French Surgeon H.Hartmann reported about a new unique surgery, which could be used in the course of treatment of rectal and colon cancer, accompanied with acute bowel obstruction. The core of the given surgical procedure was an excision of the distal end of the sigmoid colon and upper ampullary section of straight intestine with tumour, colon exteriorization in a shape of an end colostomy, closure of distal segment with putting into small pelvis or leaving in abdominal cavity. Hartmann’s operation is a radical surgery, answering all modern oncology principles, and is a part of modern standards and clinical recommendation on treatment of accompanied rectal and colon cancer.
