Problems of diagnosing adjustment disorders in servicemen of the Armed Forces

Abstract

An analysis of mental disorders that occur in servicemen with adaptation disorders is presented. A study of the syndromic structure, anamnestic and follow-up information showed that the current diagnostic criteria for ICD-10 do not always allow an accurate diagnosis for adjustment disorders in military personnel. It has been established that the use of psychometric scales does not improve the accuracy of diagnostic conclusions. To develop proposals for improving the quality of diagnosis and prognosis in adjustment disorders, a structural-dynamic analysis and a level assessment of the characteristics of adjustment disorders were carried out in 1050 servicemen who received treatment in psychiatric hospitals of military medical organizations for various forms of adjustment disorders, their differential diagnostic and prognostic characteristics were determined. New fundamental approaches have been developed to optimize the diagnosis of adjustment disorders in servicemen.

Features of the organization of anti-tuberculosis activities in military educational institutions of the Ministry of Defense of the Russian Federation

Abstract

A detailed description of the features of the organization of anti-tuberculosis activities in military educational institutions of the Ministry of Defense of the Russian Federation is presented. Cadets of military educational institutions are a special category of patients. Their share in the overall structure of tuberculosis in military personnel in different years ranges from 5 to 15%. Of the clinical forms of the disease in cadets, infiltrative pulmonary tuberculosis predominates – 47.5%, focal pulmonary tuberculosis – 20.9%, tuberculous pleurisy – 9.5%. Bacterial excretion was established in 22.3% of cases, lung tissue decay – in 33.9%. Of those who fell ill, 11.8% were in the high-risk group for TB. The major risk factor in 75% of cases was contact with TB patients during the training period. Unlike conscripted military personnel, cadet military groups do not separate for a long time (4–6 years), which creates the prerequisites for the ongoing group incidence of tuberculosis. The causes and clinical and epidemiological features of three cases of group morbidity among cadets of military educational institutions, the effectiveness of modern methods of early detection of tuberculosis in foci, are analyzed.

Radioprotective properties of natural immunomodulators – ligands of toll-like receptors (Literature review)

Abstract

Existing radioprotective drugs have serious side effects. Ligands of toll-like receptors (TLRs), which are the one of the primary pattern-recognizing receptors of innate immunity, are considered potentially useful compounds for reducing the toxic effects of radiation exposure. On cell cultures, the radioprotective activity of TLR2, TLR5, or TLR9 receptor agonists has been shown, in particular, the suppression of radiation-induced apoptosis and an increase in cell viability. The drug CBLB502 (TLR5 ligand) contributed to the bone marrow protection and intestinal epithelial cells after irradiation of mice and rhesus monkeys. Activation of TLR4 by its ligand, lipopolysaccharide, reduces bone marrow damage and increases the survival rate of mice. The most promising anti-radiation drugs are agonists of several TLRs – the TLR2/6 co-agonist drug CBLB613 and TLR2/6/4 receptor co-agonists beta-D-glucans. The data on the scientific literature indicate the promise of using TLR ligands as a basis for the new anti-radiation agents development.

Chemotherapy in the treatment of patients with pulmonary tuberculosis

Abstract

A pulmonary tuberculoma is a clinical form of tuberculosis, in which a caseous-necrotic formation with a diameter of more than 12 mm has formed in the lung tissue under the influence of a chronic tuberculous process, delimited from the adjacent lung tissue by a two-layer capsule. The lack of a full-fledged blood supply, which prevents effective specific therapy, necessitates the prompt removal of pulmonary tuberculoma, and the presence of viable and virulent strains of Mycobacterium tuberculosis in the caseous contents necessitates the use of specific chemotherapy in the pre-and postoperative periods.

Syncope in military personnel is a problematic issue of military medical expertise

Abstract

Conducting a military medical examination of military personnel who have undergone a syncopal state requires a comprehensive analysis of all links of pathogenesis and the establishment of a causal relationship when the military medical commission makes a conclusion based on a detailed study of the available medical documentation. The article provides a modern understanding of syncope. The questions of the organization of the examination are considered in detail, allowing to correctly establish the causal relationship between the syncope that took place and the state of health of the serviceman.

On the risk factors for acute kidney injury in myocardial infarction in military personnel

Abstract

The influence of various factors on the risk of developing acute kidney injury in myocardial infarction in military men was assessed to improve prevention opportunities. The study included 502 patients under 60 years of age with verified type I myocardial infarction, 12 (2.4%) of whom were diagnosed with acute kidney injury. The following factors had a significant impact on the risk of developing acute kidney injury: unstable angina pectoris, arterial hypertension with a crisis course with a frequency of hypertensive crises more than once a month, no history of excess body weight, heart rate less than 64 per minute, left ventricular myocardial mass index 158.2 g/m2 or more, its ejection fraction is 60.6% or more, stroke volume is 79.4 ml or more, its late filling rate in diastole is 0.8 cm/s or more, total pulmonary resistance is less than 290.1 dyne×s×cm-5, serum sodium level 144 mmol/l and more in the first hours of the disease. Combinations of these factors show the risk of developing acute kidney injury in military personnel with myocardial infarction, which must be considered preventing this complication.

Life quality of obstetric patients in the late postoperative period after cesarean section

Abstract

The purpose of the study was to assess the life quality of obstetric patients in the late postoperative period after a caesarean section with various options for anesthesia. 310 patients were divided into three groups. Epidural anesthesia and prolonged epidural analgesia were performed in the primary group (n=112), combined spinal-epidural anesthesia and prolonged epidural analgesia were performed in the comparison group (n=95), epidural anesthesia was performed in the control group (n=103), and postoperative period — non-steroidal anti-inflammatory drugs. Quality of life was assessed using the SF-36 questionnaire. An increase in the quality of life was found in all patients in the first 6 months after surgery (the period of recovery after operative delivery and adaptation to a newborn child). Twelve months after the operation, higher rates in all parameters were registered in patients of the primary group. The use of prolonged epidural analgesia after cesarean section improves the physical and psycho-emotional state of obstetric patients, which favorably affects the course of the late postoperative period.

Polymorphisms of the BDNF (rs6265) and LIPC (rs2043085) genes associated with the violation of military professional activity in the Arctic zone

Abstract

The association of polymorphisms of 8 different genes with the pathology of the nervous and cardiovascular systems of military personnel serving in the Arctic zone was studied. It has been shown that military personnel with the G/G genotype (G/A polymorphism – rs6265 in the brain-derived neurotrophic factor BDNF gene) in combination with the C/C genotype (T/C polymorphism – rs2043085 of the LIPC gene) are recommended for service in the Arctic belt. Ceteris paribus, it is preferable that the genotype of officers serving in the Arctic zone be G/G for the BDNF gene and C/C for the LIPC gene (rs6265 and rs2043085 polymorphisms, respectively). From a prognostic point of view, the selection criteria for military personnel by genotypes will increase the combat capabilities of groupings of troops (forces) in the Arctic zone of Russia.

Prospects for the use of modern methods of water purification and disinfection in the field

Abstract

Based on the study of published materials and by analyzing databases posted on the Internet, the problem of comparing various methods of water treatment was solved with a view of their potential use in the conditions of field deployment of troops. It has been established that no separately applied method can guarantee the required purification and disinfection of water from any underground and open sources, and existing water treatment plants work with water of a specific chemical composition. Based on the study, water treatment methods have been selected that can be recommended for post-treatment and disinfection of tap water from worn-out distribution networks. Another group of methods is proposed for water treatment in liquidation of the consequences of sabotage, terrorist acts, emergency discharges of industrial wastewater from enterprises into open water bodies, into the city sewer network, when the operation of treatment facilities and the ability of water bodies to self-purify are violated. It is concluded that modern mobile water treatment systems should be designed based on a combination of several methods of water purification and disinfection.

Optimization of accounting for medical property in a pharmacy of a military medical organization using application software products

Abstract

In military medical organizations, various automated accounting systems are used to maintain material records of medical property, which are often not compatible with accounting systems. In the current situation, it becomes difficult to quickly collect information and report on the medical property. There is a lack of communication not only between medical supply units and accounting departments within the organization, but also between military medical organizations and military command and control. This paper highlights these problems and suggests ways to unify software products and include them in a single communication system.

New technique for performing the colonic anastomosis

Abstract

The new colonic anastomosis technique takes advantage of the hardware and continuous single-row suture. After mobilization of the intestine, the anastomosed areas at the level of the planned resection are placed with the mesenteric edges to each other in the form of a double-barreled gun. Sutured in the middle of the distance between the mesenteric and antimesenteric edges on both sides, evenly stretched over the ligatures in the transverse direction. The formation of the posterior lip of the anastomosis occurs by simultaneous stitching of all four walls of the intestine with the imposition of a hardware suture with a U0-60 device and subsequent cutting off of the intestinal section. Moving away from the posterior lip of the anastomosis, all layers of the anterior walls of the intestines are dissected at a distance equal to 0.8–0.95 of its width. The anterior lip is formed with a twisted single-row continuous serous-muscular-submucosal suture using atraumatic absorbable synthetic suture material 4/0 with the immersion of the corners of the hardware suture. The creation of a combined colonic anastomosis greatly simplifies the operation, reduces its time while maintaining sufficient efficiency.

Transcranial magnetotherapy in the complex treatment of post-covid asthenic syndrome in military personnel

Abstract

The clinical efficacy of dynamic transcranial magnetic therapy in the complex treatment of post-covid asthenic syndrome in military personnel was evaluated. 2 groups were formed: complex treatment (medication + dynamic transcranial magnetotherapy, n=17) and control (medication, n=38). The procedures were carried out on the apparatus AMO-ATOS with the prefix «Headband». The connection of magneto-therapy to the medical treatment of post-covid asthenic syndrome allows an earlier time (by the end of the 2nd–3rd week of treatment, 1–2 weeks earlier than in the control group) to lead to a subjective improvement in well-being, a decrease in anxiety, an increase in concentration and workability.

The history of the Main Center for State Sanitary and Epidemiological Surveillance (special purpose) of the Ministry of Defense of the Russian Federation is described. The stages of development of the organization are given. It is shown that over the 50 years of its existence, the specialists of the center have made a significant contribution to the preservation and strengthening of the health of military personnel. The main achievements of the team and its most prominent representatives are presented. The actual directions of the center’s activity at the present stage are demonstrated.

Organization of the provision of medical equipment to partisan formations during the Great Patriotic War 1941–1945

Abstract

The article presents data on the peculiarities of organizing the provision of medical equipment to partisan formations operating behind enemy lines during the Great Patriotic War. A description is given of the order of their medical supply during the formation of the partisan movement and after the transition to the leadership of partisan detachments from the front headquarters of the partisan movement, subordinate to the Central Headquarters of the partisan movement. Directly in the pharmacies of the medical units of the partisan formations, some necessary medicines and disinfectants were made. Information is provided on the transfer of medical property to assist the population of the occupied territories. The professionalism, patriotism, courage, and heroism of pharmaceutical specialists are shown, who in the most difficult conditions selflessly worked as part of partisan detachments, carried out partisan tasks in the underground, and contributed to the preservation of the life and health of partisans and underground workers.
