Abstract

The first published chapter from the book of memoirs of a veteran of the Great Patriotic War of 1941-1945, retired medical colonel Nikolai Atroshkin (born in 1923), is presented. In the army, a graduate of the Leningrad Military Medical School, military medical assistant N.T.Atroshkin was in May 1942, then participated in the battle on the Kursk Bulge, freed the Poltava region from the Nazis, forced the Dnieper to be seriously wounded. He began to write his memoirs in the period of the 1980s. The central theme for the author remains front-line medicine with all its nuances. Memoirs do not leave a bitter residue on the reader, for in the person of the author is a person with a kind heart and a heightened sense of justice.

Military psychiatry during the Great Patriotic War

Abstract

The features of the organization of psychiatric care during the Great Patriotic War are analyzed. The pre - war period of military psychiatry was studied when there was no experience in providing psychiatric care in combat conditions. The structure of sanitary losses of a psychiatric profile during the war years is studied. It is shown that the most significant part of the victims of this category was «contused». This term, as a clinical and organizational concept, due to the specifics of its attitude to mentally ill military personnel, was widely used only in our country. The role of individual military psychiatrists in the formation of the organization of psychiatric care during the war years is investigated. It is shown that in the absence of consistency and unification of the system for providing this type of medical care for all fronts, it changed dynamically based on current capabilities and needs, as well as acquired experience. The Great Patriotic War had a significant impact on the development of military psychiatry. Her experience was analyzed and generalized in a series of scientific works, most of which became classical.

The characteristic of the production activities of sanitary warehouses and military pharmacies is given, attention is paid to the work of pharmacists and pharmacists in besieged Leningrad. The significant contribution of the S.M.Kirov Military Medical Academy to the training of qualified pharmaceutical personnel for the needs of the fronts and armies was revealed.

The system of staged medical rehabilitation of patients after hip arthroplasty in a military clinical health resort

Abstract

The paper sets out the modern principles of medical rehabilitation of patients with total hip replacement. Based on which and with the involvement of the author’s experience in surgical treatment and rehabilitation of 128 patients in a specialized balneological health resort, an algorithm for the staged rehabilitation of such patients is proposed. Long - term results were traced in terms of 12-72 months. Pain after surgery was absent in 105 patients or was weak in 14 (93% of all patients), lameness was completely absent in 86 (67.2%), was weakly expressed in 28 (21.9%). In general, positive results were obtained in 98.4% of patients: excellent - in 76 (59.4%), good - in 29 (22.6%), satisfactory - in 21 (16.4%) patients, unsatisfactory - only in 2 (1.6%).

A model of expert decisions in cases of medical care of a cardiological profile

Abstract

In the Kostroma region, a departmental examination of the quality of medical care for cardiac patients and their dynamic monitoring (800 completed cases, including 400 of the outpatient level; 400 of emergency medical care) was conducted for research purposes. The effectiveness of medical care was evaluated according to 4 criteria: compliance with the standard, achievement of a clinical result, patient satisfaction, and resource - saving. It has been established that one of the medical and organizational problems of medical care is the high frequency of defects in its provision, including 6440.7 at the outpatient stage and 866.3 per 100 cases at the emergency medical stage. The structure of the causes of defects is dominated by insufficient qualifications of doctors, lack of continuity in the transmission of information, lack of equipment, incomplete scope of diagnostic research. The results of the analysis of departmental examination data for cases of cardiological care allowed us to identify ten groups of options for ensuring the effectiveness of medical care, which became the basis for the developed model of expert decisions on cases of assessing the quality of medical care of a cardiological profile.

Videothoracoscopic interventions for thymic neoplasms

Abstract

The purpose of the work was to study the immediate results of a thymectomy from «open» and video - assisted thoracoscopy. The results of examination and treatment of 74 patients with thymus tumors were analyzed. Patients operated on from «open» and minimally invasive approaches were comparable in age, gender, and morphological structure of the tumor. The duration of thymectomy from thoracoscopic access, the volume of blood loss was significantly less during video thoracoscopic surgery. The early postoperative period of minimally invasive interventions was characterized by a significantly lower frequency of complications, the duration of drainage of the pleural cavities, shorter stay of patients in the intensive care unit and the hospital. Endovideo surgical thymectomy has several advantages over traditional access.

Diagnostic value of various methods of radiation diagnosis in military personnel with ruptures of the anterior cruciate ligament

Abstract

A comparative analysis of Х-ray studies of the ligamentous apparatus of the knee joints and magnetic resonance imaging data with arthroscopy results for 400 military personnel (18-60 years old) with a history of a knee injury and suspected anterior cruciate ligament rupture is performed. It was shown that there is a relative discrepancy between the data of functional X-ray studies on the instability of the ligamentous apparatus of the knee joints with the results of arthroscopy. Results of the functional Х-ray studies of the ligamentous apparatus of the knee joints for instability were confirmed only in 79% of cases, and magnetic resonance imaging results of arthroscopy in 95%, which indicates a higher specificity of the method.

Simultaneous and consecutive lung resection in bilateral tuberculomas

Abstract

The purpose of the work was to justify the feasibility of expanding surgical care for patients with bilateral pulmonary tuberculosis by introducing simultaneous bilateral lung resections from transsternal access. The most radical and commonly used interventions for bilateral tuberculosis are bilateral lung resections performed in the same or sequential versions. Under the supervision of the author, there were 420 military personnel members operated on for pulmonary tuberculoma, including bilateral operations were performed in 130 (31%) patients, of which successively - in 18 (13.8%) and simultaneously - in 112 (86.2 %). The optimal condition for performing simultaneous bilateral lung surgery is to achieve clinical and radiological stabilization of the underlying disease, which consists of resorption of perifocal infiltrative changes, delimitation of the pathological process by 1-2 segments or lobes of the lung. All patients at the time of discharge were in satisfactory condition. An X-ray examination revealed that all patients had their lungs straightened, normal pneumatization, no signs of active tuberculosis.

Medical and economic effectiveness of the use of the human papillomavirus vaccine in female military personnel

Abstract

The human papillomavirus is one of the main risk factors for women to develop several oncological diseases and anogenital warts. Vaccination provides a reduction in the incidence of these diseases and mortality from them. The study aimed to assess the medical and economic effectiveness of the use of the tetravalent vaccine against human papillomavirus in female military personnel. Epidemiological data on the incidence of anogenital warts in St. Petersburg, as well as published Russian data on the frequency of other diseases associated with the papillomavirus, were used as model data on the medical and economic effectiveness of the vaccine. Based on the results of the analysis of the obtained data, it was concluded that vaccination of female soldiers with the indicated vaccine would provide a pronounced social and economic effect: per 10,000 vaccinated, increase the average duration of the survival period by 280 person - years and reduce budget costs for treatment of virus - associated diseases 2.55 million rubles.

The reaction of the cadets of the military educational institution at the initial stage of adaptation to the learning environment

Abstract

Three groups (control - KG, 2 experimental - OG1 and OG2) of cadets (20 people each) from a military school who arrived in the Kaliningrad enclave from other regions of Russia were examined. Blood sampling was carried out three times: in the first 2-3 days after passing the entrance exams and the beginning of the learning process, after 15 and 45 days. The reaction of the body in the initial period of acclimatization and adaptation to the learning conditions was evaluated by the levels of protein and protein fractions. Disadaptation changes in the body are established, proved by changes in the levels and percentage of albumin, a1-, a2-, b - and g - globulins of blood serum. The effectiveness of prophylaxis of dysaptation shifts in the optimization of the diet by products manufactured using cryogenic technology was evaluated. OG1 took a protein - vegetable product, OG2 from plant materials (2 times a day, 3 tablets. 0.3 g each with food for 15 days). KG did not accept products. The inclusion of concentrated foods in the diet is accompanied by a significant decrease in inflammatory reactions, stimulation of humoral immunity. During the adaptation period, preference should be given to products containing animal protein.
