Organization of hematological care in the Armed Forces of the Russian Federation: acute issues

Abstract

A clinical audit of 50 hematological patients in the departments of hematology (37) and bone marrow transplantation (13) was carried out. The problems that require an audit have been identified and a strategy for their solution has been worked out. The characteristics of the stages of clinical audit are presented. The implementation of requirements for the use of antimicrobials in patients with neutropenia, the implementation of a set of measures for the diagnosis of neutropenia, as well as compliance with recommendations for the care of patients are assessed. The full implementation of the latter in the department of bone marrow transplantation and partial (25-85%) - in the department of hematology was established. An assessment was made of the extent to which the standards of antibiotic therapy and patient care for each department were complied with. A high level of bacteremia in patients with neutropenic fever was revealed. Questionnaires on the clinical audit of antibacterial therapy and compliance with the rules of care for patients with neutropenic fever, which will improve the quality of medical care.

Indicators of mental health of servicemen who served on conscription in 2003-2016

Abstract

A statistical analysis of the mental health indicators of military personnel who served on conscription in the Armed Forces from 2003 to 2016 is presented. Medical and statistical indicators for the V class of diseases «Mental and behavioral disorders» were studied. The average annual levels of mental disorders were established: the total morbidity - 20±0,9‰, the primary - 12,9±0,4‰, the need for dispensary dynamic observation - 2,2±0,2‰, hospitalization - 12,6±0,6‰, days labor losses - 368±15‰‰, layoffs for health reasons - 8,9±0,5‰. A tendency has been found to reduce the proportion of mental health disorders in the structure of the basic medical and statistical indicators of the health of military personnel taking military service on conscription, while stabilizing the rates of dismissal due to mental disorders. It is shown that during the psycho-prophylactic work in the troops, attention should first of all be focused on the 5th group (neurotic stress-related disorders and somatoform disorders), whose high morbidity rates and dismissal determine its greatest medical and social significance.

Organization of treatment-and-prophylaxis measure by radiation injury

Abstract

In the article, features of the organization of treatment-and-prophylactic measures at radiation injuries at the stages of medical evacuation are considered. It is shown that the organizational solutions for the provision of medical care in the case of radiation injuries are determined by the characteristics of the damaging factors, the time elapsed after the radiation exposure, the number of medical organizations profiled to provide a certain volume of medical assistance, and a number of others, dislocated, and a number of others. Specific characteristics of the antiradiation protection, recommended for use, are presented: stimulants of radioresistance of the organism; prevention and reduction of the primary reaction to irradiation; prevention of lesions from irradiation with incorporated radionuclides; treatment of radiation lesions of the skin and mucous membranes.

Influence of preventive application of b-estradiol on survival rate and bone-marrow hemopoiesis in irradiated mice

Abstract

The radioprotective efficiency of b-estradiol was studied in its preventive administration under conditions of acute irradiation of mice. We studied 30-day survival, the average life expectancy of dead animals. To study bone marrow hematopoies, methods of endogenous and exogenous colony formation were used, and the dynamics of hematological parameters of irradiated mice were determined. It has been established that the prophylactic use of b-estradiol at a dose of 20 mg/kg for 5 days prior to radiation exposure protects against radiation death of animals exposed to radiation at doses of SD50-70/30, increasing their survival by 17-58%, and contributes to a decrease in the severity of post-radial disorders of bone marrow hematopoiesis.

Influence of high epidural block on myocardial contractility in patients with unstable angina

Abstract

The influence of high thoracic epidural block with Naropin® on the diastolic function of the left ventricle in patients with unstable angina has been studied. The study was performed on the basis of analysis of the results of examination and treatment of 36 patients, men aged 66,3±2,1 years, suffering from coronary heart disease with unstable angina and disturbing diastolic function of the left ventricle. In 24 patients, the left ventricular ejection fraction was lower than 50%. A local anesthetic Naropin® (0,5% solution) was administered epidural at a dose of 50-150 mg per day for 1-7 days before stabilization. The study (Doppler echocardiography) was performed an hour after the epidural blockade. It is proved that epidural blockade improves local and global contractility of the left ventricle, its diastolic function, increases perfusion in the area of stenotic coronary vessels. The revealed properties of epidural blockade allow us to recommend it as an effective means of intensive care for unstable angina.

Possibilities of MRI with the use of diffusion weighted imaging as a part of liver lesion diagnostics

Abstract

Five hundred forty five focal liver lesions were studied (249 patients). The study was carried out on a high-field magnetic resonance tomograph with the use of magnetic field strength of 1,5 T. In the course of the study, the possibilities of magnetic resonance diffusion in the diagnosis of these pathological conditions were studied. It is shown that the technique for obtaining diffusion-weighted images is effective and requires the mandatory inclusion in the scanning protocol of the discussed patient groups.

Ethiology of acute respiratory diseases in conscripts in the Central military district

Abstract

The article presents an assessment of the etiological structure of respiratory infections, based on data on sicknesses of conscripts during the epidemic of influenza and acute respiratory viral infections in 2016-2017. The high epidemiological significance of the viruses of non-influenza etiology was determined, which was determined 3,3-4,5 times more often among the patients in different collectives than the influenza viruses. The share of adenoviruses detected in patients with acute respiratory viral infections was 53,3-75% of the total number of identified viruses, which indicates the need for additional protection of the military personnel with antiviral drugs.

First experience of water purging from natural radionuclide with the use of new generation nanosorbent

Abstract

He results of studying the sorption properties of new filtering materials of the CARBOVER-PSUM type obtained on the basis of innovative nanotechnology are presented. Assessment of sorption characteristics was carried out in laboratory conditions for basic hygienically significant natural radionuclides contained in fresh water (226Ra, 210Pb, 210Po, 238U). The efficiency of CARBOVER-PSUM type nanosorbents, first obtained in the world practice on the basis of plasma stimulation of carbon materials, is established. Attention is focused on studying the processes of sorption of the natural radionuclide 226Ra. When filtration of distilled water with dissolved in it 226Ra, the natural radionuclide practically settles on the surface of sorbent grains. A similar effect is observed with natural radionuclides 224Ra and 228Ra, which are chemically complete analogues of the natural radionuclide 226Ra. A number of scientific and technical solutions discussed in the article have patent protection, and the proposed filter compositions are a trademark ®.

Use of high technology and modern technical means for production of medically pure oxygen under hospital and battlefield conditions

Abstract

The article describes the technology of short-cycle heatless adsorption, as well as the main blocks necessary for the creation of a mobile unit for the production, storage (storage), oxygen distribution of medical gaseous 93% in military medical organizations. It is shown that the technical characteristics of the installation and the quality of the oxygen obtained by the medical gaseous 93% largely depend on the correct selection of the component materials, components and elements. It is revealed that the creation of a domestic installation that realizes the technology of short-cycle adsorption will allow to obtain the oxygen of medical gaseous 93%, its storage and bringing it to functional units in field and stationary conditions.

Physiological and psychological assessment of the specialists of the mountain unit that participated in the competition «Elbrus Ring-2016»

Abstract

Within the framework of the International Army Games-2016, the Elbrus Ring Mountain Competition was held for the first time. In accordance with the program of the contest in the highlands, the teams performed radial acclimatization trips (1-9th day), a 5-day 85-kilometer march (10-15th day), climbing Elbrus (17-18th day). To assess the functional state of the body of servicemen and to predict the degree of success in performing tasks for the purpose in the conditions of combat training, methods that took into account volitional qualities (timed inspiratory capacity, the methodology of Nemov R.S.), the stability of the nervous system (indicators of simple visual-motor reaction), reserve capabilities of the body (timed inspiratory capacity, Martinet-Kushelevsky test). The application of these methods allows us to assess the functional state of the body of servicemen of the mining subdivision and to predict the degree of their success in carrying out tasks for their intended purpose during the period of combat training.

Current expert tasks in examining victims and people acting as a possible source of sexually transmitted diseases and HIV infection should be clearly structured and solved in a certain sequence, dictated by the content of clinical and laboratory examinations and analyzes. The evaluation of the data obtained should be carried out in accordance with the content of articles 121 and 122 of the articles of the Criminal Code of the Russian Federation and the results obtained. The main issue of this type of examination is the establishment of medical signs of a causal relationship between the victim’s contact with the suspect and the consequences of infection with a particular form of the disease.

Adaptation of infantile cardiovascular system to education in cadet schools

Abstract

To study the adaptation of the cardiovascular system to the new conditions of life, 119 students of cadet school were examined, including 64 boys and 55 girls aged 10-12 years. Predictors of sinus bradycardia, detected two years before its manifestation in the surveyed, were an excessive orthostatic increase in heart rate and the duration of the maximum pause of the rhythm, exceeding the threshold for a given age values. The most significant prognostic markers for the resistance of elevated blood pressure in children were: SBP and / or DBP values і90 and Ј95 percentiles with gender, age and growth in one-stage measurements, hypertension in relatives of the first degree of kinship, excess (over 75 percentiles for this age and height) body weight. The ordered mode of organization of life activity in a cadet educational institution facilitated the rapid (within six months) adaptation of children, which resulted in an improvement in night sleep indicators, a decrease in the degree of night breathing disorder, and a decrease in the daily profile of blood pressure.

Case of a difficultly diagnosed long-term, not resolved cryptogenic organizing pneumonia

Abstract

A description of the case of an atypical course of pneumonia is presented, which required a lot of efforts of clinicians and radiologists for diagnosis and positive outcome of the disease. It is shown that under the guise of the course of a common infectious disease, a rare pathology - cryptogenic-organizing pneumonia - can hide. Such patients are often long treated for «protracted pneumonia» and take many courses of antibacterial drugs for several months without effect. When pathogenic substantiated treatment with systemic glucocorticosteroids, there was a complete resolution of changes in the lungs and clinical recovery.

Past and present of the Military-medical museum (to the 75th anniversary of the founding)

Abstract

The Military-medical museum was established on November 12, 1942. It is the successor of the first medical museums: the artisian’s house, the Surgical museum at the Medical-surgical academy, the Museum of the N.I.Pirogov’s surgical society, the Military-sanitary museum of the Red Army and Navy, and others. More than 450 thousand museum items, including more than 300 personal funds of prominent domestic scientists: Botkin S.P., Pirogov N.I., Pavlov I.P., and etc. An archive of military medical documents has been created. The museum conducts scientific research on the history of military medicine, fulfills an important humanitarian mission. The modern stage of the museum’s life is connected with the formation of a new image of the museum, its exposition-exhibition, stock, restoration and other spaces, the material and technical base, as well as the emergence of young creative personnel, the mastering and introduction of new museum technologies. A significant place in the work is devoted to patriotic education, the formation of a fully developed personality. The museum uses a variety of forms of work with visitors (author’s tours, lectures, quizzes, quizzes, quests, etc.). The museum is a permanent participant and a diploma of intermuseum actions and competitions (Night of Museums, Intermuseum, Children’s Days in St. Petersburg, Museum Olympus, etc.).

Miltary-medical museum at the international festival «Intermuseum-2017»

Abstract

The article is devoted to the participation of the Military-medical museum in the international festival «Intermuseum», held on May 25-29, 2017 in the Moscow Manege, and united about 300 of the best Russian and foreign museums. The Military-medical museum made a special program dedicated to a healthy lifestyle, having passed to the semifinal of the contest in the nomination «The best project aimed at social interaction». The stand of the museum at the festival, designed in sports stylistics, attracted children, young people and experienced football fans.
