Improving the system of medical rehabilitation in a multidisciplinary treatment and rehabilitation center

Abstract

Developed and implemented into the practice of the branch No. 2 of the A.A.Vishnevsky 3rd Central Military Clinical Hospital effective organizational system of medical rehabilitation. The development is based on a unique long-term experience in the rehabilitation and treatment of combatants, military personnel, veterans of the armed forces and their families. The system includes the standardization of the process of providing medical services; multidisciplinary technology of medical rehabilitation; assessment of the quality of medical services; the use of classical and innovative kinesitherapy, aqua-kinesitherapy, apparatus physiotherapy, hydrotherapy, balneotherapy and thermotherapy in combination with psychotherapy, hyperbaric oxygen therapy, hypoxytherapy, manual and osteopathic therapy, shock wave therapy, herbal therapy, reflex therapy, hirudotherapy, salt therapy, and assessment of effectiveness of the applied methods.

Definition of concepts in combustiology

Abstract

The combustiology thesaurus contains a large number of indefinite concepts and terms, which makes the methods and methods of studying an object unsuitable, and information is distorted. In this connection, the organization of scientific research, the training of specialists and the creation of regulatory documents are complicated. To translate the content of general concepts and terms into concrete indicators, they need to be operationalized - bring them to a form that allows you to work with them on a practical level, operate with them when solving specific analytical and forecast tasks, verify or falsify research hypotheses. With this method, I class III burns are clinically classified according to the severity of the injury. An exception was proposed in the final diagnosis of injury of the actual area of I degree burns.

The article reflects the characteristics of the concept of «the heart of a soldier» and how the understanding of this problem has historically changed. The publication is based on a study of available domestic and foreign sources of literature. First, a historical background of the symptom complex, called «the heart of a soldier», during periods of various hostilities. Further, studies on the genetic diagnosis of the etiology and pathogenesis of this condition are described in more detail.

Cardiac sarcoidosis: clinical manifestations, modern diagnostics, and treatment

Abstract

Systemic damage involves a multidisciplinary approach to its diagnosis and treatment and regular follow-ups. Damage of the heart and nervous system is often fatal. At the same time, cardiac sarcoidosis is often asymptomatic. In this connection, early diagnosis and timely treatment are extremely important. The article describes the clinical manifestations of cardiac sarcoidosis, modern methods of examination and possible treatment regimens. Clinicians in the initial examination should remain wary of cardiac lesions. Modern non-invasive method of visualization of the heart - positron emission tomography/Computed tomography allows the doctor to exclude or confirm cardiac sarcoidosis at the initial stage of the examination, correctly stratify the risk of adverse outcome and initiate optimal therapy.

The role of evaluation factors of caries resistance in atopic dermatitis patients with chemical bleaching teeth discoloration

Abstract

The aim of the study was to determine the dependence of the effectiveness and safety of chemical bleaching of dental discoloration in adult patients against the background of atopic dermatitis on the state of mineral metabolism, factors of caries resistance, oral microbiota and some antimicrobial peptides. The main group consisted of 38 patients with dental discoloration against the background of atopic dermatitis, the comparison group - 46 patients with dental discoloration but without atopic dermatitis. It has been shown that patients with atopic dermatitis should consider the characteristics of solubility and mineralization of tooth enamel, impaired ratio of colonization of the oral cavity by cariogenic flora and its antagonists simultaneously with a decrease in the activity of innate antimicrobial immune reactions, since these factors may adversely affect the effectiveness of whitening and contribute to the development of complications.

The use of shock wave therapy in the treatment of patients with coronary heart disease with refractory angina

Abstract

The effectiveness of the use of shock wave therapy in the treatment of patients with ischemic heart disease with refractory angina has been studied. The results of treatment and medical rehabilitation of 46 male patients aged from 59 to 71 years, who made up the main and control groups of 23 people in each, are analyzed. Patients of the main group, along with standard drug therapy, were prescribed a course of shock wave therapy for the heart, consisting of 9 procedures. After the meeting, the number of angina attacks requiring additional intake of nitropreparations decreased, and after 6 months the total number of attacks decreased. The number of ischemic myocardial segments decreased. Improved contractile function of the left ventricle, increased exercise tolerance, decreased functional class of angina.

The state of psychological adaptation and refractive-accommodative system of visually-active labor servicemen.

Abstract

Professional activities of military specialists of visual-intensive work are associated with the risk of developing signs of psychological disadaptation, which is manifested in pronounced subjective symptoms (asthenopia, chronic fatigue syndrome) and a general decrease in the quality of life. The results of a comprehensive survey of 88 military personnel at the age of 25-34 years, whose professional activity is associated with electronic means of displaying information, are presented. It is shown that the effective prognostic indicators for assessing the level of psychological and visual disadaptation are the parameters of objective accommodation management. The presence of signs of psychological disadaptation indicates a possible decrease in the professional reliability of a specialist and requires the use of a multidisciplinary approach to correcting these violations.

Application of the method of computer biofeedback in the complex medical rehabilitation of military personnel with psychosomatic pathology

Abstract

The results of a study of the effectiveness of complex medical rehabilitation using computerized biocontrol in military personnel with psychosomatic pathology are presented. 69 patients were diagnosed with hypertension, 65 with myocardial infarction, and 63 with persistent bronchial asthma (the main and control groups were identified in each category). Rehabilitation programs for all patients included regimen, diet number 10, physiotherapy, phytotherapy, physiotherapy, and pharmacotherapy. Patients of the main groups additionally received bioacoustic correction sessions. It has been established that optimization of the functional state of the central nervous system with the help of bioacoustic correction contributes to the improvement of the vegetative regulation of the cardiorespiratory system, the normalization of blood pressure, and the reduction of peripheral vascular resistance. Increases tolerance to exercise capacity, improves bronchial patency. All this significantly increases the effectiveness of rehabilitation measures.

The use of high-intensity laser radiation for medical rehabilitation of patients undergoing arthroscopic surgery on the knee joint

Abstract

The effect of high-intensity laser radiation (wavelength 1064 nm, output power 12 W) on the efficiency of medical rehabilitation of 50 patients operated on by arthroscopy for chronic degenerative and traumatic pathology of the knee joint was studied. Were allocated to the main and control groups of patients by 25 people each. In both groups, in the postoperative period, drug treatment was applied. In addition to the generally accepted postoperative rehabilitation, patients of the main group received high-intensity laser therapy. Its course application led to the correction of microcirculatory perfusion and gave a pronounced anti-inflammatory and anti-edema effect, which contributed to the reduction of pain, improved locomotor function of the affected joint and increased the effectiveness of medical rehabilitation in general. It has been shown that the use of high-intensity laser radiation is an effective method for treating patients who have undergone arthroscopic surgery on the knee joint.

Osteopathic treatment for humeroscapular periarthrosis

Abstract

A comparative evaluation of the effectiveness of treatment of patients with shoulder-blade periarthrosis using the standard and complex (standard and osteopathic) approaches was carried out. The study included 30 patients aged 50 to 55 years (12 men and 18 women), divided into main and control groups. The control group received standard medical and physiotherapeutic treatment (nonsteroidal anti-inflammatory drugs, local anesthetics, massage, physiotherapy procedures), the main group received additional osteopathic treatment. During osteopathic examination, the state of the musculo-skeletal, craniosacral and visceral systems was assessed, after which treatment was carried out according to generally accepted regimens. The inclusion of osteopathic methods in the treatment protocol for patients suffering from humeroscapular periarthrosis allowed to relieve pain syndrome faster, increase the volume of movement in the affected joint, which indicates an increase in the effectiveness of rehabilitation measures for this pathology.

The authors presented a 30-year history of operation and the current state of the branch No. 2 of the A.A.Vishnevsky 3rd Central Military Clinical Hospital of the Ministry of Defense of the Russian Federation. Currently, the branch is provided with all necessary medical and rehabilitation equipment. The main practical activity of the institution is aimed at further improving medical care for servicemen and their families by introducing innovative methods of medical rehabilitation. The accumulated long-term experience of specialists in conducting medical rehabilitation of the wounded and sick allows us to provide medical care at the level of world standards.

The scientific biography of one of the leaders is presented. national clinic of internal diseases of the beginning of the 20th century, professor of the Military Medical Academy Sergey Sergeevich Botkin (1859-1910). The authors analyzed the extensive body of work of the scientist, publications about him, several archival sources for the first time found in scientific circulation. S.S.Botkin is also known for his activity in the field of artistic gathering, since 1904. He was a full member of the Imperial Academy of Arts. According to the authors, due to the early death, the entry of S.S.Botkin into the group of leaders of the therapeutic elite of that time remained unfinished.

Abstract

Presented a report on the conduct of based on the Military Innovation Technopolis «Era» and branch No. 4 of the 419th Military Hospital of the Ministry of Defense of Russia field training cycle «Principles of treatment of bone fractures in military medical organizations of the Ministry of Defense of the Russian Federation.» The purpose of the cycle was to obtain basic knowledge of the principles of surgical treatment of bone fractures and the acquisition of practical skills for use in everyday practice in the conditions of the trauma department of a military hospital. Successful conduct of the first cycle in the history of military traumatology allows planning the annual holding of this event in the military districts (fleets) of the Armed Forces of the Russian Federation.
