THERAPEUTICALLY RESISTANT ASTHMA

Abstract

Therapy resistant asthma (TRA) is most difficult to treat and its course is least predictable. TRA remains least studied notwithstanding the fact that its many mechanisms have been detected. To this point there has been no clear definition of TRA and this terminology has not been widely used so far. This paper gives the results of an investigation, which could identify the phenotype of TRA, by taking into account its pathogenetic and clinical heterogeneity, reveal the predictors of the disease, and elaborate unique criteria to make its diagnosis.

HIGH-DOSE FIXED ANTIHYPERTENSIVE COMBINATIONS IN THE TREATMENT OF ARTERIAL HYPERTENSION

Abstract

Recent trials have shown that the use of fixed-dose antihypertensive combinations increases the number of patients who can achieve target blood pressure (BP), significantly enhances patient compliance, and reduces the rate ofside effects. The proportion offixed-dose combinations of antihypertensive drugs (AHD) is, unfortunately, as high as 7.2% of the AHDs, as shown by the data of the PIFAGOR III trial and only 43% of physicians prescribe them. According to the results of this investigation, high-dose fixed combination of Noliprel A В-forte is safe and allows one not only to increase the number of patients with BP normalization in uncontrolled grade 2 and 3 arterial hypertension, but also to accelerate target BP achievement.

Abstract

Beta-blockers (BB) are known to be effectively reduce mortality and the risk of myocardial infarction, stroke, and other cardiovascular events in patients with coronary heart disease (CHD), chronic heart failure, arterial hypertension (AH), and arrhythmias. However, the use of nonselective and secondary generation relatively selectivite BB can be complicated by psoriasis exacerbation or psoriasiform erythema developement, an adverse reaction generally related to the undesirable β 2-adrenoreceptor blockade. One hundred and two hypertensive patients with severe exacerbated psoriasis and CHD were enrolled in the trial aimed to asses efficacy and safety of highly selective b1-blockerbisoprolol (Bidop, Gedeon Richter). The study results showed that bisoprolol while significantly reduced number of anginal attacks and lowered blood pressure, did not affect the psoriasis course and symptom improvement.

STRESS-PROTECTIVE EFFICACY OF PERSEN ® FORTE

The paper presents the results ofa large-scale open-label trial to evaluate the efficacy and safety of Persen® forte in arresting the conditions associated with acute and chronic psychosocial stress in 50 650 health care facility outpatients followed-up by 6300 physicians (neurologists and therapists). After two-week Persen® forte therapy, a considerable improvement was reported by 87 and 63% ofpatients with symptoms ofchronic andacute stress, respectively. Persen® forte was excellently and well tolerated by 87 and 89% of male and female respondents, respectively.

Russian National Research Medical University, Moscow The efficiency of using the fixed combination perindopril and amlodipine (Prestance, Servier, France) wasconsideredin patients with arterial hypertension (AH). Fifteen patients (mean age 55.13±11.1 years) with Stage II AH, grades ll-lll, andamoderate-to-high-risk for cardiovascular events, including those concurrent with coronary heart disease, were examined. Biochemical blood tests, Korotkoff office blood pressure (BP) measurements, and 24-hour BP monitoring were performed before and during treatment. Prestance was given once daily; the follow-up was 4 weeks. The use of Prestance resulted in a significant reduction in systolic and diastolic BP (SBP and DBP), a morning rise of SBP, and the rate of morning increase in DBP and unchanged both daytime and nocturnal heart rate. In addition, metabolic neutrality and no side effects of the fixed combination perindopril and amlodipine (Prestance, Servier, France) wereshown.

The paper analyzes the results of treatment with Pepsan-R that has anti-inflammatory activity by virtue of the fact that the inflammatory mediator is histamine and Pepsan-R blocks its containing mast cells in 50 patients with gastroesophageal reflux disease (GERD). Two formulations of the drug (gel and capsules) were evaluated. The average treatment duration was 28 days. Pepsan-R compares well with the currently accepted treatment options in the time of exacerbation abolishment, which causes the authors to recommend it for the treatment of GERD, by using mono- or combination therapy in relation to the form and severity of the disease.

The current standards recommend combination therapy for arterial hypertension (AH), but only 5% of the interviewed specialists prescribe 2 agents or more in its early stage. Over 85% of the respondents use a combination ofafew drugs in patients with grade III AH and signs of target organ damage. Only 56% of the interviewed patients show complete prescription compliance. One of the ways to enhance compliance is to prescribe the medicines that positively affect quality of life (OL). Thus, the use of a fixed combination of an angiotensin-converting enzyme inhibitor and a calcium antagonist could improve OL in six SF-36 items.
