Rehabilitation of patients with post-traumatic deformation of the orbital walls using thin-profile titanium nickelide implants with shape memory

Abstract
BACKGROUND: Despite the development of modern methods of reconstructive surgery, a number of controversial issues remain related to the lack of a unified approach to the treatment of traumatic fractures of the orbital walls and to the use of one or another method of orbital plastic surgery. Many methods of surgical rehabilitation are often technically complex or require expensive consumables.
AIM: To develop a technique for the restoration of the inferior and medial walls of the eye socket in patients with post-traumatic deformity of the eye socket using superelastic biocompatible titanium nickelide implants.
METHODS: A single-center pilot observational longitudinal study was conducted using a consecutive sample of 20 patients with traumatic injuries of the facial skeleton, specifically isolated fractures of the inferior and medial orbital walls. In all cases, restoration of the walls of the orbit and the natural position of the eyeball was carried out using thin-profile, super-elastic implants made of titanium nickelide. The study was prospective in nature, and qualitative outcomes of the restoration of the natural eyeball position were assessed. Outcome monitoring and evaluation of rehabilitation quality were carried out using instrumental methods according to the following diagnostic algorithm: ophthalmologic examination; intraoperative assessment of the implant position using a C-arm, as well as follow-up evaluation at all rehabilitation stages using computed tomography; and investigation of reparative processes in the implantation area using Karl Storz 30° and 45° endoscopes on the 10th postoperative day.
RESULTS: In all operated patients, it was possible to maintain or restore the position and function of the eyeball as an organ of vision. This method makes it possible to produce an implant that best matches the prosthetic area, which reduces and simplifies the installation time of the implant and, thereby, reduces the time and improves the quality of the entire surgical intervention.
CONCLUSIONS: The method developed and presented in the work has proven high efficiency and stability. In all operated patients, it was possible to maintain or restore the position and function of the eyeball as an organ of vision.
