POLYDRUG ABUSE AS A PATHOPLASTIC FACTOR INFLUENCING THE MANIFESTATIONS OF SCHIZOPHRENIA SPECTRUM DISORDERS

Abstract

The comparison of clinical-dynamic characteristics and social functioning indicators of 76 patients with schizophrenia and alcohol addiction and 46 patients with schizophrenia and polydrug addiction was carried out. The correlation between type of addiction, clinical-dynamic characteristics and indicators of social functioning were identified. Addiction to alcohol is associated with low intensity of psychopathological symptoms and higher rate of social functioning while polydrug addiction is associated with more severe schizophrenia course and low indicators of social adaptation.

SOCIAL AND PSYCHOLOGICAL PREDICATES OF ADDICTIVE BEHAVIOUR AT TEENAGERS

Abstract

The purpose of the study is to identify factors that affect the formation of addictive behavior in adolescents. It has been revealed that adolescent male addicts have a feminine type of gender role. Significant differences were found between the girls of the main group and the comparison group on the femininity parameter; adolescent addicts in the repertoire of coping strategies are dominated by unproductive cognitive coping strategies. A high level of development of such mechanisms of psychological defense as denial, compensation, substitution in the main group is revealed. In the group of addictive adolescents, a low level of indicators of social and psychological adaptation on the scales «adaptation», «self-acceptance», «acceptance of others», «emotional comfort”, «internality», and also «escapism» was diagnosed.

COGNITIVE DISORDERS IN OVARIAN CANCER PATIENTS AND THEIR CONNECTION WITH CLINICAL AND PSYCHOLOGICAL CHARACTERISTICS

Abstract

Neuropsychological assessment has been completed for 51 patients with histologically verified diagnosis of ovarian cancer. The results obtained have been compared with self-reported domains. It has been demonstrated that ovarian cancer patients experience cognitive impairment in at least two domains; the results reveal a statistically significant relation between neurocognitive dysfunctions, clinical parameters and psychopathological symptoms.

ASSOCIATION OF LRRK2 GENE POLYMORPHISMS WITH THE DEVELOPMENT OF PARKINSON’S DISEASE

Abstract

Mutations in the LRRK2 gene occur in 1-5% of patients with sporadic form and in 5-20% of patients with the family form of Parkinson’s disease, as well as in 1.8% of healthy individuals. The aim of the research was to study the association of polymorphisms rs7966550, rs1427263 and rs11176013 in the LRRK2 gene with the development of Parkinson’s disease. The research covered 49 patients with Parkinson’s disease, as well as 46 control persons. As a result, it was revealed that the carriage of the mutant CC genotype of polymorphism rs7966550 increases the odds ratio by 37.9 times. There were no significant differences in other polymorphisms. There was no correlation between the genotypes of the studied polymorphisms and the clinical picture of the disease. Statistically significant differences in ethnicity and frequency of different genotypes in polymorphisms have not been identified.

COMPLEX ASSESSMENT OF THE DYNAMICS OF CLINICAL SYMPTOMS AFTER TERMINAL SURGICAL INTERVENTIONS IN PATIENTS WITH TANDEM-STENOSES OF THE CERVICAL AND LUMBAR SPINE

Abstract

The paper analyzes the results of a study of 51 patients with tandem-stenosis of the cervical and lumbar spine at two levels and more due to herniated intervertebral discs, hypertrophy of the yellow ligament and arthrosis of the arcuate joints operated on the basis of the Neurosurgery Center of the Road Clinical Hospital Irkutsk JSC Russian Railways. In 22 cases, the primary operative intervention was performed at the lumbar level, in 29 cases - at the cervical level. The interim phase gap was 19 (15; 28) days. Anthropometric data (sex, age, body mass index) and duration of the disease were investigated. In the dynamics, the clinical parameters were assessed - neurological symptoms, the level of the pain syndrome according to the visual analogue scale, the severity of movements in the cervical spine (NDI), the quality of life of the patients according to the questionnaire for patients with back pain Oswestry (ODI), the degree of patient satisfaction with the operation performed on the scale of Macnab and objective dynamics of the neurological outcome on the Nurick scale, recorded before the operation, at discharge and 24 months after the operation. As a result, it was established that staged operative interventions in patients with symptomatic tandem-stenosis of the cervical and lumbar spine can significantly improve clinical symptoms in the postoperative period. At the same time, the important criteria for long-term clinical effectiveness are the duration of the disease before surgical procedures, the tactics of conducting primary surgical manipulations and the time between stages.

In this study we present the results of neurotic disorders observation taken during higher medical education. We analyzed such parameters as anxiety, obsessions and phobias, vegetative disorders, asthenia, and neurotic depression. Result of this study was determination of basic structure and nature of these parameters, which was described above by age, course of study and sex. Statistical processing was also included in the way of confirmation of our scientific theory.

Abstract

The high level of plasticity of the brain is mainly determined by the behavior of synapses, which can change their structure, functional activity; they can form again or disappear throughout the life cycle. Synapses are closely related to the presynaptic processes of astrocytes, which induce education, consolidate the structure and support the function of synapses, as well as participate in their elimination. Astrocytes produce numerous synaptonemal molecules that bind to the neurons and control synaptic plasticity. The review deals with the molecular aspects of violations of the mechanisms of interaction of astrocytes with synapses, which are crucial in the pathogenesis of a number of cognitive impairment.

Abstract

Hallucinogenic substances are usually classified into several different classes. Most classifications are based on the pharmacological mechanisms of action of these substances and their chemical structure. Psychedelics, entactogens, dissociative substances and “atypical” hallucinogens belongs to chemically heterogeneous group of substances with a hallucinogenic effect. All of the above classes of substances basically do not have common primary mechanisms of action. At the same time, they are similar in the ability to cause temporary, different in depth changes in consciousness, combined with changes in somatic status, perception, cognitive processes, and the emotions. Similar changes contribute to the recreational use of substances of this group. In recent years, a large number of works have appeared in the scientific literature, indicating the possibility of using these substances for therapeutic purposes. This review presents current literature data on several classes of hallucinogens, the therapeutic use of which is documented as much as possible.

The paper deals with an interpretation of the depression phenomenon from the perspective of the psychodynamic approach, in a framework of which we treat the loss and violence as a key etiological factor of psychic suffering. Dissociation mechanisms neutralize the psycho trauma and this inevitably causes more or less pronounced identity disorders. Corresponding abstract concepts provide the basis for psychotherapy to the depressed patient.

ABOUT ALCOHOL CONSUMPTION IN RUSSIA AND KALININGAD REGION

Abstract

The article analyzes the validity of calculations of the average per capita alcohol consumption for officially registered production and sale of alcohol. It is noted that there are other original methods for calculating the total (legal and illegal) level of per capita alcohol consumption by the number of deaths by violent death in a drunken and sober state. It is concluded that in a country with a high level of alcohol consumption there is no accurate data on the scale of alcohol consumption and alcohol policy does not take into account scientific data.

Empirically confirmed the increase in accuracy and differentiation of touch, increasing the accuracy of recognition of familiar faces and identification of photos of strangers on a given pattern, as well as the restoration of the integrity of the perception of scene pictures (which indicates the effectiveness of the restoration of Gnostic functions); increase the accuracy and pace of the motor act, as well as the restoration of motor programs.

The article presents a rare case report of combined development of multiple sclerosis and glomerulonephritis in a clinical observation of the patient who is on program hemodialysis. Possible disorders of the adhesive endothelial function in case of multiple sclerosis and glomerulonephritis were analyzed in detail. The role of intercellular adhesion molecules (ICAM-1 and VCAM-1) in the development of comorbid autoimmune diseases is discussed.

Abstract

The prevalence of age-related hypogonadism, cognitive and psychoemotional disorders in men of mature age with initial manifestations of cerebrovascular pathology is estimated. The initial manifestations of CCI were evaluated by cognitive and psycho-emotional indicators, hypogonadism - by the results of clinical and laboratory studies. High representation of age-related hypogonadism in patients with initial manifestations of chronic cerebrovascular disease was revealed. This may reflect the pathogenetic role of androgenodeficiency in the development of cerebrovascular disease and opens up the prospect of finding pathogenetic therapy for chronic cerebral ischemia.

A UNIFIED APPROACH TO THE DIAGNOSIS AND EARLY DETECTION OF SPEECH DISORDERS IN CHILDREN

Abstract

The aim of our research was the development of a unified approach to diagnosis and early detection of speech disorders in preschool children. The function of the articulatory apparatus is largely influenced by hypoxia of the fetus in the antenatal period and the effect of infectious agents in the postnatal period, while on the processes of formation of cortical speech centers - somatic malaise of the mother during pregnancy. Identification of adverse factors affecting the nervous system of the child in the perinatal period allows to determine the risk group for speech pathology. The scale for assessing the psychomotor and speech development of preschool children allows for a comprehensive assessment of the level of development of the child, on the basis of which the specialist can make the most individualized program for correcting developmental deviations.

THE ROLE OF THE BRAIN NEUROTROPHIC FACTOR IN DEVELOPMENT OF THE DIABETIC FOOT SYNDROME

Abstract

The quantitative content of brain neurotrophic factor in the serum of patients at the late stage of diabetic polyneuropathy was studied. Clinical and neurophysiological signs of moderate and severe diabetic neuropathy correlated with serum levels of brain neurotrophin were revealed. In patients with diabetic foot syndrome, there is a pronounced BDNF deficiency, which reflects a violation of the compensatory recovery capabilities of the nervous system. The depletion of brain neurotrophin at a late stage requires further study of the pathogenesis of diabetic neuropathy from the standpoint of neuroplasticity to improve the timely diagnosis and prevention of diabetic foot syndrome.
