Abstract

The article discusses the results of studies of drugs for the treatment of acute heart failure (AHF). The focus is on drug serelaxin (recombinant analogue of human relaxin-2), which is a promising drug for first-line treatment of AHF, allows to relief symptoms and improve clinical outcomes. Results of the study were the basis for assigning the expedited review status for registration of serelaxin in the Food and Drug Administration.

Abstract

The basic approach in the treatment of patients with Parkinson's disease (PD) is the use of levodopa and dopamine receptor agonists (DRAs). Prolonged intake of levodopa is accompanied by the development of fluctuations in symptoms and drug-induced dyskinesias; so, in some cases of PD, especially in young patients, ADRs are considered as drugs of choice and can be used both alone and in combination therapy. One of the best representatives of the ADRs that have extensive evidence base is pramipexole (Mirapex). The article analyzes the multiform properties of pramipexole and its influence on motor and non-motor manifestations of PD; possible neuroprotective effects of the drug are discussed. The potential of a new form of pramipexole with controlled release and sustained 24-hour action (Mirapex PD) is considered.

Pain affects the quality of life, negatively affects family relationships, can lead to disability or loss of productivity, and is associated with high costs for health care system and society as a whole. Non-steroidal anti-inflammatory drugs (NSAIDs) are the most popular class of drugs used as analgesics, especially when the pain is determined by the presence of tissue damage or inflammation. Ketorolac is NSAID available in parenteral, tablet and gel forms, which has potent analgesic effect, and its efficacy and safety in acute pain syndromes are well studied in numerous studies.

In ischemia, metabolism of myocardium is characterized by abnormal transition to preferential consumption of glucose as an energy source. This process results in activation of в-oxidation of fatty acids, which leads to undue consumption of oxygen, accumulation of toxic metabolites with prooxidant properties, and damage of cardiomyocytes. Trimetazidine, preparation with metabolic action, blocks the β-oxidation of fatty acids and provides energy-saving, anti-ischemic and anti-oxidant activities, and therefore is recommended for the wide use in cardiological practice.

Abstract

The article pays attention to the combination of the significant cardiovascular risk factors as arterial hypertension and dyslipidemia, which justifies the need for a comprehensive therapeutic intervention for many patients. On the example of patients with metabolic syndrome, diabetes mellitus and familial combined hyperlipidemia, the indications for antihypertensive and lipid-lowering therapy, effective drug combinations, target values of blood pressure and lipid levels are discussed. The results of clinical studies on pharmacological efficacy, tolerability, and adherence to therapy in patients with arterial hypertension and dyslipidemia on the example of a fixed combination of amlodipine and atorvastatin are presented, indicating the effectiveness of this approach, « two drugs in one pill» for reducing cardiovascular risk in primary prevention.

Abstract

Antihypertensive drugs of different pharmacological groups are used for the correction of blood pressure in patients with arterial hypertension (AH) and associated pathology of the digestive system. In the treatment of hypertensive patients with liver diseases, hydrophilic antihypertensive agents that are not metabolized by the liver should be used. Some antihypertensive drugs have a positive effect on the state of the gastrointestinal tract of hypertensive patients, increasing tone of the lower esophageal sphincter, preventing gastrooesophageal reflux, improving blood flow to the stomach, and have a protective effect on the gastric mucosa in peptic ulcer disease, and reduce the pressure in the portal system in liver cirrhosis.

Abstract

Article is devoted to a problem of cognitive impairment against the background of arterial hypertension, and potentials for the use of antihypertensive therapy for their correction. Data of own research on the effect of amlodipine, third generation dihydropyridine calcium antagonist, on level of blood pressure and cognitive functions at elderly patients with an 1 degree arterial hypertension are presented. It is shown that according data both on routine blood pressure measurements and 24-hour blood pressure monitoring, this drug has high anti-hypertensive effect. The results of study indicate that amlodipine improves a series of the parameters of cognitive functions. Abstract

Chronic migraine is a severe maladaptive disease, the pathogenesis of which is actively being studied, and the criteria and approaches to the treatment are continuously developed. One of the most studied drugs in the treatment of chronic migraine is a botulinum toxin type A (Botox®). PREEMT is the large-scale study of patients with chronic migraine, which consists of two parts: a 24-week double-blind, placebo-controlled phase of the random distribution, and a 32-week open-label phase. The study showed a significant reduction in the number of days with headache in the Botox® group compared with the placebo group, and the superiority of Botox® for a number of secondary efficacy parameters, including the degree of maladjustment.

Abstract

Sufficient magnesium concentration in the body is essential for the health. It is known that preparations based on magnesium salts of organic acids, in which the acid anion is a «carrier» of magnesium into cells, are characterized by high bioavailability. At the same time, these anions- carriers may also have their own «target» effects, also important for the maintenance of various systems of the human body. The article presents the results of the first chemoinformation analysis of orotic acid (OA) molecule. Similarities of orotate with a number of molecules of human metabolome and with various drugs were found. The results of the analysis in part explain the effectiveness of magnesium orotate in cardiological patients, which is well-known from clinical practice. Cardioprotective effects of OA may be realized by the regulation of the N-acetylglucosamin transferase, inhibition of phosphodiesterases, the regulation of the effects of PQQ coenzyme and, probably, anti-inflammatory action. In addition, the results of the chemoinformation analysis indicate antioxidant, immunomodulating, neuroprotective properties of OA, which currently remain largely unexplored. Thus, the results indicate that OA is independent active substance synergistic to magnesium, and outline the prospects for further research on the use of magnesium orotate in clinical practice.

Abstract

The article presents the results of use of drug Mexidol for the treatment of patients with dyscirculatory encephalopathy. It was found that Mexidol regulates cerebral hemodynamics, improves memory, emotional state, motor activity, increases the frequency of L-rhythm in both hemispheres of the brain, and reduces blood levels of cholesterol and low-density lipoproteins, thereby significantly improving the quality of life of patients with dyscirculatory encephalopathy and significantly reducing the risk of stroke in the future.

Abstract

This study was aimed to the evaluation of clinical efficacy, tolerability and safety of artradol in patients with cervical spine dorsopathies. The comparative clinical evaluation of 90 patients aged from 35 to 70 years was performed. Patients were divided into two groups. Main group of patients received artradol in addition to standard therapy of dorsopathies; comparison group received standard therapy alone. The results of study allow to conclude that artradol has a positive effect on the dynamics of neurological manifestations, good tolerability and high efficacy.
