MODERN CONCEPTS OF COMPLEX THERAPY OF HUMAN PAPILLOMAVIRUS INFECTION

Abstract

According to WHO, about 20 million people worldwide are currently infected with human papillomavirus (HPV). Low sexual culture of the population, frequent change of sexual partners increase the risk of developing HPV infection, contribute to the growth of the number of inflammatory diseases of the genital tract, which subsequently leads to violations of the reproductive function, as well as to the development of precancerous cervical diseases. The above-mentioned facts determine the search for effective therapy of HPV infection in order to prevent the development of cervical cancer. The article presents modern data on the feasibility of comprehensive treatment of HPV, which includes not only influence on pathologically altered tissues, but also non-specific antiviral and immunomodulating therapy

THIN" ENDOMETRIUM - MODERN VIEW OF THE PROBLEM

Abstract

This review is devoted to the analysis of the state of the problem of «thin» endometrium. One of the main ways to solve the problem of overcoming reproductive failures includes the correction of endometrial dysfunction associated with a violation of endometrial receptivity and the reparative potential, called «thin» endometrium. The features of the clinical manifestation and diagnosis of the syndrome of «thin» endometrium are described; the principles of choice of therapy aimed at restoring the morphofunctional state of the endometrium are determined. Diagnostic possibilities regarding the endometrial receptivity are currently insufficient. Their search will allow to increase the effectiveness of overcoming reproductive failures associated with the syndrome of «thin» endometrium in the future

ANALYSIS OF THE EFFICACY OF THE MINI-COMBINED TEST FOR THE PREDICTION OF MODERATE LATE-ONSET PREECLAMPSIA

Abstract

Background. Despite a significant number of studies, the predictors of preeclampsia (PE), especially the more frequently occurring late-onset PE, remain understudied, which limits the ability to predict and treat this threatening complication of pregnancy. Objective. Analysis of the efficacy of the mini-combined test for the Prediction of moderate late-onset PE. Methods. Based on a retrospective analysis of the clinical data of 645 pregnant women, two groups were formed: pregnant women with moderate late-onset PE and patients without PE. Clinical and anamnestic parameters (age, body mass index, mean blood pressure) and biochemical marker -pregnancy-associated plasma protein A - were assessed. Results. Prognostic significance of clinical maternal factors determining the prognosis of PE - age, mean blood pressure, and body mass index - was confirmed. The predictive efficacy of the mini-combined test was 44%, with 10% false-positive results. Conclusion. The low prognostic significance of the mini-combined test for late-onset PE is shown. It is concluded that for a timely early prediction of moderate late-onset PE, a search for new markers is necessary.

THERAPY OF LICHEN SCLEROSUS ET ATROPHICUS IN WOMEN

Abstract

Background. Lichen sclerosus et atrophicus (LSA) - a chronic inflammatory lesion of the skin and mucous layers, most often found in the anogenital region and manifested by itching, soreness, atrophy and scar deformation of tissues. The problem of LSA in genitals in women of different age is not currently solved due to the lack of standard methods of treatment. To prevent atrophic processes in stratified squamous epithelium of the genitals and recurrence of the disease, prolonged local therapy with glucocorticoids is carried out. However, long-term local use of strong glucocorticoids (according to the European classification Miller & Munro) cause epithelial atrophy and worsen the course of the disease. In this regard, it is advisable to use glucocorticoids with the absence of this side effect, including the cases of conjunction with drugs of other pharmacological groups. The appearance on the market of topical corticosteroids containing emollients (fats and fat-like substances that can create a protective lipid layer on the surface of the skin), as well as drugs with anti-inflammatory action, proved the development of a scheme for their effective use in LSA. Objective. To evaluate the effectiveness of topical corticosteroids of Methylprednisolone aceponate with ceramides (Komphoderm K) and topical calcineurin inhibitor Tacrolimus (Tacropic 0,03%) in the treatment of LSA in women at different age periods. Methods. 60 women with LSA voluntarily participated in the study. The first group consisted of 22 patients aged 18-45 years (reproductive period), the second - 46-75 years (menopausal transition, menopause and postmenopause). All patients were prescribed complex therapy: application of the cream containing topical GCS/ceramides (weak activity class), once a day for 2 weeks; after - 2 times a week for 4 weeks; from the 15th day after the start of treatment - application of topical calcineurin inhibitor (tacrolimus 0.03%) 2 times a day for 6 months were carried out. The effectiveness of treatment was evaluated at 2, 6, 12 weeks and 6 months after the start of treatment by the dynamics of the presence/absence of symptoms, as well as the severity of the most frequent of them (itching, atrophy and pallor of the vulva), revealed by the results of the pre-treatment examination. Results. The most labile symptoms of SAL, mostly occurring within 6 months after the start of treatment, were: in patients of all age groups (18-75 years) - itching, scaling, burning and lichenification of the vulva; In addition, in women of reproductive age (18-45 years) -dyspareunia, sclerosis, atrophy, hemorrhage and rigidity. Symptoms, relatively resistant to the therapy, were characteristic of older women and included dyspareunia, sclerosis, pallor, atrophy and vulva rigidity. The severity of the most significant symptoms of SAL (itching, atrophy, pallor) in the first 2 weeks after the start of treatment was significantly reduced, which indicates the early effectiveness of the therapy. Conclusion. Complex application of topical drug Komfoderm K (unhalogenized topical corticosteroid of a weak class of activity with a high level of safety, capable of long-term stay in the affected tissues, with the addition of humectants in the form of ceramides with emollient functions) and a topical inhibitor of calcineurin (Tacropic 0,03%) showed a high effectiveness in the treatment and prevention of recurrence of LSA in women in different age periods.

REALIZATION OF REPRODUCTIVE OPPORTUNITIES AND PROLONGATION OF REMISSION OF UTERINE FIBROIDS AFTER SURGICAL TREATMENT

Abstract

Objective. Assessment of the realization of reproductive opportunities and the frequency of recurrence of the disease in patients who underwent organ-preserving treatment for uterine fibroids (UFs) and a course of adjuvant therapy with mifepristone (Gynestril) for two years after drug discontinuation. Methods. A cohort prospective comparative study included 136 women who underwent organ-preserving treatment (myomectomy or uterine artery embolization) for UFs. Mifepristone was administered at a dose of 50 mg/day (1 tablet) starting from the 1 day of the menstrual cycle after surgical treatment in a continuous mode for 3 months, taking into account contraindications to its use. Results. It was shown that the use of mifepristone in continuous mode after surgical treatment for proliferative UFs led to the absence of relapses of the disease within two years after drug discontinuation. The use of mifepristone after uterine artery embolization significantly reduced node size by 25% at 12 months and by 50% (p<0.05) at 24 months. Complex treatment of UFs, including myomectomy and drug therapy with mifepristone, allowed to realize reproductive function in 46% of patients. Conclusion. Long-term results of the study make it possible to recommend mifepristone in the postoperative period for patients who underwent organ-preserving treatment for proliferating UFs in order to prevent recurrence of the disease within 24 months after drug discontinuation.

FEATURES OF CEREBRAL PATHOLOGY IN PREGNANT WOMEN: RESULTS OF RETROSPECTIVE CROSS-SECTIONAL STUDY

Abstract

Background. In the past decade, worldwide attention has been focused on reducing maternal morbidity and mortality. At the same time, there is an increase in the number of pregnancies against the background of extragenital pathology, among which a special role is assigned to diseases of the nervous system. Objective: to evaluate the features of the course of cerebral pathology in pregnant women for the expansion of the possibilities of differential diagnosis of various neurological complications during pregnancy. Methods. Archival medical records of pregnant and puerperant women who were admitted to intensive care units with suspicion of acute neurologic pathology for the period from 2012 to 2016 were analyzed. The study was conducted at the premises of a city multifield hospital in St. Petersburg. The analysis included 28 case histories. The patient age, gestational age, the final diagnosis, the results of instrumental methods of diagnosis during hospitalization, the results of laboratory methods of examination at admission and at discharge, as well as the blood pressure level of arterial pressure at admission were registered. Results. It was found that the age of patients during the study period ranged from 19 to 46 years. All patients were delivered at gestational age of 28-41 weeks. The structure of neurological pathology during pregnancy was multiform and included encephalopathy, strokes, brain tumors, and epilepsy. The most common cause of hospitalization of pregnant women was mixed encephalopathy. As for laboratory data, it should be noted that the presence of anemia, dyslipidemia, hypoalbuminemia, and hyperfibrinogenemia contributed to the development of neurological complications in pregnant women. Conclusion. Diagnosis of cerebral pathology in pregnant women causes significant difficulties. During pregnancy, particular attention should be paid to the such manifestations as headache, nausea and vomiting, which are considered the first symptoms of the pathology of the nervous system. Neurological complications during pregnancy remain an interdisciplinary problem requiring involvement of such specialists as obstetrician-gynecologist, neurologist, ophthalmologist, therapist and neurosurgeon in diagnostics and treatment.

TOPICAL THERAPY OF VAGINITES IN POSTMANOPAUSE

Abstract

Background. The problem of atrophic vaginitis (AV) in postmenopause is widespread; it is considered as a natural aging process and is not currently solved. To ensure the proliferation of stratified squamous epithelium for the formation of vaginal resistance to infection in postmenopause, topical therapy with estrogens is performed. Before restoration of vaginal functions using estrogen preparations, pre-treatment sanitation is reasonable. The appearance of combined preparations with antimicrobial and fungicidal action on the market of requires the development of schemes for their effective application. Objective: to evaluate the clinical efficacy of complex therapy of AV in postmenopausal women, including the use of vaginal cream containing 2% butoconazole, clindamycin 2%, for vaginal use, followed by the use of suppositories with estriol. Methods. The main group (I) consisted of 54 women who were prescribed cream butoconazole+clindamycin 5 g daily intravaginally with an applicator for 6 days and then vaginal suppositories with estriol containing 0.5 mg of estriol, one suppository once a day before bed for 15 days. The comparison group (II) comprised 20 women who received only suppositories with estriol. The evaluation of the effectiveness of the treatment was carried out 2 weeks and 1 month after the treatment according to the results of the clinical and laboratory examination, including evaluation of smears of vaginal secretion from the cervix, and real-time polymerase chain reaction (Femoflor test). Results. Two weeks after the course of the medications, the most significant efficacy of the combined treatment of AV was noted: the acidity of the vaginal contents was restored to the average statistical value (pH 4.5); the white blood cell count in the vaginal secretion decreased more than by 10 times; the number of small rods increased by 2, 3 times; and 9-fold decrease in the representation of cocco-bacillary flora was observed. A month after the end of the combined treatment, the patients were clinically healthy. After monotherapy, 8 (40%) patients had a burning sensation in the vagina, all patients had low acidity of the vaginal contents (within the pH range of 6.1), a small number of lactobacilli (within 35.0±3.7%); representation of microflora remained at a high level due to coccobacillary flora (up to 50%). Conclusion. Pre-treatment application of the drug with a wide range of antimicrobial and fungicidal action allows to prevent the appearance of dysbiotic disorders in restoring the function of the cells of the vaginal mucosa during the treatment of AV in postmenopause. Complex therapy with combined cream for intravaginal use containing butoconazole+clindamycin (Clindacin B Prolong) and suppositories containing therapeutic doses of estriol (Ovipol Klio) serves as a simple and safe therapeutic strategy for AV in the postmenopausal period.

ETIOLOGY OF ENDOMETRITIS AFTER CESAREAN SECTION AND ASSESSMENT OF EFFECTIVENESS OF ANTIBACTERIAL PREPARATIONS IN VITRO

Abstract

Background. For rational antibacterial therapy of postpartum endometritis, it is important to choose the right drug based on the local sensitivity of the pathogen to antibiotics. Objective. Evaluation of the etiological structure of endometritis after cesarean section and identification of the most effective antibacterial drugs based on in vitro studies. Methods. Bacteriological study of metoaspirate with determination of antibiotic susceptibility of obtained isolates in 70 puerperas with postpartum endometritis. Results. The microbiological studies have revealed that conditionally pathogenic microorganisms inhabiting the gastrointestinal tract (enterobacteria and enterococci) were predominantly isolated. Their proportion was 44%. In 15% of cases, the cause of metroendometritis was Staphylococcus aureus and Streptococci. In general, the isolated flora had a low resistance to the antibiotics most often used in maternity hospitals. Only a small number of strains had multiple drug resistance. Conclusion. Beta-lactam antibiotics remain the most effective drug in the treatment of endometritis after cesarean section.

HYALURONIC ACID OR ARGON-PLASMA COAGULATION: WHAT WILL STRENGTHEN THE UTERINE SCAR? EXPERIMENTAL DATA

Abstract

Objective: to compare tissue effects in the wound area on the uterus after applying argon-plasma coagulation (APC) using Fotek ЕА141 and the injection of hyaluronic acid. Methods. An experimental study on animals was carried out. The study involved 30 female rats. On the 14th day after the surgery, scars of the left uterine horn treated with APC (n=15 - group 1) or injection of 1.0 ml hyaluronic acid (n=15 - group 2) were examined. To evaluate the structure of the scar, a histological examination of sections with Van Gieson staining was performed. Control - scars of right uterine horn (n=30). Evaluation of the results of the study was carried out using the developed grading scale of histological changes. Results. Comparison of the methods studied has revealed a more pronounced effect of APC on angiogenesis in the wound area. At the same time, the injection of hyaluronic acid has increased the number of myocytes in the scar. Evaluation of the histological structure of the sections showed that the mean scar score in the group 1 was 10.8±1.28 points, in group 2 - 11.2±1.32, and was better than in the control(8.0±1.16) as expected. Moreover, this pattern was noted both in the whole group and in comparison of the scar on the right and left horns of the uterus in the same individual. Scars in the studied groups were histologically regarded as «small, with a noticeable muscle ingrowth» or as a «scar with a large amount of muscle tissue», whereas in the control group «dense, rough scars» or «scars with a predominant connective tissue» predominated. Conclusion. The positive effects of both methods on myometrium can be used to form a regular scar after surgery on the uterus.

Abstract

Objective: to evaluate the results of the use of dose-intensive neoadjuvant chemotherapy (NACT) with doxorubicin and cisplatin in patients with locally advanced cervical cancer stage IB2-IIB. Methods. In the patients participating in the study, the efficacy and toxi-city of 3 cycles of intravenous dose-intensive NACT according to the AP scheme (cisplatin 75 mg/m2, doxorubicin 35 mg/m2) and subsequent radical surgical intervention in 62 (89%) cases were evaluated. Results. The study involved 70 patients with locally advanced cervical cancer at the age of 27-68 years (mean age, 45 years). Against the background of the treatment, an objective response was received from 58 (83%) patients. Progression of the disease was detected in 6 (8.6%) cases; relapse of the disease was diagnosed in 2 (2.8%) patients. Neoadjuvant therapy allowed to perform radical surgical intervention in 61 (87.1%) patients. The tumor pathomorphological response of various degrees was obtained in 58 (82.8%) cases; complete clinical regression of the tumor was confirmed by a complete pathomorphological response in 5 (7.14%) of these cases. The dose-intensive regimen of chemotherapy did not lead to significant complications at the pharmacological and surgical stages of treatment. Conclusion. Analysis of the study results has showed that dose-intensive NACT is an effective method and can be considered as an alternative to standard treatment of locally advanced cervical cancer.

COMPARISON OF EFFICACY AND SAFETY OF DYNAMICO FORWARD, VIAGRA AND DYNAMICO IN MEN WITH ERECTILE DYSFUNCTION

Abstract

Background. Bioequivalence studies of the of sildenafil orally dissolving films show similar plasma concentrations of the active substance compared to tableted forms of the same preparation [1-3], while there are no comparative studies of the therapeutic equivalence of sildenafil orally dissolving films and tableted forms of this drug. Objective. Evaluation of the clinical efficacy and safety of Dynamico Forward (orally dissolving films) in comparison with Viagra and Dynamico in patients with erectile dysfunction (ED). An additional object was to evaluate the efficacy and safety of Dynamico in comparison with Viagra in patients with ED. Methods. The study involved 120 men with ED aged 20 to 70 years who had a total score of 11-25 in the erectile function domain of the ICEF (International Index of Erectile Function). The treatment lasted 12 weeks. Results. The frequency of positive treatment responses in the study groups at the final visit was 55% in the Viagra group, 75% in the Dynamico group, and 70% in the Dynamic Forward group. In all three groups, a statistically significant increase in the quality of sexual life was recorded at visits 3 and 5 compared to baseline, while Dynamico Forward showed clinical efficacy comparable to that of Dynamico and Viagra. There were no significant differences in the safety and the tolerability of therapy between the groups. Conclusion. The study results confirmed comparable therapeutic efficacy of Dynamico Forward and Dynamico, as well as Dynamico Forward and Viagra, with satisfactory tolerability and safety of the use of all three studied drugs.

PERIPARTUM CARDIOMYOPATHY: 10-YEAR CLINICAL FOLLOW-UP

Abstract

Background. Peripartum cardiomyopathy (PPCM) is one of the most severe forms of extragenital pathology, which often results in a fatal outcome. Actually, PPCM is variant of dilated cardiomyopathy, which develops in the last trimester of pregnancy and in the early postpartum period, leads to the development of congestive heart failure, and can be accompanied by thromboembolic complications and life-threatening rhythm disorders. Description of the clinical case. The article presents a 10-year clinical follow-up in the settings of the cardiological department of the female patient with PPCM, which managed to achieve stabilization of hemodynamics, reduction of the size of the heart cavities, increase in the myocardial contractility. The complexity of diagnosis and selection of adequate therapy are described. Conclusion. Each clinical experience in the management of patients with PPCM seems to be useful for developing a general concept of diagnostic and therapeutic approaches for this disease.

Abstract

The article discusses the effect of vitamin D deficiency on the severity of endometriosis (EM). These data presented indicate the significant role of vitamin D in the reproductive health of women. There are numerous studies confirming an increased risk of developing EM with vitamin D deficiency. Vitamin D can be recommended in the complex treatment of EM, in particular, vitamin D therapy can reduce the dose of non-steroidal anti-inflammatory drugs in the treatment of patients with chronic pelvic pain syndrome. Also, the use of vitamin D can be recommended for the prevention of EM in patients with a burdened history.

ORGANIZATION OF SPECIALIZED CARE FOR PATIENTS WITH POSTPARTUM SEPSIS

The article presents the experience of treating patients with infectious-inflammatory postpartum complications (IIPCs) in the gynecological department of the St. Petersburg Mariinsky City Hospital, whose history history dates back for 40 years. The department employs the fifth generation of doctors who have approaches to diagnosing and treating these serious diseases. Treatment of severe forms of IIPCs, taking into account the increasing resistance of pathogens of the disease, and concomitant extragenital pathology, requires an individual approach, large time, moral and material costs. The average cost of only medicinal provision for a septic patient is 59,254 rubles, not including laboratory and instrumental examinations, stay in intensive care units, etc.

PD-L1 EXPRESSION IN BLADDER CANCER AGAINST THE BACKGROUND OF CHRONIC HERPESVIRUS INFECTION: RESULTS OF A PILOT STUDY

Abstract

Background. Currently, the use of checkpoint inhibitors in the treatment of bladder cancer (BC) is one of the most promising directions. PD-L1 expression in patients with EBV-associated BC is associated with an unfavorable prognosis of the disease. Description of case series. PD-L1 expression in patients with urothelial BC and the presence of viral DNA (cytomegalovirus - CMV, Epstein-Barr virus - EBV, high-risk human papillomavirus - HPV) in tumor tissue was determined in the study. PD-L1 expression of in tumor cells in patients with the presence of viral DNA in tumor tissue >60% was detected in 3 of 8 patients, in immune cells >1% occurred in 9 of 10 patients. Conclusion. Thus, in patients with BC, including non-muscle invasive BC, and with the signs of latent viral infection, especially combined (EBV+CMV+HPV), it is reasonable to determine the PD-L1 expression as a marker of unfavorable course of the tumor process. In this category of patients, treatment with anti-PD-L1 agents is possible in order to increase the disease-free period and cancer-specific survival. However, further research is required.

Abstract

Background. Pelvic inflammatory diseases (PIDs) retain their leading position in the structure of gynecological morbidity and account for 60.4-65% of the entire pathology of the organs of the reproductive system. Currently, particular attention is paid to the evaluation of the effect of intrauterine contraceptives on the development of PIDs. The development of pyometra is especially dangerous. In postmenopausal women, the frequency of purulent complications against the background of long-term use of intrauterine devices (lUDs) is significantly increasing. Description of the clinical case. This article describes the clinical case of pyometra that evolved against the background of a multi-year presence of self-made intrauterine device in the uterine cavity. This case very clearly illustrates the problem of development of purulent pelvic diseases of against the background of long-term and uncontrolled use of lUDs. It is important to inform patients about the possible risks of using IUDs longer than needed, about the risk of purulent-septic complications, especially in postmenopausal women. Conclusion. Gynecologists need to carry out a thorough follow-up of women using IUDs, as well as diagnose PIDs at the proper time, and adequately and competently choose the tactics for treating such patients.
