XI National congress of internal medicine
Abstract
23-25 ноября 2016 г. в Москве в международном выставочном центре «Крокус Экспо» прошел XI Национальный конгресс терапевтов. Как и остальные мероприятия, проводимые Российским научным медицинским обществом терапевтов в 2016 г., конгресс был приурочен к 135-летию со дня рождения выдающегося отечественного клинициста и ученого В.Ф. Зеленина.

Abstract
22-23 сентября 2016 г. в Якутске прошел II Съезд терапевтов Республики Саха (Якутия). Организаторами мероприятия являлись: Российское научное медицинское общество терапевтов (РНМОТ), Министерство здравоохранения РФ, Комитет по социальной политике Совета Федерации, Общество врачей России, Министерство здравоохранения Республики Саха (Якутия; РС(Я)) , Медицинский институт СВФУ им. М.К. Аммосова, Научно-практическое общество терапевтов РС(Я) и Национальное научно-практическое общество скорой медицинской помощи при участии Международного общества внутренней медицины (ISIM) и Европейской федерации внутренней медицины (EFIM). Съезд был приурочен к 135-летию со дня рождения выдающегося отечественного врача и ученого В.Ф. Зеленина.

XV European congress of internal medicine: connecting with the patient
Abstract
2-3 сентября 2016 г. в Амстердаме (Нидерланды) состоялся XV Европейский конгресс терапевтов, прошедший под девизом «Объединяя врача и пациента». Организатором конгресса являлась Европейская федерация внутренней медицины (EFIM). В мероприятии приняли участие более 1300 специалистов из стран Европы и других континентов. Наибольшее количество делегатов представляли Португалию, Турцию и Испанию. Делегация Российского научного медицинского общества терапевтов (РНМОТ) была представлена президентом, академиком РАН А.И. Мартыновым и ученым секретарем В.А. Кокориным.

Expert support for medical decisions in rational use of medicines in the context of polymorbidity
Abstract
Currently, problem of comorbidity and polymorbidity is becoming increasingly urgent in developed countries due to the increase in life expectancy. Rational drug prescribing with account of drug-drug interactions in this category of patients will avoid iatrogeny and polypharmacy and, thus, will optimize therapeutic strategies. Development of interactive systems of expert support for medical decisions in the context of poly- and comorbidity is designed to improve health care efficiency and enhance the level of professional education.

Subclinical hypothyroidism - new cardiovascular risk factor
Abstract
In last years there is discussion about subcLinicaL hypothyroidism (SHT) as a cardiovascular risk factor. We examined 870 patients with coronary artery disease. Height, weight, body mass index, thyroid-stimulating hormone level, lipid profile, homocysteine level were assessed in all patients. The presence of a history of arterial hypertension, diabetes mellitus type 2, and conducted therapy were considered. Elevation of thyroid-stimulating hormone level (so called SHT) has been associated with obesity, diabetes mellitus type 2, arterial hypertension, atherogenic dystipidemia, hyperhomocysteinemia. Multiple coronary lesion significantly higher revealed in patients with SHT. SHT was additional independent risk factor for coronary atherosclerosis.

The evaluation of lipid profile in patients with chronic heart failure and concomitant chronic obstructive pulmonary disease
Abstract
Purpose of study. The revaluation of the main indicators of the Lipid profite in patients with chronic heart faiture (CHF) and concomitant chronic obstructive pulmonary disease (COPD). Materials and methods. The study included 102 patients (58 men and 44 women, mean age 65.7±4.5 years). According to the results of spirometry all patients were divided into 2 groups: the basic group included 54 patients suffering from CHF in conjunction with COPD, the control included 48 patients with CHF without bronchopulmonary pathology. The Lipid profite was determined by the level of the total cholesterol (TC), triglycerides, high density lipoprotein cholesterol (HDL) and low density lipoprotein cholesterol (LDL-C), and the concentration of C-reactive protein (CRP), interteukin-8 and tumor necrosis factor-α. Results. Patients with comorbid disorders had a higher levels of TC and LDL-C, Low HDL-C concentration. According to the result of the correlation analysis we identified the positive correlation relationship between cholesterol, CRP and pro-inftammatory cytokines. Conclusions. The combination of CHF and COPD is associated with the development of the lipid metabolism disorders, manifested as hypercholesterolemia, high LDL and low HDL cholesterol that are most likely caused by activation of systemic inflammatory response.

Oral hypoglycemic therapy in the practice of a general practitioner
Abstract
Review article presents a contemporary view on the application in practice of a general practitioner first-line drugs in the treatment of type 2 diabetes: metformin and a sulfonylurea. Particular attention is paid to the practical application of the latest generation of sulfonamides - glimepiride. Possible combination with other pharmacological agents and antidiabetic drug titration algorithm. Author considers the latest scientific data on the cardiovascular safety of antidiabetic therapy and presents data of new studies that have shown cardioprotective and anti-atherosclerotic properties glimepiride.

Human placenta hydrolyzates in reconstructive therapy of connective tissue of cartilage and joints
Abstract
Treatment of rheumatoid arthritis is one of promising directions for the clinical application of Laennec and other preparations based on human placenta hydrolysates (HPHs). It is shown that HPH peptide components can contribute to a significant acceleration of recovery of connective tissue due to antiinflammatory and trophic actions. The results of experimental studies of HPH-containing medications (including Laennec) for the treatment of arthritis models are presented. The prospects for clinical applications of HPH-containing preparations for the treatment of arthritis are discussed.

ANCA-associated systemic vasculitis in clinical practice
Abstract
Diagnosis of ANCA-associated vasculitis including lung and kidneys involvement, and laboratory signs of inflammation poses difficulty. The presence of anti-neutrophil cytoplasmic antibodies (ANCA) and the results of histological investigation confirm diagnosis of ANCA-associated vasculitis. Cyclophosphamide/ azathioprine in combination with a high dose of steroids are usually used to induce and maintain remission of systemic vasculitis. The clinical trials also showed efficacy of rituximab. The article tackles a clinical case of microscopic polyangiitis.

Optimization of antibacterial therapy in the treatment of acute bacterial rhinosinusitis
Abstract
The paper summarizes data on the prevalence of major pathogens of upper respiratory tract infection. Taking into account the recent recommendations formulated the basic principles of modern antibiotic policy in relation to these patients. The place of amoxicillin cfavufanate antibiotic therapy algorithm in patients with acute bacterial rhinosinusitis is described. It is shown that a dispersible form of the drug is comparable to conventional form of amoxicillin clavulanate in bioequivafence, and is characterized by improved pharmacokinetic parameters.

Hypothyroidism: initiation of replacement therapy
Abstract
Primary hypothyroidism is one of the most common endocrine diseases. Treatment of symptomatic hypothyroidism involves the appointment of replacement monotherapy with levothyroxine that maintains the disease compensation. The article discusses the issue of initiation of replacement therapy of hypothyroidism for different groups of patients.

Clinical efficacy of fixed-dose combination antihypertensive therapy (focus on Hypotef)
Abstract
It is now known that arterial hypertension can lead to fatal complications such as myocardial infarction, stroke, and progression of renal failure up to the terminal stage. Adherence to antihypertensive therapy and achievement of target blood pressure values still remain an important problem of practical cardiology. This article presents the results of a multicenter study on the use of domestic fixed-dose combined antihypertensive drug Hypotef in patients with arterial hypertension of 1-2 degrees. The results of the study indicate a high antihypertensive activity of Hypotef that is similar to the efficacy of enalapril 20 mg/day and metoprolol tartrate 50 mg/day (± indapamide 2.5 mg/day).

Comparative evaluation of chronic iron overload in the application of iron preparations in the sub-toxic doses
Abstract
Iron supplements used for the treatment of iron deficiency anemia differ in their safety. Untike organic iron salts, inorganic iron compounds such as oxides, hydroxides and sulfates do not have high bioavailability and significantly overload tissues with insoluble iron oxides. In this paper, an experimental study of iron supplementation on the basis of iron hydroxides in the composition polymaltose complex (Maltofer®) and iron sulfate is reported. Oral administration of iron sulfate for 2 months induced chronic iron overload, reflect both by histological damage by hemosiderin in parenchyma of liver, kidneys and brain, by an increase of ALT in blood and of protein levels in the urine. In comparison with ferrous sulfate, Maltofer has advantages for chronic use: hemosiderin deposits were absent in brain and in kidney tissue; small deposits of hemosiderin in the liver were found only in intralobular sinus capillaries, but not in hepatocytes.
