Abstract

The review article present an analysis of conceptual and methodological tendencies in working up biliary ducts iatrogenic injuries problem. On the basis of using traditional and modern methods presented rational diagnostic algorithm, determinated optimized surgical tactics necessity.

Immediate and long-term treatment results of patients with biliary ducts iatrogenic injuries confirm the Fedorov’s clinic conceptual positions, accentuated necessity of combination principles biliary surgery and modern medical technologies abilities.

Investigation of the anti- hypoxic properties of reamberine

Abstract

The antihypoxic effect of reamberine was shown on the model of rat sharp histotoxic hypoxia produced by the respiration blocker sodium fluoride. This effect was more evident as compared with GOBA. The anti- ischemic activity of reamberine was displayed on the early stage of myocardium ischemia. In the model of the liver toxic damage the hepatoprotective action of reamberine was found out. It may be compared with carsil.

Molecular and genetic mechanisms of atherosclerosis and ischemic heart disease

Abstract

We genotyped polymorphisms from candidate genes in a prospective cohort of male patients (<=45 years old) for association with early-onset myocardial infarction in a case-control study (study 279 cases, 203 con­trols).

We tested single-nucleotide polymorphisms -455 G/A in fibrinogen gene, 4G/5G (-675 position) in plas- minogene activator inhibitor 1 gene, A1166C in gene of type 1 receptor of angiotensine II, G-6A in angiotensinogen gene, Sst I in apolipoprotein CIII gene, l/D polymorphism in angiotensin-converting enzyme gene, restrictive polymorphism in apolipoprotein E gene, PIA2 in platelet llb/llla receptor gene.

Results show that the PIA2 allele of platelet glycoprotein receptor llb/llla is a risk factor of young myocar­dial infarction in males and that carrying of PIA2/A2 genotype is a risk factor of sudden death because of thrombosis of coronary artery.

Neoadjuvant endocrine therapy versus chemotherapy in patients with hormone dependent (ER+) breast cancer

Abstract

This randomized controlled phase II study evaluated the efficacy of neoadjuvant chemotherapy (doxoru­bicin + paclitaxel) compared with endocrine treatment with aromatase inhibitors (anastrazole or exemestane) in postmenopausal women with ER-positive and/or PgR-positive breast cancer. Preoperative neoadjuvant endocrine therapy with aromatase inhibitors was well tolerated and resulted in similar rates of overall objec­tive response and breast-conserving surgery as chemotherapy in postmenopausal women with ER-positive and/or PgR-positive tumors.

Rehabilitation of children with attention-deficit/hyperactivity disorder based on consensual self-regulation under EEG-acoustic feedback

Abstract

Procedures of involuntary self-regulation under EEG- acoustic feedback of the brain functional condition were used for rehabilitation of children with attention-deficit/hyperactivity disorder. Decrease of hemispheric asymmetry and normalization of EEG rhythmic structure were revealed after treatment. The reorganization of EEG has been accompanied with reduction of diagnostic symptoms numbers and with promotion of attention function. The data point on high effectiveness of involuntary self-regulation under EEG- acoustic biofeedback for treatment of children with hyperactivity combined with attention deficit and feebleness of will.

Description of multidrugresistant Mycobacterium tuberculosis (MT) strains in Karelia region

Abstract

243 isolates of Mycobacterium tuberculosis (116 - drug-sensitive (DS), 127 - multidrug-resistant (MDR)) were investigated. 44 isolates from newly diagnosed patients (13 - DS, 31 - MDR) were genotyped. Fast (less than 30 days) and massive (more than 100 colonies) growth was 10 times more prevalent in MDR than DS strains. The Beijing genotype stains prevailed (about 90%) among MDR isolates. 92,3% of DS isolates had non-Beijing genotypes. The data obtained proved the epidemic spread of MDR-ТВ in Karelia.

Polychlorbiphenyls and public health in Irkutsk Region

Abstract

Data on the pollution of abiotic (air, water, soil) and biotic (food, breast milk) objects of the environment of the Irkutsk Region with PCB are presented in the investigation. The daily intake of PCB was calculated and carcinogenic and noncarcinogenic risks for the population of the region were assessed on the base of data obtained. It was found that HQ from PCB impact amount 0. 84 in average and the individual carcinogenic risk comes to 3. 4*10-5 that correspond to 34 cases of cancer among 1 000 000 people. The highest HQ and risk were found for the population of Usol’e-Sibirskoe and for families of fishers living along the shore of Lake Baikal and especially along the Angara River. New-borns being on natural alimentation are the group of the risk too. In keeping with the medical statistic levels of morbidity are higher in some areas of the region where people from the group of the risk live than the average morbidity for total region. Research has shown PCBs to be an important contaminant in the Lake Baikal Region and in Lake Baikal that may be harming human.

Abstract

Sergey Petrovich Botkin is a famous clinician and teacher, the founder of the national scientific medicine, the creator of the fist Russian and one of the earliest world scientific therapeutic schools. Upon Botkin’s initiative new clinics of infectious diseases, dermatology and venerology diseases, otorinolaringology were founded. Botkin was the outstanding public statesmen, the chief of Russian medical society, one of the initiators of women high medical education in Russia. The article is dedicated to 175 years birthday of S. R Botkin.
