To the treatment of submucosal and interstitial fibrous tumors of the uterus in the postpartum period
Abstract
A significant number of works, both in foreign and domestic literature, are devoted to the treatment of fibrous intrauterine tumors during pregnancy and childbirth. I will point out here only some - Berlin), S. Braun), Hellwig), Ish.-Wall), Kashkarov), Pozzi), V. V. Sutugin), Baissent), Duret), Flgischlen) and many others. At the same time, regarding fibrous and intrauterine tumors in general, complicating the postpartum period of a woman, this cannot be said - they are still waiting for their turn in the sense of a more detailed elaboration of the issue.
Two cases of vaginal stenosis
Abstract
Acquired cicatricial narrowing of the vagina, which is quite rare, is of great interest in the therapeutic relationship for the obstetrician and gynecologist. What a serious obstacle is vaginal stenosis for a pregnant uterus during childbirth - can be seen from the article by Merz, posted in Archif für Gynäcologie, Bd XLV: for 230 cases of rupture of the pregnant uterus collected from the literature over the last 20 years, 16 caused vaginal stenosis. Timely removal of this obstacle would save 16 lives! It must be assumed that a huge number of cases, in addition, remain unpublished; in November last year (1893), together with my friend Dr. Bobkevich, I was a witness of the lethal outcome of urgent childbirth of a woman who had a second birth in the result of scarring contraction of the vagina.
A very rare case of multiple births
Abstract
I. A multiple or complex pregnancy is a condition of a woman in which two or more fetuses develop in the cavity of her uterus and, depending on the number of postpartum pregnancies, it is called a pregnancy of twins, triplets, etc. the Largest number of fetuses that a woman can be pregnant is limited to five. More than five fruits have not yet been lost. Even Aristotle says that there is no pregnancy with more than five fetuses. Most often it is necessary to observe pregnancy with twins, pregnancy with triplets is observed much earlier. Cases in which the uterus contained only four and five fetuses belong to all extremely rare exceptions, especially five fetuses.

Abstract
Patient 67 years old, does not wear blood since 50 years of age, suffers from 5 years of umbilical hernia and progressive enlargement of the abdomen. The hernia cannot be adjusted, the abdomen is greatly increased in volume. Above the hernia, the sound, when tapped, tympanic, with the sides - dull, passing at a distance of 4 fingers from the spine, in tympanic; below the hernia, the sound is dull. In the area of a dull sound, there is a ripple.

Abstract
The author is skeptical about the fear of many modern surgeons who perform gluttony, the air and everything that is in the operating room at the time of the operation, although there is no contact with the wound controversy. In his opinion, the care of some surgeons to arrange their gluttons to the extreme strictly, in relation to the environment, is unnecessary. He himself operates in a private house. Abstract
The forensic medical significance of the fainting of women in labor arose due to the fact that women accused of causing death to their newborn baby very often point to the fainting state in which they were at the time of delivery, or immediately after them. The author, on the basis of historical and literary data and his own observations, tries, as far as possible, to find out the grounds that the expert should be guided by when resolving questions about fainting in childbirth.

Abstract
The act of childbirth, even under the most favorable conditions, puts the woman in labor in a position close to pathological. If a woman in labor has a pathological inheritance, a personal neuropathic predisposition, and on the part of childbirth, there is any deviation from the norm, then the physical and mental state of the woman in labor cannot remain in the physiological limits. Cases of crimes during childbirth were noted by many.

Abstract
24 / ix, the author, invited to the primiparous about the prolonged labor, finished labor with forceps. The child's place has partially grown to the uterine wall; with manual detachment, its accreted part of the membranes remained in the uterus; Successive attempts by two doctors to separate these remnants have been in vain. Abstract
In March 1891, a patient, 33 years old, married, did not give birth, turned to the author with symptoms of cystitis. The undertaken treatment was of little use, on the contrary, soon there were even symptoms of a disease of the right kidney. Since the patient also had gynecological suffering, she was sent to the Moscow University Gynecological Clinic, where, according to the patient's examination, pyonephritis et salpyngo-oophoritis duplex was suggested. In May of the same year, it was performed by the patient prof. Snegirevym removal of the kidney and uterine appendages (the ovaries were found cystically degenerated, and the common tubes were calcified, thickened and adhered to the surrounding tissues).

Abstract
The author describes 2 cases of bleeding during pregnancy (5 months and 4 months), where, after unsuccessful attempts to stop the bleeding, he miscarried according to the method proposed by Peltzer. This method, as is known, consists in the fact that glycerin is poured through a catheter inserted a few centimeters between the walls of the uterus and the membranes. (Glycerin, having properties to irritate the mucous membrane of the uterus, acts more surely than water).

Abstract
In November 1892, a woman in labor was admitted to the Kazan Obstetric Clinic, V-para, with t ° 39.1 °. The abdomen is swollen, very sensitive when touched, in the lower parts of the abdomen crepitus; the fundus of the uterus of the finger 2 from the xiphoid process; contracting ring on the finger below the navel, above the entrance to the pelvis head; huge swelling of the external genital organs; from the sleeve, a sanguine liquid is released, with a putrid smell; the heartbeat of the fetus is not audible. Sizes of the pelvis: dist. spin. 24, dist. crist. 26.5, dist. troch. 29, conjug. ext. 18, conjug. diagon. 9.5. Complete opening of the os, presenting the head, descending only in a small segment beyond the entrance of the pelvis. Strong caput succedaneum. The intervals between attempts are 5-10 minutes.

Abstract
The description of the case seen by the author is preceded by an indication of the reasons for suppuration, in general. The established opinion that suppuration is caused by specific microorganisms (staphylococcus pyogenes aureus et albus and streptococcus pyogenes) and that where suppuration is present, the named microorganisms must be found there, is now abandoned.

Abstract
If after the birth of the head, the essential requirements of the correct genital mechanism are present on the face, that is, the largest dimensions of the torso (shoulders) coincide with the largest dimensions of the genital canal, there is also sufficient expelling activity, natural or artificial, and the birth still does not stop, then all natural on the assumption of the existence of a spatial inconsistency between the birth canal and the fetus. The fruit in such cases dies pretty soon and therefore the fruit-destructive operation in one or another form is the most expedient medical aid.

Abstract
The weakness of pushing, which is encountered so often in the practical activity of obstetricians and leading not very often to serious surgical intervention, which does not always end happily for the mother and the baby, made the more innocent people look for ways to fight it, than the operation. All currently available means of enhancing labor pains are not always effective, therefore it is completely understandable and desirable to search for new more effective means. The author, wanting to try any of the newly proposed remedies with the weakness of the labor pains, settled on a corn smut.

Abstract
The author describes ten cases of chronic inflammatory processes in the appendages, in which he removed these appendages. Indications were used in every case of local pain and general nervous phenomena, which were not inferior to any therapeutic effects before.

Abstract
In 1893 and the beginning of 1894, the author observed five cases: 1) A peasant woman, 33 years old, married, giving birth, 3 months without blood. Attempts at an artificial miscarriage (midwife) caused a slight bleeding. The author, having identified a lingering tumor on the left near the uterus and assuming an extrauterine pregnancy, pierced the tumor through the vagina with a thin troocar and released about a teaspoon of liquid.

Abstract
In the second-bearing, 22 years old, with a wide pelvis, the transverse position of the fetus with the handle falling out. Rotate and extract with a successful outcome. After childbirth, it was found that the postpartum woman had a clear form of a two-horned uterus (uterus bicornis, arcuatus).

Abstract
The question touched upon by the author is interesting for institutions that have a need for newborn babies. Congenital syphilis does not always manifest itself immediately after birth, sometimes after a week, sometimes even after seven months. During this period of the latent state of the disease, a syphilitic child can easily infect a healthy wet nurse.

Abstract
Data on this issue, obtained by thirty-four authors, are collected in this article and developed. These data concern 64,759 cases. According to the conclusions made by the author, the first months of the inhabitants of Russia come on average in 15.83; most often at the age of 16, (216% of all cases).

Abstract
The number of gynecological patients is 293. Some of them were sent by zemstvo doctors of Tambov province. for special treatment 38. Age predominantly 21-30 years. 116 women had diseases of the uterus (mainly neoplasms and inflammatory processes), 90 - diseases of the appendages, 56 - external genital organs and vagina. Died 17 (5.8%). Perineal ruptures were according to L. Tait'y with modifications of Slavyanskiy. One of these cases ended in septic parametritis, with a fatal outcome, as a result of leakage from the rectum into the tissue of fecal fluid through the former ulcer (?).

Abstract
Multiparous, 43 years old. The last three pregnancies ended in miscarriage. Suffering from bleeding. Recognition of backward bend, chronic metritis and endometritis. After an unsuccessful treatment with massage and rings, hysteropexia abdominalis anterior was made. After 81/2 months after the operation, a three-month miscarriage. For several more than a year after the operation, its effect is not violated.

Abstract
Anatomical studies of Eckhard, Roehrig, and Winkler in the mammary gland have established vasomotor and nerves supplying the muscular walls of the milk ducts, sinus and excretory ducts; secretory nerves were not found. Physiological experiments of the same authors, as well as Laffont, De-Sinety, Partscha, Hammerbacher, gave diametrically opposite results. The scheme of the innervation of the mammary gland in a goat, according to the author, is as follows: the main nerve trunk - item spermaticus externus - enters the gland from the outside.

Abstract
9-para, 29 years old. Childbirth ended with forceps (slightly flat pelvis). During childbirth, who were at home, a rupture of the uterus was ascertained. I arrived at the hospital with an unremoved post-dom, very weak, with the manifestations of the beginning inflammation of the peritoneum.

Abstract
Married, nulliparous, 35 years old, fell ill after severe traumatic injury. Nearby with a retroperitoneal tumor (partly interconnective in the direction of the left broad ligament), there was a whole series of common nervous disorders that went parallel with the decline in nutrition and exhaustion due to blood loss.

Abstract
Making a brief outline of the pathology of narrowing of the cervical drip, the author points out the narrowing of the external opening, as a common cause (96%) of infertility in women and as a cause, together with inflammatory changes in the neck, painful months. When reducing the contraction, the author often resorts to cutting the neck (Sims) and praises this method.

Abstract
In a woman, 32 years old, at 5 months of pregnancy after a miscarriage, an eversion of the uterus was found. Treatment is expectant (disinfecting vaginal washing). After a few days from the fundus of the uterus, a tumor (myoma) separated from a hen's egg (not recognized earlier) and an arbitrary reduction of the uterus occurred.

Abstract
In 28 years of healthy primiparous in 9 months of pregnancy, due to the strong, due to the presentation of the child's place, the bleeding was reduced when only 2 fingers were passable. The operation was performed on a transverse bed under chloroform anesthesia, which was immediately stopped after the leg was captured. When this last woman was taken to the entrance to the vagina, the woman in labor felt pain and strained herself greatly, and a stream of amniotic fluid with colored blood poured out of the vagina.

At the sight of postpartum hemorrhage, the doctor is usually so lost that he does not stop at the choice of means to stop it, often exposing the woman in labor in this way to the danger of infection, while the danger of further bleeding or of the whole does not exist, or can easily be eliminated by a more rational way. Truly, if you do not act rashly, but rather coolly to navigate every time unto the state of affairs, it is very easy to do completely aseptic measures, it is not resorting to such a dangerous manipulation, as manual separation of the placenta, the introduction of full hands unto the uterine cavity or even tamponato by iodine-phorme gauze. The cause of bleeding in the postpartum period is, as usual, either the injury of the soft parts, or the atony of the uterus.

In the child's place, there are changes in both the vascular apparatus and the connective tissue skeleton, while suffering prevails in varying degrees, then one or the other of the constituent parts (endo-and peri-arteritis, phlebitis, cellular proliferation of the connective tissue skeleton development of connective tissue and without it). These changes are either scattered throughout the placenta, developing independently under the influence of syphilis, nephritis and endometritis of the mother, or they have a local character, being a consequence of local hemorrhages and heart attacks.

The skepticism expressed by many manuals and practitioners regarding the success of the treatment of chronic metritis is based, as it seems to me, not on the fact that this kind of suffering is not at all given to treatment, but on the difficulties of the task that the doctor has to solve. The best here should be very long, (for which there is not enough patience on the part of the patients, as well as the doctor!) not only symptomatic, but strictly thought out for each individual case, representing its own individual characteristics, which have to be considered.
Two cases of extrauterine pregnancy operated on in the Saratov city hospital
Abstract
Although almost every day in the literature there are accumulated cases of extrauterine pregnancy, which were operated with excellent outcomes, but to this day, ectopic pregnancy does not cease to be of particular interest to practitioners, not only for the excellent results of surgical intervention, but mainly, for difficulties diagnosis and in its topographic diversity, in view of which in the following I will cite two stories of patients with ectopic pregnancy, operated by me in the Saratov city hospital in 1893/4.
