Two cases of vaginal stenosis

Abstract

Acquired cicatricial narrowing of the vagina, which is quite rare, is of great interest in the therapeutic relationship for the obstetrician and gynecologist. What a serious obstacle is vaginal stenosis for a pregnant uterus during childbirth - can be seen from the article by Merz, posted in Archif für Gynäcologie, Bd XLV: for 230 cases of rupture of the pregnant uterus collected from the literature over the last 20 years, 16 caused vaginal stenosis. Timely removal of this obstacle would save 16 lives! It must be assumed that a huge number of cases, in addition, remain unpublished; in November last year (1893), together with my friend Dr. Bobkevich, I was a witness of the lethal outcome of urgent childbirth of a woman who had a second birth in the result of scarring contraction of the vagina.

A very rare case of multiple births

Abstract

I. A multiple or complex pregnancy is a condition of a woman in which two or more fetuses develop in the cavity of her uterus and, depending on the number of postpartum pregnancies, it is called a pregnancy of twins, triplets, etc. the Largest number of fetuses that a woman can be pregnant is limited to five. More than five fruits have not yet been lost. Even Aristotle says that there is no pregnancy with more than five fetuses. Most often it is necessary to observe pregnancy with twins, pregnancy with triplets is observed much earlier. Cases in which the uterus contained only four and five fetuses belong to all extremely rare exceptions, especially five fetuses.

Ginzburg. Artificial legal miscarriage according to the Peltzer method in case of severe bleeding in pregnant women. (Weekly, 1894, No. 10)

The number of gynecological patients is 293. Some of them were sent by zemstvo doctors of Tambov province. for special treatment 38. Age predominantly 21-30 years. 116 women had diseases of the uterus (mainly neoplasms and inflammatory processes), 90 - diseases of the appendages, 56 - external genital organs and vagina. Died 17 (5.8%). Perineal ruptures were according to L. Tait'y with modifications of Slavyanskiy. One of these cases ended in septic parametritis, with a fatal outcome, as a result of leakage from the rectum into the tissue of fecal fluid through the former ulcer (?).

The cause of bleeding in the postpartum period is, as usual, either the injury of the soft parts, or the atony of the uterus.

On the question of the treatment of metritis chronica

Abstract

The skepticism expressed by many manuals and practitioners regarding the success of the treatment of chronic metritis is based, as it seems to me, not on the fact that this kind of suffering is not at all given to treatment, but on the difficulties of the task that the doctor has to solve. The best here should be very long, (for which there is not enough patience on the part of the patients, as well as the doctor!) not only symptomatic, but strictly thought out for each individual case, representing its own individual characteristics, which have to be considered.

Although almost every day in the literature there are accumulated cases of extrauterine pregnancy, which were operated with excellent outcomes, but to this day, ectopic pregnancy does not cease to be of particular interest to practitioners, not only for the excellent results of surgical intervention, but mainly, for difficulties diagnosis and in its topographic diversity, in view of which in the following I will cite two stories of patients with ectopic pregnancy, operated by me in the Saratov city hospital in 1893/4.
