CHARACTERISTICS AND TRENDS OF PRIMARY DRUG RESISTANCE OF MYCOBACTERIUM TUBERCULOSIS IN RYAZAN REGION

Abstract

The article is devoted to drug-resistant tuberculosis in the Ryazan region. There is a tendency to the increase of the amount of tubercular patients among women of middle ages. It is stated that in the period from 1999 to 2007 mycobacteria’s structure of drug-resistance has not changed considerably. The exception was a dramatic increase of cases with simultaneous drug-resistance to the four first-line antibiotics. The direct interdependence between the character of drug-resistance and the speed and intensity of tuberculosis microorganisms growth has not been found yet.

The pharmacoepidemiological research of medicines supply for asthmatic patients with the influence of the demographic characteristics of patients and a territorial factor was made. The Defined Daily Dose methodology was used. It was determined more intensive consumption of asthma controller therapies in the regional center and more intensive use of symptomatic therapy in the regional districts. Differences in using anti-asthmatic drugs between groups of patients differentiated by sex and age were also found.

EXPERIMENTAL VALIDATION OF A GEL BASED ON CHITOSAN ACETATE FOR THE TREATMENT OF BURNS

Abstract

In the work in vivo test of the gel based on chitosan with additions of miramistin, methyluracil and ascorbic acid were performed. The gel was prepared by solution of 2% chitosan in 0.5% acetic acid during 24h with the subsequent filtration through the glass filter with the average pore size. To obtain the modified gels the above mentioned substances were added to the solution in the proper concentrations. In vivo investigations were conducted using laboratory male rats with the modeled burns of IIB degree, which were randomized in the control and experimental groups. The last 3 days after application of burns, twice a day the wound surface was treated with different modifications of chitosan-based gels. Histological study has shown the decrease of inflammation reaction and edema in biopsy material, acceleration of epithelization on the wound surface and decrease of wound repair terms by 2-3 days. Microbiological investigation revealed the high bacteriostatic action of chitosan gel against the most species of burn surface microflora, which allowed avoiding infectious complications.

FINASTERIDE INFLUENCE ON P-GLYCOPROTEIN FUNCTIONAL ACTIVITY

Abstract

In the research on rabbits influence of finasteride on the functional activity of the protein efflux transporter P-glycoprotein (Pgp) was studied. Pgp activity was assessed by pharmacokinetic analysis of it’s marker substrate fexofenadine. It was found that the 14-days course of finasteride administration leads to tissue-specific induction of Pgp functional activity in liver and kidney.

Abstract

Serum matrix metalloproteinase-9 (MMP-9) was assayed at 3-4 days after the first acute myocardial infarction symptoms onset in 36 patients (40-72 years old, 83% male, 89% ST-elevation). Serum concentration of MMP-9 had association with neither sizes nor function of the left ventricular of the heart (LV) assessed by echocardiography performed during hospital admission and before discharge. In contrast, anterior infarction, higher arterial blood pressure during index admission and wall motion index score (WMIS) turned to have positive correlation with LV sizes.

ON TIMELY DETECTION OF ACUTE PYELONEPHRITIS PATIENTS WITH DIABETES

Abstract

The paper analyzes data for early detection of acute pyelonephritis in patients with diabetes. The study was conducted from 2006 to 2011 GG The study group included 149 patients with UCP. During the analyzed period (2007-2011 years) was an increase in frequency of the isolation of microbial inoculation microflora from 47.8 to 53.4%. Predominant pathogen infections of the upper urinary tract in patients with diabetes was E.coli - 80-85%, enterococci isolated in 5-6,6%, Pseudomonas, (primarily P. Aeroginosa) at 1.7-3% Streptococcus - 0,7-2,2%.

STUDY OF SYSTEMIC INFLAMMATION IN PATIENTS BRONCHIAL ASTHMA IN COMBINATION WITH METABOLIC SYNDROME

Abstract

In order to study the systemic inflammation in patients with associated pathology a survey included 48 patients with bronchial asthma, 34 of them were diagnosed with metabolic syndrome, had been performed. In patients with bronchial asthma in association with metabolic syndrome systemic inflammation was more expressed in comparison with patients without metabolic syndrome. An inverse relation between the parameters of respiratory function and C-reactive protein level had been revealed, and it’s concentration increased with the degree of abdominal obesity, severity of atherogenic disorders, fasting glucose levels, with the presence of type 2 diabetes.

A CONDITION OF CONNECTIVE TISSUE AND A BLOOD FLOW IN THE SPERMATIC CORD IN MEN WITH INGUINAL HERNIAS, AS FACTORS OF SURGICAL TACTICS OF TREATMENT

Abstract

The paper studies the influence of severity of undifferentiated connective tissue dysplasia and changes in blood flow through the vessels of the spermatic cord in 36 men with inguinal hernias who underwent open Preperitoneal hernioplasty with “PROLENE” mesh. The average term of life with hernia was 21,5 months. Symptoms of connective tissue dysplasia revealed in 35 (97,2%) patients. The average speed of blood flow decreases from 11,9 cm/s in patients with mild UCTD to 9 cm/s - with a severe and shortage of blood flow increased from 12,5% to 25%. The difference between average speed of blood flow on the affected and healthy side, which before surgery was 2,78 cm/s, decreased to 1,0 cm/s, i.e. almost three times. Deficiency of blood flow before the herniotomy was 19,62%, after surgical treatment - 5,45%. Tension methods of hernioplasty cause a persistent decrease in average speed of blood flow on the affected side, even after several years.

PECULIARITIES OF DIAGNOSTICS AND TREATMENT POSTTRAUMATIC HEMOBILIA

Abstract

The article describes the exploration and currency of posttraumatic hemobilia in the surgical department. The frequency of the disease is 0,18 - 3%. The lethality in case of hemobilia is still high (20-40%) in spite of the new diagnostic and treatment methods usage. The concrete clinic example demonstrates probable mistakes in diagnostics and possible methods to liquidate them; the algorithm of treatment is defined. The article describes further ways of posttraumatic hemobilia diagnostic improvement in the conditions of the surgery department in Russian Federation. The method of selective angiography should be installed necessarily as an invasive method of diagnostics and treatment of these conditions in the stationary of the 21 st century.
